Rational treatment of addiction.

Treatment of alcohol and drug addictions, which has been neglected medically for a long time, is currently sparked with optimism. Craving for alcohol can be treated with two newly registered drugs: naltrexone and acamprosate. New approaches to symptom relief during detoxification or during maintenance therapies are rationally based on experimental and clinical work. It is now clear that addictive drugs are surrogates of natural substances involved in the 'reward system'.

Review article: pharmacotherapy for alcohol dependence - the why, the what and the wherefore.

BACKGROUND: The development of alcohol dependence is associated with significant morbidity and mortality. For the majority of affected people the most appropriate goal, in terms of drinking behaviour, is abstinence from alcohol. Psychosocial intervention is the mainstay of the treatment but adjuvant pharmacotherapy is also available and its use recommended. AIM: To provide an updated analysis of current and potential pharmacotherapeutic options for the management of alcohol dependence. In addition, factors predictive of therapeutic outcome, including compliance and pharmacogenetics, and the c

Pharmacological Treatment of Bipolar Disorder with Comorbid Alcohol Use Disorder.

Bipolar disorder (BD) spectrum and alcohol use disorders (AUDs) commonly occur together. Comorbidity between the two conditions predisposes patients to elevated risks of adverse outcomes, including hospitalization and suicide, compared with either condition alone. Despite the consistent relationship observed between BD and AUD, the underlying cause remains incompletely characterized. Few trials conducted have been able to identify promising interventions for patients with these disease states. The antipsychotic quetiapine has been evaluated most commonly as a therapeutic agent for patients wit

Nalmefene. Alcohol dependence: no advance.

Alcohol dependence is a chronic, severe and sometimes fatal disease. Psychological and social support is a crucial element of patient management. Acamprosate and naltrexone are the drugs of choice to help patients remain abstinent, but they are only moderately effective. Nalmefene, an opioid receptor antagonist related to naltrexone, has been authorised in the European Union to help alcohol-dependent patients reduce their alcohol consumption. Nalmefene has not been compared with naltrexone or acamprosate in clinical trials. Clinical evaluation is mainly based on two double-blind, randomised, p

Repurposing drugs for treatment of alcohol use disorder.

Repurposing drugs for the treatment of alcohol dependence involves the use of drugs that were initially developed for other conditions, but have shown promise in reducing alcohol use or preventing relapse. This approach can offer a more cost-effective and time-efficient alternative to developing new drugs from scratch. Currently approved medications for alcohol use disorder (AUD) include acamprosate, disulfiram, naltrexone, nalmefene, baclofen, and sodium oxybate. Acamprosate was developed specifically for AUD, while disulfiram's alcohol-deterrent effects were discovered incidentally. Naltrexo

[Depression in alcohol addicted patients].

In this review, authors considered characteristics of depression and alcohol addiction. Depressive disorders comorbid to alcoholism are heterogeneous in their nature and structure and can be observed during alcohol withdrawal syndrome and in remission. Antidepressants, and first of all selective serotonin reuptake inhibitors (SSRIs), along with the specific anti-relapse drugs (disulfiram, acamprosate and naltrexone), are the drugs of choice in treatment of alcohol addiction. Efficient combinations of pharmacotherapy and psychotherapy can improve outcomes of both comorbid disorders.

Self-efficacy change as a mediator of associations between therapeutic bond and one-year outcomes in treatments for alcohol dependence.

Empirically-supported treatments for alcohol dependence exist, yet understanding of influences contributing to the intended behavior change is limited. The current study, a secondary analysis of the recent multisite COMBINE trial (The COMBINE Study Research Group, 2003), tested a mediational model wherein change in client self-efficacy for abstinence was examined as a potential mediator of associations between client report of the therapeutic bond and one-year outcomes of drinking frequency, drinking consequences, and psychiatric functioning. For analyses, the 1383 COMBINE trial participants w

Practical considerations of new drugs: new choices for old problems.

As drug therapy experts, pharmacists are often called upon for facts and opinions about recently introduced drugs. New drugs are introduced at a rate that exceeds the reading time of many busy practitioners. This article is intended to provide an objective information and broad-based discussion about new drug options that have particular relevance for the elderly. Drugs that will be reviewed include: acamprosate, darifenacin, eszopiclone, exenatide, ibandronate, nitazoxanide, pramlintide, pregabalin, ramelteon, tigecycline, tinidazole, trospium, and solifenacin.

Research advances in the understanding and treatment of addiction.

The likelihood of developing an addictive disorder is a function of multiple simultaneous, pharmacologic, genetic, and environmental variables, such as the pharmacology of the drug, the route of administration, the personality of the individual, and the availability of the drug. There is increasing evidence that long-lasting changes in the brain result from the progression of casual user to addict. Moreover, the course of the progression of addiction is similar to that of other chronic diseases. In this paper, strategies for effective treatment of addiction, including long-term relapse managem

A Bayesian mixed-treatment comparison meta-analysis of treatments for alcohol dependence and implications for planning future trials.

BACKGROUND: Several treatments for alcohol dependence have been tested in randomized controlled trials, giving rise to systematic reviews with a network of evidence structure, or mixed treatment comparisons (MTCs). Within the network, there are few direct comparisons of active treatments. Thus far, this network has not been adequately analyzed. For example, "indirect comparisons" between treatments (e.g., the comparison of treatments B:C obtained via estimates from A:B and A:C trials) have not been incorporated into estimates of treatment effects. This has implications for the planning of futu

Therapeutic drug monitoring for drugs used in the treatment of substance-related disorders: literature review using a therapeutic drug monitoring appropriateness rating scale.

BACKGROUND: The efficacy of drugs for the treatment of substance-related disorders is moderate at best. Therapeutic drug monitoring (TDM) could be an instrument to improve outcomes. Because TDM for most of those drugs is not established, the authors reviewed the literature and built a rating scale to detect the potential added value of TDM for these pharmacologic agents. METHODS: A literature search was performed for acamprosate, bupropion, buprenorphine, clomethiazole, disulfiram, methadone, naltrexone, and varenicline. The rating scale included 22 items and was divided in five categories: e

Review of Evidence-Based Strategies to Treat Alcohol Use Disorder.

Alcohol use disorder (AUD) is a serious, prevalent disorder that affects millions of people. There are numerous evidence-based treatments and strategies to treat AUD, but they are under-utilized for a variety of reasons, including provider stigma, lack of knowledge, lack of professional support, shortage of willing providers, and patient barriers. Disulfiram, naltrexone, and acamprosate are approved but underused medications for the treatment of AUD. Nonpharmacological strategies and treatments include the use of motivational interviewing when talking to patients about their alcohol use, peer

Patient perspectives on medications for alcohol use disorder: A systematic scoping review.

Front-line treatments for alcohol use disorder (AUD) include psychotherapy and medication, and both treatments are underused. However, utilization rates of medications for alcohol use disorder (MAUD) are particularly low. The goal of the present scoping review is to characterize patient perspectives about MAUD to identify barriers to MAUD and potential areas of future work to increase access, initiation, and retention on MAUD. Searches of titles and abstracts were conducted on PubMed, EMBASE, PsycINFO, and CINAHL until March 2024 with patient perspective-, MAUD-, and alcohol-related keywords.

Advances in Pharmacotherapy Development: Human Clinical Studies.

For more than 25 years, researchers have made advances in developing medications to treat alcohol use disorder (AUD), highlighted by the US Food and Drug Administration's (FDA's) approval of disulfiram, naltrexone (oral and long-acting), and acamprosate. These medications are also approved in Europe, where the European Medicines Agency (EMA) recently added a fourth medication, nalmefene, for AUD. Despite these advances, today's medications have a small effect size, showing efficacy for only a limited number of individuals with AUD. However, a host of new medications, which act on variety of ph

Pharmacotherapy for alcohol use disorder among adults with medical disorders in Sweden.

BACKGROUND: Alcohol-attributable medical disorders are prevalent among individuals with alcohol use disorder (AUD). However, there is a lack of research on prescriptions of pharmacological treatment for AUD in those with comorbid conditions. This study aims to investigate the utilization of pharmacological treatment (acamprosate, disulfiram and naltrexone) in specialist care among patients with AUD and comorbid medical diagnoses. METHODS: This was a descriptive register-based Swedish national cohort study including 132,728 adults diagnosed with AUD (N = 270,933) between 2007 and 2015. The exp

Understanding Indian Doctors' Attitudes Toward Alcohol Pharmacotherapy and Prescription: A Mediational Analysis.

BACKGROUND: Alcohol use disorder (AUD) causes significant morbidity and mortality. Prescription of AUD pharmacotherapies remains low. Attitudes toward AUD pharmacotherapies influence their prescription rates. We conducted an online survey to assess how attitudes modulate the association between knowledge and prescription rates for four commonly used AUD medications among Indian clinicians. METHODS: An online survey was sent nationwide to Indian physicians between February and April 2023 to assess knowledge, attitudes, and practices of various AUD medications. An a priori model was defined to

Patient Perspectives on Pharmacotherapy of Alcohol Dependence.

INTRODUCTION: Pharmacotherapy with drugs like naltrexone or acamprosate is a well-evaluated element in the treatment of alcohol dependence (AD). However, in many countries, these medications are rarely administered. The objective of the present study was to identify from patients' perspective factors that prevent the initiation and compliance with pharmacological treatment of AD. METHODS: Patients from inpatient alcohol withdrawal treatment underwent a standardized interview. Questions included socio-demographic data, history of AD, treatment history, knowledge and personal experience regardi
