Acoustic measurement of subglottic stenosis.

A device that determines cross-sectional area (CSA) of the airway by acoustic reflections (Hood, Inc) was used to measure subglottic area. Airway models were made from Plexiglas rings with known internal dimensions similar to clinically encountered stenoses of various lengths and diameters. Acoustic measurements of airway area were made and compared to actual CSA. There is a strong correlation between CSA measured acoustically and the actual area of simulated stenoses. However, when the CSA of the stenosis was < 0.64 cm2, the signal was impaired, resulting in overestimation of the stenotic CSA

Binaural summation of the acoustic reflex.

OBJECTIVE: The ways in which summation of energy or of information occurs have long been used to explore sensory mechanisms. In the case of the acoustic reflex, some binaural summation is known to occur, but both data and specification of method have been sketchy. Accurate estimates of mean and standard deviation were therefore sought to compare binaural acoustic reflex summation (BARS) with binaural loudness summation (BLS) to determine whether these have a common basis. DESIGN: A specified method was developed for determining reliable acoustic reflex values from admittance/intensity functio

Delays in the diagnosis of acoustic neuromas.

From the medical files of 164 consecutive patients who underwent surgical treatment for a unilateral acoustic neuroma between 1980 and 1992, we collected data on the delay until the diagnosis was made. A distinction was made between the patient's and general practitioner's delay (delay 1) and the delay after the specialist's first visit until the radiologic diagnosis (delay 2). The average delay was 35.7 months (SD, 62.2) for delay 1 and 15.2 months (SD, 36.3) for delay 2. Specialist's delay (otolaryngologist or neurologist) was divided into a delay of a maximum of 12 months (134 patients) and

Unusual presentations of acoustic tumours.

A series of 238 consecutive patients with acoustic neuromas, operated on in Paris has been studied to identify unusual presentations and varied symptomatology. The most common history was that of a progressive unilateral hearing loss (in 68.1%), with tinnitus (in 49.1%) or disequilibrium (in 49.1%) or both. Sudden hearing loss (in 14.7%) or fluctuating hearing loss (in 6.3%), and a single or repeated episodes of acute vertigo (in 8.8%) were seen less commonly. Headaches occurred as an associated symptom in 10.5%, tinnitus was the sole symptom in 2.8% and other uncommon symptoms included otalgi

Epithelioid Schwannoma of the acoustic nerve.

A 25-year-old woman presented with left progressive deafness due to an intracranial tumor of the cerebello-pontine angle. Biopsy and subsequent total surgical excision indicated origin from the acoustic nerve. Histologically, the tumor showed typical feature of Schwannoma associated with a population of epithelioid cells. Three years after surgery no local invasion, no relapse, and no metastasis were noted. Immunohistochemistry showed a strong positivity for S100 protein in the cytoplasm of Schwannian and epithelioid tumoral cell components. This positivity in the epithelioid cells correspond

Postoperative headache in acoustic neuroma.

Recently, it has become obvious that disabling postoperative headache is a major problem with acoustic neuroma surgery. A questionnaire was used to retrospectively evaluate the incidence, clinical features, prognosis and possible therapeutic measures of this particular form of headache. Forty-two percent (42%) of patients had some headache prior to surgery but this was not a major complaint. After surgery, 75% of patients experienced headache. Only 24% had complete relief of headache. A very gradual improvement of the pain occurred in 32%. Pathogenesis remains unclear, but clinical characteris

Spontaneous haemorrhage in acoustic neurinomas.

Gross intratumoural haemorrhage in acoustic neurinoma is rare. The authors describe seven patients, out of a consecutive 99 cases of acoustic neurinoma operated by the first author, who presented with haemorrhage. Risk factors for haemorrhage appear to be large size, mixed Antoni type and secondary vascular changes.

When to suspect an acoustic neuroma.

Acoustic neuroma is a rare but important cause of sensorineural hearing loss. Patients with asymmetric hearing loss or unilateral tinnitus should be evaluated expeditiously to prevent further neurologic damage. Gadolinium-enhanced magnetic resonance imaging is the best tool for making the diagnosis of acoustic neuroma. Surgical removal using a translabyrinthine approach, is the favored treatment, although radiation therapy and expectant management may be chosen for patients at high surgical risk.

[Analysis of misdiagnostic acoustic neuroma].

72 cases (74 tumors) of acoustic neuroma pathologically confirmed after operation were analysed with special regard to the misdiagnosis made in their initial seeking for medical advice from ENT doctors. The average period of misdiagnosis was 5.48 years. The disease entities that the acoustic neuroma had been mistaken for were sensorineural hearing loss of unknown origin, idiopathic sudden deafness, tinnitus of unknown origin, cervical osteophytosis, nasopharyngeal carcinoma, etc. Emphasis was placed on the discussion of various causes of misdiagnosis.

Acoustic impedance of the maternal abdomen.

Contact sensors to monitor fetal heart, breathing, and movement sounds with increased sensitivity and bandwidth are under development. To understand the inherent acoustical properties of the maternal abdomen and its interaction with these sensors, the driving-point impedance Z(j omega) was measured in nine women during their last trimester of pregnancy. An electromechanical shaker with a contact area of 2.85 cm2 produced abdominal vibrations between 10 and 500 Hz, and the resulting force and acceleration were measured. After digitally integrating the acceleration signal to obtain the velocity

Microsurgical anatomy of acoustic neuroma.

Because acoustic neuromas most frequently arise in the posteriorly placed vestibular nerves, they usually displace the facial and cochlear nerves anteriorly (Figs. 11, 12, and 13). The facial nerve is stretched around the anterior half of the tumor capsule. Variability in the direction of growth of the tumor arising from the vestibular nerves may result in the facial nerve being displaced, not only directly anteriorly, but also anterior-superiorly or anterior-inferiorly. The nerve is infrequently found on the posterior surface of the tumor. Because the facial nerve always enters the facial can

Complications in acoustic neuroma surgery.

There has been a continuous lowering of mortality and morbidity rates in the 20th century, thanks to increasing experience and newer technology. Despite this, the readership should be aware of the hazards involved in this challenging field. Complications occur, and their incidence, mechanism, management, and prevention have been outlined here so physicians performing these procedures will be better prepared to manage them. We have found that the occurrence of complications can be greatly reduced through a team approach using the combined talents of a neurotologist, neurosurgeon, and frequently

MR of hemorrhagic acoustic neuromas.

The magnetic resonance (MR) appearance of hemorrhagic acoustic neuromas has not previously been reported. Four patients with surgically proven acoustic neuromas containing spontaneous hemorrhage were preoperatively evaluated with MR imaging. All patients presented with new onset of symptoms and three had subarachnoid hemorrhage. Signal changes on T1- and T2-weighted spin echo sequences were characteristic for acute, subacute and chronic intraparenchymal hemorrhage. Recognition of hemorrhagic changes on MR is important since more rapid surgical intervention is required in this patient subgroup.

CT in diagnosis of acoustic neuromas.

A detailed analysis of the CT findings in 75 cases of acoustic neuroma is presented. The method of examination included plain and enhanced CT, metrizamide CT cisternography (M-CTC), and gas CT cisternography (gas-CTC). The common CT appearances of acoustic neuromas were as follows: 93.6% appeared as isodense or hypodense on precontrast scan; homogeneous enhancement was observed in 53.8% on postcontrast scan; the tumor center, mostly located at the level of the internal acoustic canal, was spherical in shape with an acute angle between the lateral tumor border and petrous bone; and there was wi

Melanotic schwannoma of the acoustic nerve.

Melanotic schwannoma is a rare neoplasm, with only 23 previously reported cases. We report an additional case of a melanotic schwannoma arising from the acoustic nerve in a 74-year-old man who had a probable forme fruste of neurofibromatosis. We briefly review the literature of this neoplasm and discuss theories regarding the histogenesis of these tumors.
