Voices of homeless alcoholics who frequent Bellevue Hospital: a qualitative study.

STUDY OBJECTIVE: We describe the evolution, environment, and psychosocial context of alcoholism from the perspective of chronically homeless, alcohol-dependent, frequent emergency department (ED) attendees. We use their words to explore how homelessness, health care, and other influences have contributed to the cause, progression, and management of their alcoholism. METHODS: We conducted detailed, semistructured, qualitative interviews, using a phenomenological approach with 20 chronically homeless, alcohol-dependent participants who had greater than 4 annual ED visits for 2 consecutive years

Holotranscobalamin is a useful marker of vitamin B12 deficiency in alcoholics.

BACKGROUND: Measurement of serum cobalamin (Cbl) levels is the standard investigation for assessing vitamin B12 deficiency. Falsely increased values of Cbl can be caused by alcoholic liver disease. Measurement of total vitamin B12 serum levels might be misleading in alcoholics, because a tissue metabolic deficiency is possible even with normal serum Cbl levels. Holotranscobalamin (HoloTC), the Cbl metabolically active fraction, is considered as a better index of vitamin B12 deficiency. METHODS: For assessing vitamin B12 status, we evaluated 22 adult alcoholic male patients by measuring in par…

Papillary atrophy of the tongue and nutritional status of hospitalized alcoholics.

BACKGROUND: Atrophy of the papillae, mucosa, and dorsum of the tongue are considered classical signs of nutritional deficiencies. OBJECTIVE: To assess the nutritional status of hospitalized alcoholics with or without papillary atrophy of the tongue. METHODS: This study was performed with 21 hospitalized alcoholics divided into Atrophic Glossitis Group (n=13) and Normal Tongue Group (n=8). Healthy, non-alcoholic volunteers composed the Control Group (n=8). Anthropometry and bioelectric impedance were performed, and serum vitamins A, E, and B12 were determined. RESULTS: There were no statisti…

Pathological gambling in treatment-seeking alcoholics: a national survey in France.

AIMS: To estimate the prevalence of pathological gambling (pG) in people addicted to alcohol and drugs, particularly those with alcohol use disorders (AUDs), including new forms of gambling such as video and on-line games; and to update the socio-demographic profile of people suffering from both AUD and pG. METHOD: A cross-sectional survey was conducted in 1 week of 2009 in 55 French addiction treatment centres, including all consecutive patients attending that centre, using an anonymous questionnaire which included a tool specific for pG screening, the DEBA-jeu questionnaire (Détection et Be

Impairment on a simulated gambling task in long-term abstinent alcoholics.

BACKGROUND: Recent research indicates that currently active or recently detoxified substance abusers make more disadvantageous decisions on a simulated gambling task (SGT). This study expands on the current literature by using the SGT to examine decision making in long-term abstinent alcoholics (mean of 6.6 years' abstinence) who do not have antisocial personality disorder or a history of conduct disorder. METHODS: A total of 102 subjects (58 controls and 44 abstinent alcoholics) were tested on the SGT, in which subjects choose cards from 4 different decks that vary in terms of the magnitude …

MRI and muscle signal intensities in alcoholics compared with control subjects.

OBJECTIVE: To evaluate by magnetic resonance imaging (MRI) the difference in muscle signal intensities between alcoholics and control subjects. METHODS: Thirty-four healthy subjects and 46 alcohol-dependent individuals were assessed. MRI was carried out using a low-field magnet (0.2 Tesla) and a limb-dedicated coil. The presence of muscle changes was evaluated by measuring signal intensities at the medial (MG) and lateral heads of the gastrocnemius muscle by T1-/T2-weighted and gradient-echo short tau inversion recovery sequences. The mean signal intensities of the two sample groups were comp…

Ketoacidosis and lactic acidosis--frequent causes of death in chronic alcoholics?

In clinical medicine, severe keto- or lactic acidosis associated with vomiting, nausea, abdominal pain, tachycardia or pathological respiration, has been described in chronic alcoholics. This study reports on fatalities of chronic alcoholics where the cause of death could not be determined by thorough autopsy, histology and toxicology including determination of alcohol concentration. In a first series, acetone was determined in the blood of such chronic alcoholics (n = 24), diabetics with metabolic decompensation (n = 7), cases of hypothermia (n = 7) and controls (n = 218). Among the 24 chroni…

Changes in HIV-related behaviors among heterosexual alcoholics following addiction treatment.

In order to measure changes in HIV-related behaviors among heterosexual alcoholics following treatment, we conducted a prospective cohort study of 700 self-identified alcoholics recruited from five public alcohol treatment centers, all of which included HIV risk-reduction counseling. Respondents underwent an HIV antibody test and interviewer-administered questionnaire at entry to alcohol treatment and after a mean of 13 months later. Compared to baseline, at follow-up there was an overall 26% reduction in having sex with an injection-drug-using partner (23% versus 32%, P < .001) and a 58% redu…

Impaired delay eyeblink conditioning in amnesic Korsakoff's patients and recovered alcoholics.

The performance of amnesic Korsakoff patients in delay eyeblink classical conditioning was compared with that of recovered chronic alcoholic subjects and healthy normal control subjects. Normal control subjects exhibited acquisition of conditioned responses (CRs) to a previously neutral, conditioned tone stimulus (CS) following repeated pairings with an unconditioned air-puff stimulus, and demonstrated extinction of CRs when the CS was subsequently presented alone. Both amnesic Korsakoff patients and recovered chronic alcoholic subjects demonstrated an impairment in their ability to acquire CR

Helicobacter pylori, active chronic antral gastritis, and gastrointestinal symptoms in alcoholics.

The frequency of Helocobacter pylori (Hp.) infection and active chronic antral gastritis among people with excessive alcohol consumption is not known. A high alcohol intake regularly causes acute gastroduodenitis. In this study, the prevalence of Hp. infection and active chronic antral gastritis in alcoholics compared with nonalcoholic controls was studied. Further, the frequency of gastrointestinal symptoms was registered. Diagnostic methods for Hp. were compared. Twenty-four alcoholics admitted to the hospital for detoxification underwent upper gastrointestinal endoscopy regardless of sympto…

Chronic alcoholics have substantial glial pathology in the forebrain and diencephalon.

We have analyzed glial changes in forebrain and diencephalic regions in 19 alcoholics with different histories of chronic alcohol consumption and related medical complications including Wernicke's encephalopathy and alcoholic liver disease. Cases with postmortem evidence of hepatic encephalopathy were excluded. Two of the alcoholic patients had ceased drinking for several years prior to death. Brains were obtained postmortem and fixed in formalin. Serial 50 microns sections of the forebrain and diencephalon at 750 microns intervals were stained with standard histochemical stains (haematoxylin …

The development of alcoholics' and heavy drinkers' consumption: a longitudinal study.

Results are reported from two prospective longitudinal studies of heavy drinkers and alcoholics who have received treatment. The present study focuses on the drinking habits of subjects who have been interviewed four times after the end of treatment. The data uncover substantial changes in heavy drinkers' and alcoholics' consumption level over time, and these observed changes are only partly explainable in terms of measurement error and short-term fluctuations. Changes show signs of accumulation over time. This suggests that heavy drinkers' and alcoholics' consumption level is very unstable ov…

Parental alcoholism: an examination of male and female alcoholics in treatment.

This study investigated the implications of parental alcoholism in 639 male and female alcoholics who were in treatment. In comparison with persons who did not report a parental history of alcoholism (PA-), the 264 offspring of alcoholics (PA+) were characterized by higher levels of alcohol dependency and alcohol-related consequences, increased reports of use of other drugs and higher levels of MMPI-measured psychopathology. Comparisons of PA+ patients with PA- patients with the same age of onset of problematic alcohol use suggested that PA+ persons are characterized more by an early onset of …

MMPI experimental scale correlates of the MAACL--R with male alcoholics.

Concurrent and discriminative validity of the MAACL--R scales were studied by means of correlations with selected MMPI experimental scales (AR, DR, HOS, Poor Morale, and ES) for a sample of 88 male VA alcoholics. Concurrent validity of Anxiety, Depression, Hostility and PASS, and discriminative validity of the Anxiety scale were confirmed.

The construct validity of an aftereffect-based subtyping system for alcoholics.

In an earlier project, we identified five alcohol-consumption aftereffect factors, which were named Hangover, Euphoria, Flushing, Seizures, and Sleepiness. In this study (N = 100) we assessed the construct validities of the five, using 47 MMPI, self-report, and recidivism criteria. The number of significant relationships between the factors and the criteria substantially exceeded chance. The Hangover factor related to social maladjustment and to the MMPI Psychopathic Deviate, Paranoia, Psychasthenia, Hypomania, and Masculinity-Femininity scales. The Euphoria factor was associated with a high n

Prostaglandin E2 and leukotriene B4 synthesis by peripheral leucocytes in alcoholics.

Alcohol inhibits phospholipase (PL) activity in a number of animal models. We have therefore measured prostaglandin E2 (PGE2) and leukotriene B4 (LTB4), liberated by stimulated peripheral blood mononuclear cells (PBMC) and neutrophils respectively in chronic alcoholics and in control subjects. Peripheral blood mononuclear cells from alcoholics produced less PGE2 (p less than 0.01) and neutrophils produced less LTB4 (p less than 0.025). Reduced PGE2 production by PBMC of alcoholics was corrected by the addition of exogenous arachidonic acid (p less than 0.005) whilst neutrophil LTB4 production …

[Clinical study on alcoholic pancreatitis in alcoholics (especially in ERP findings)].

The pancreatic functions (PFD test and 75g GTT), pancreatic enzymes (serum-amylase, urine-amylase, serum-elastase I and serum-lipase), alcohol consumption histories, clinical symptoms, histological findings in the liver and ERP findings have been examined in 66 alcoholic patients. Fourty two out of 66 cases (64%) showed abnormal ERP findings which were compatible with chronic pancreatitis. But among these 42 cases, only 9 cases (21%) showed clinical symptoms such as epigastralgia, back pain, diarrhea and emaciation which suggest the existence of chronic pancreatitis. The degree of liver damage
