Epidemiology of non-fatal injuries among Egyptian children: a community-based cross-sectional survey.

BACKGROUND: Injuries are a major cause of childhood morbidity and mortality worldwide. We aimed to determine the magnitude and characteristics of child injuries in Egypt and to identify the associated risk factors. METHODS: A community-based, cross-sectional survey was conducted over 27 Egyptian governorates from June to October, 2011. The target population was 1977 households with children aged 0-18 years who had experienced accidental injuries. RESULTS: In the 6-month period before the investigation, 1576 injuries were reported in 1472 children from a sample population of 1399 households (…

Imaging of spinal injury in abusive head trauma: a retrospective study.

BACKGROUND: Spinal imaging has been a neglected part of abusive head trauma (AHT) imaging. As most of the radiographs and CT spine are negative in AHT in infants, the cervical spine is assumed to be normal. There is increasing evidence in the role of injury to brainstem and cervical cord in the pathogenesis of AHT. In addition, in courts of law, there is fierce debate about AHT, its mimics and other disparate nontraumatic diagnoses explaining the neuroradiological and skeletal findings. However, this discussion ignores the evidence and significance of spinal injury. We sought to study the cerv

Distal Internal Maxillary Artery Occlusion with Palatal Necrosis following Cheek Injection with Calcium Hydroxylapatite.

Accidental intravascular injection of cosmetic fillers resulting in dermal infarction represents one of the most feared complications in aesthetic medicine. Despite the infrequency of these injuries, the rapid rise in filler use has caused a sudden increase in the incidence of filler-induced skin necrosis. Because the complex vascular anatomy of the face subserves a wide range of cutaneous and mucosal tissues, it has been surmised that occult injuries affecting the oronasal cavities could occur as a result of accidental intravascular injection of dermal fillers. We report an instance of ischem

Sharp force injuries in "clinical" forensic medicine.

In general, the forensic evaluation of sharp force injuries in living and dead individuals follows the same morphologic principles. Still, there are some special features of sharp force injuries in the clinical context, which have to be considered as examination findings on the living are interpreted to differentiate between accidental origin, self-infliction or homicidal assault. These include the frequency and localization of defence injuries, injuries of the perpetrator, and artificial injuries, especially those inflicted for the purpose of insurance fraud. Characteristics and differential

Pathophysiology of iatrogenic and traumatic skull base injury.

Traumatic skull base injuries can be divided into iatrogenic injuries from surgery and accidental traumatic injuries. The most common cause of iatrogenic skull base injury is endoscopic sinus surgery. The reported rate of cerebrospinal fluid (CSF) leak as a complication of endoscopic sinus surgery is less than 1%. Intraoperative injury most commonly occurs at the ethmoid roof and the lateral lamella of the cribiform plate. Case complexity, such as revision surgery, as well as surgical experience has been shown to be a contributing factor to iatrogenic CSF leaks during endoscopic sinus surgery.

The Management of Pediatric Genital Injuries at a Pediatric Emergency Department in Japan.

OBJECTIVES: Genital injuries among children are often associated with consumer products or specific activities. There are few descriptive studies from Asia on pediatric genital injuries seen in the emergency department (ED). The aim of this study was to describe the characteristic features of accidental genital injuries among children. METHODS: A retrospective chart review of children aged 15 years or younger who visited our ED for genital injuries between March 2010 and November 2014 was conducted. Data on age, arrival time at the ED, location of the incident, mechanism of injury, objects, i

Accidental head trauma during care activities in the first year of life: a neurosurgical comparative study.

OBJECTIVES: This study aims to describe clinical-epidemiological data regarding accidental fall injuries occurring during homecare activities among children up to 1 year of age and to compare their outcomes according to the type of trauma. METHODS: We searched four different hospital databases on head injuries from 1999 to 2009. Patients recorded under the descriptors "accidental fall" and "home-related" in the subtext were selected. Patients were classified into two groups: those who flipped over and fell from a changing table (n = 253) and those who fell from the bed sustaining a direct imp

The Groningen Trauma Study. Injury patterns in a Dutch trauma centre.

During the period 1970-1993, 245,251 visits were recorded in the trauma registry of the University Hospital Groningen, The Netherlands. An analysis of injury antecedents revealed five principal causes (ICD-CM): accidental falls (28%), sports and unspecified accident (26%), traffic (19%), cutting and piercing instruments (10%) and violence (4%). The trend analysis across the 24 year period showed that the incidence of injuries due to traffic and accidental falls decreased, while the rate of injuries due to assault increased 2-fold. Within the subgroup of traffic accidents, injuries caused by ca

Diagnostic criteria for cutaneous injuries in child abuse: classification, findings, and interpretation.

Physical abuse of children has many manifestations. Depending on the type of force involved, specific injury patterns are produced on the body of the child, the morphology and localization of which are forensically relevant in terms of diagnostic classification as child abuse. Typical patterned bruising includes, for example, tramline bruises resulting from blows with oblong, stick-like objects. In addition to rounded or one-sided horseshoe-shaped bite injuries, injuries of different ages, clustered injuries (e.g., three or more individual injuries in the same body region), and thermal injurie

Fatal Transorbital Intracranial Penetrating Injury Due to a Bicycle Brake Handle.

Transorbital intracranial injuries are uncommon and mostly accidental in nature. These injuries are usually associated with high mortality. The key to preventing this mortality involves an early diagnosis of intracranial injuries and immediate and appropriate management. Any delay can pose a serious risk to the patient's life. A case of a 55-year-old man who had experienced an accidental self-fall while riding a bicycle is presented. Initially, he was treated at a primary health center. He was referred to the tertiary care institute due to unavailability of ancillary investigation at the prima

A child with bone fractures and dysmorphic features: remember of pycnodysostosis and craniosynostosis.

Accidental bony injuries are common in children. Children may also present with bony injuries following non-accidental injuries. Pathological fractures, though extremely rare, are an important entity and constitute fractures that occur in abnormal bones, usually after minor trauma. Pycnodysostosis is a rare skeletal dysplasia characterised by a clinical phenotype that includes short stature, skull deformities, osteosclerosis, acroosteolysis and bone fragility. Often the disease is diagnosed at an early age as a result of the investigation of short stature. However, the diagnosis is sometimes d…

Injury to the prepuce.

Injury to the prepuce is uncommon. A total of 32 patients were treated within a 3-year period. A difference in the aetiology between boys and adults was noted. Accidental injury to the prepuce occurred in 6 boys, with zipper injuries being the commonest among children. Coital and self-inflicted injuries accounted for 85% of adult cases. In patients with coital injuries, predisposing phimosis or a short frenulum was common.

Eye injuries caused by elasticated "octopus" straps.

"Octopus" elasticated straps are a common cause of severe accidental eye injuries, which we believe are largely preventable. In a retrospective study (January 1990 to August 1996), we identified 42 patients with such injuries severe enough to warrant admission to hospital. The injuries included hyphaema, vitreous haemorrhage, retinal detachment, and choroid and globe rupture, with 28% (12/42) of the injuries resulting in permanent visual loss. We believe octopus straps should not be available for sale in their current form.

Fatal head injury in children younger than 2 years in New York City and an overview of the shaken baby syndrome.

CONTEXT: Shaken baby syndrome is a controversial topic in forensic pathology. Some forensic pathologists state that shaking alone is insufficient to explain death and that an impact must have occurred even if there is no impact site on the head. OBJECTIVE: To examine a large cohort of fatal, pediatric head injuries for patterns of specific autopsy findings and circumstances that would support or dispute pure shaking as the cause of death. DESIGN: We retrospectively reviewed 59 deaths due to head injuries in children younger than 2 years certified in our office during a 9 year period (1998-20

Asphyxiation from shoulder seat belts: an unusual motor vehicle injury.

Two cases of accidental asphyxia caused by lap-shoulder seat belts are reported. Lethal neck injuries from shoulder seat belts are uncommon, and episodes of asphyxiation are even rarer. Positioning of the shoulder belt over the neck, short stature, and incapacitation from other injuries were predisposing factors to upper airway compromise in these cases.

Accidental and Abusive Mandible Fractures in Infants and Toddlers.

OBJECTIVE: Mandible fractures are uncommon injuries in infants and young children and may raise concern for nonaccidental trauma. Our study describes several children with mandible fractures to identify features that might differentiate abuse from accident. METHODS: Records and imaging were reviewed for children aged 24 months and younger who were diagnosed with mandible fractures at 2 tertiary pediatric care centers. Twenty-one cases were included, 8 of whom had formal child abuse consultations. Cases were reviewed for mechanisms of injury, physical examination findings, and occult injuries
