Three case reports of patients with HELLP-syndrome show how different the clinical courses in HELLP-syndrome may be. A maternal case of death after eclampsia and HELLP-syndrome explains that early diagnosis and immediate therapy are important for success in the treatment of HELLP-syndrome. After delivery--at present the only efficient causal therapy (we recommend a quick cesarean section after making the diagnosis)--an increase of the number of thrombocytes and a normalization of the liver enzymes can be observed within a few days. An intensive co-operation of obstetricians, anesthetists, and …

A 10-year-old female with a history of a complete C2 fracture, tracheostomy, and ventilator dependence, presented for a T2 to pelvis posterior spinal fusion. She encountered airway obstruction causing rising end-tidal CO2 (ETCO2), mild oxygen desaturation, and increased peak inspiratory pressures to obtain adequate tidal volumes. Airway suctioning was not successful. A fiberoptic scope was utilized to assess the obstruction and check the position of the endotracheal tube, revealing hard secretions that were resistant to removal. A vibrating mesh nebulizer (VMN), which was already connected to

STUDY OBJECTIVE: In 2018, the American Society of Anesthesiologists stated that student registered nurse anesthetists (SRNAs) "are not yet fully qualified anesthesia personnel." It remains unclear, however, whether postprocedural outcomes are affected by SRNAs providing anesthesia care under the medical direction of anesthesiologists, as compared with medically directed anesthesiology fellows or residents, or certified registered nurse anesthetists (CRNAs). We therefore aimed to examine whether medically directed SRNAs serving as in-room anesthesia providers impact surgical outcomes.

A female patient, known to have hypermobile Ehlers-Danlos syndrome (hEDS), underwent several elective gastroscopies under sedation in different hospitals. Except for a single incident of mild laryngospasm during emergence, all procedures were uneventful. On that occasion, following the procedure in the postanesthesia care unit, the patient suffered severe airway obstruction, and standard airway rescue techniques exacerbated adequate ventilation. After the removal of all stimuli and maintaining only an indirect oxygen supply via a mask in front of her face, her airway improved, and the patient

Approximately 0.6% of the United States population identifies as transgender or gender nonbinary. The term transgender refers to a person who experiences incongruence between their sex at birth and their gender identity. The care of the transgender patient is complex, often requiring the expertise of various medical and surgical specialties. The prevalence of gender-affirming surgery is increasing as societal acceptance and cultural norms are shifting. While gender-affirming procedures include a variety of body systems, airway and facial procedures are of particular importance to anesthesia pr

BACKGROUND: Professions requiring frequent and/or long lasting wet-work are at high risk for contact dermatitis (CD). CD may lead to loss of work productivity, sick leave and reduced quality of work. The 1-year prevalence of healthcare workers varies between 12% and 65%. However, the prevalence of CD among surgical assistants, anaesthesia assistants and anaesthesiologists is unknown.
OBJECTIVES: (1) To establish the point-prevalence and 1-year prevalence among surgical assistants, anaesthesia assistants and anaesthesiologists and (2) to determine the impact of CD on work and daily activities.

The 7th National Audit Project of the Royal College of Anaesthetists studied peri-operative cardiac arrest in the UK, a topic of importance to patients, anaesthetists and surgeons. We report the results of the 12-month registry phase, from 16 June 2021 to 15 June 2022, focusing on management and outcomes. Among 881 cases of peri-operative cardiac arrest, the initial rhythm was non-shockable in 723 (82%) cases, most commonly pulseless electrical activity. There were 665 (75%) patients who survived the initial event and 384 (52%) who survived to hospital discharge. A favourable functional outcom

Dr. Richard Gill published a textbook in London in 1906 titled The CHCl3 - Problem. Gill was the Chief Chloroformist at St Bartholomew's Hospital, London, and was recognized as an excellent clinical anesthetist and was of an intelligent but reclusive and eccentric personality. This textbook is rarely found and has not been appreciated in the history of anesthesia for several reasons including that it was generally ignored at publication and few copies exist in libraries around the world. The CHCl3Problem is written in a verbose, archaic, and convoluted fashion and is rarely quoted. It has no r

BACKGROUND: There is limited information on UK anaesthetists' perspectives and experiences of perioperative anaphylaxis. This baseline survey of the Sixth National Audit Project (NAP6) aimed to identify relevant departmental preparedness and practices, and individual experiences, perceptions and drug-avoidance patterns.
METHODS: All anaesthetists in 356 UK NHS hospitals were invited to complete an electronic survey.
RESULTS: 11 104 anaesthetists (77% crude response rate) from 341 (96%) hospitals responded. Most had immediate access to guidelines for anaphylaxis treatment (87%) and establishe

This year, 2016, marks the centenary of some of the worst battles of World War I. It is timely to reflect on the experiences of those who lived through those events, and the significant effects on the development of anaesthesia that occurred as a result. War has been a catalyst that has accelerated the development of medicine and surgery throughout history. The challenges of the Western Front, with its massive numbers of injured, saw progress in the understanding and management of trauma and shock, and significant improvement in the provision of anaesthesia and the training of anaesthesia prov

Intraoperative hyperthermic intraperitoneal chemotherapy (HIPEC) may have a significant impact on physiological functions and is therefore a special challenge for anesthetists. In the presented case after opening the parietal pleura during subphrenical peritonectomy the HIPEC solution accidentally leaked into the right hemithorax with subsequent pleural effusion of more than 2,000 ml. After extubation the patient presented with acute oxygen desaturation and orthopnea. Following pleural sonography and chest X-ray in the operating theatre and recovery area, a thorax drainage was inserted into th…

Respiratory obstruction and stridor in infants and children are not uncommon. A rare cause of these life-threatening symptoms is congenital saccular cyst. An accurate diagnosis of saccular cyst can be made by eliciting a good history, by endoscopic visualization of the lesion, and by computed tomography. Endoscopic excision is the preferred treatment for infants, whereas an external approach is reserved for older children. It can be difficult for anesthetists to intubate infants when the anatomy of the larynx is distorted, but the choice of tracheotomy for an infant has many drawbacks as well.…

Ischaemic cardiomyopathy with low ejection fraction (EF) poses a perioperative challenge to the anesthetist due to the risk of hemodynamic instability, cardiovascular collapse, and heart failure. More so when a patient has an Automated Implantable Cardiovertor Defibrillator (AICD) in situ. We report the anesthetic management of a patient with ischaemic cardiomyopathy with an EF of 20% and AICD in situ posted for open right hemicolectomy. Dynamic hemodynamic monitoring with preparedness to manage fluid shifts, hemodynamic fluctuations, and adequate pain management is essential to successful ane

This article discusses anesthesia assessment concepts related to airway evaluation and airway maintenance for safe and reliable selection of either open system (entrainment of room air) or closed system (no entrainment of room air) airway devices, which can be used during office-based oral surgical procedures, depending on the needs of a patient. Dental facial and oral structures are integral to an anesthetist's preoperative patient evaluation prior to surgery. The preoperative medical history and physical examination as well as the nature of the oral surgical procedure affect the selection of

Recognizing and acting early on airway compromise reduces morbidity and mortality in patients with airway obstruction. Causes include foreign bodies, toxic/hot fumes, difficult intubation, laryngeal spasm, and tumors. Before definitive control of the airway is possible, provide 100% oxygen with a tightly fitting mask to optimize body oxygen stores. Pulse oximetry is a poor indicator of airway compromise; a decreasing arterial hemoglobin oxygen saturation is a late sign of impending hypoxemia. Basic airway maneuvers improve the patency of an obstructed airway. Getting help from an anesthetist e

The aim of monitored anesthesia care (MAC) in neurosurgery and interventional neuroradiology is to provide optimal surgical conditions including patient immobility and comfort, facilitate intraoperative neuromonitoring, and rapidly treat any complication. The anesthetist has a crucial role in facilitating procedures, and this requires an understanding of specific procedures, their potential complications, and their management. Choice of anesthetic technique varies between centers with little data to support any specific technique. Dexmedetomidine is useful as a sedative and hypnotic for patien

As a consequence of the fact that the population is getting progressively older it is to be expected that the numbers of aged patients requiring surgical interventions on the musculoskeletal system will continue to increase. The decision as to where to draw the line for surgical treatment must not be based on the calendar age of the patient, but should be taken by the patient him/herself after comprehensive counseling by the family doctor, surgeon and anesthetist, and an assessment of the impairment of his/her current quality of life. In the field of orthopedics, the majority of the relevant p

Ophthalmic medications are often overlooked in the preoperative evaluation for patients experiencing surgery and anesthesia as "just eye drops." Both acute and chronic administration of these agents affects the anesthesia practitioner. Virtually every patient experiencing anesthesia for cataract extraction will have other nonanesthetic medications administered perioperatively. Medications are dispensed throughout the age continuum for the treatment of glaucoma on a chronic basis. All of these medications have the potential to cause morbidity and mortality by themselves or in combination with o
