Soluble P-selectin is a marker of plaque destabilization in unstable angina.

Unstable coronary syndromes usually involve platelet activation and thrombus formation at the site of atherosclerotic plaque. P-selectin in platelets and endothelial cells mediates adhesive interaction with leucocytes to form thrombi. The aim of this study was to asses the level of soluble P-selectin (CD62P) as a non invasive marker of coronary plaque destabilization in unstable angina (U.A.). Serum samples were collected from 23 male patients with UA, 20 male patients with stable angina (SA) and 13 healthy controls. Soluble P-selectin (sP-selectin) level was measured by ELISA technique. The m

Immunomediated and ischemia-independent inflammation of coronary microvessels in unstable angina.

This study investigated whether the myocardium is involved in the acute inflammatory reaction associated with bursts of unstable angina (UA). We looked for the presence of activated DR+ inflammatory cells and the expression patterns, localization, and immunostaining identification of genes for cytokines (IL-1beta, TNF-alpha, IL-6, and IFN-gamma), MCP-1, and iNOS in the left ventricle biopsies from 2-vessel disease anginal patients, 24 with UA and 12 with stable angina (SA), who underwent coronary bypass surgery. Biopsy specimens from 6 patients with mitral stenosis who underwent valve replacem

Increased levels of monocyte-related cytokines in patients with unstable angina.

Inflammatory cytokines play important roles in coronary artery disease. We investigated the clinical significance of monocyte-related cytokine expression in patients with angina pectoris. We studied 26 patients with stable effort angina and 20 patients with unstable angina in whom stenotic lesions of the coronary arteries were confirmed by selective coronary angiography. Plasma levels of interleukin-6 (IL-6), macrophage colony stimulating factor (MCSF), and monocyte chemoattractant protein-1 (MCP-1) were measured. Plasma levels of IL-6, MCSF, and MCP-1 in patients with unstable angina were sig

[Lectin-induced aggregation of neutrophil granulocytes in patients with unstable angina].

Lectin-induced aggregation of neutrophilic granulocytes was studied in patients with unstable angina pectoris. PHA-P, WGA, ConA, LCL, PHA-L, PHA-E, PSL, HPL, and LABA were used as aggregation inductors. The results indicate restructuring of carbohydrate determinants of glycoprotein receptors in neutrophilic granulocytes which stimulates their aggregation.

[Stratification of cardiac risk in unstable angina: value of troponin measurements].

Recent studies have identified patients with unstable angina and increased troponin I or T as a high risk population gaining benefit from adjunctive treatment with glycoprotein IIb/IIIa receptor antagonists and early reperfusion by coronary interventions.

Increased plasma levels of soluble selectins in patients with unstable angina.

BACKGROUND: Inflammation plays an important role in the pathogenesis of unstable angina. Adhesion molecules, such as selectins, mediate the interactions between leukocytes, platelets and endothelial cells during inflammation and thrombogenesis. HYPOTHESIS: The purpose of this study was to determine whether soluble E-selectin, P-selectin and L-selectin levels are increased in patients with unstable angina (UA). METHODS: Soluble E-, P- and L-selectin levels were measured by enzyme-linked immunoassay in the peripheral blood of 23 patients with UA, 26 patients with stable angina (SA) and 15 cont

[Simultaneous surgery for unstable angina and gastric cancer: a case report].

This report describes a case in which an 81-year-old male underwent two operations simultaneously for unstable angina and gastric cancer. Successfully performed procedures were off pump CABG with bilateral IMA, and total gastrectomy. The post operative course was uneventful. Off pump CABG is an effective procedure in patient with malignant neoplasm.

Unstable angina: the first 48 hours and later in-hospital management.

Unstable angina is a common cardiovascular condition associated with major adverse clinical events. Over the last 15 years, therapeutic advances have dramatically reduced the complication and mortality rates of this serious condition. The standard of therapy in patients with unstable angina now incorporates the combined use of potent antithrombotic (aspirin, clopidogrel, heparin and glycoprotein IIb/IIIa receptor antagonists) and anti-anginal (beta-blockade and intravenous nitrates) regimens complemented by the selective and judicious application of coronary revascularisation strategies. [Correlation of coronary angiographic morphology with clinical presentation in unstable angina].

OBJECTIVE: To seek the relation between clinical presentation (Braunwald classification) or electrocardiogram(ECG) and angiographic morphology in patients with unstable angina (UA) and to determine which clinical markers could reliably predict unstable lesions and in-hospital cardiac events. METHODS: 120 hospitalized patients with a clinical diagnosis of UA and an abnormal angiograms(>or=50% diameter stenosis) were selected. Patients were classified according to Braunwald criteria before cardiac catheterization was performed. Coronary arteriograms were reviewed. The lesion morphology (includi

Plasma lipoprotein(a) levels and fibrinolytic activity in patients with unstable angina.

The relationship between plasma Lipoprotein(a) (Lp(a)) levels and plasminogen activator inhibitor (PAI) activity and tissue plasminogen activator (t-PA) antigen levels were studied in 15 patients with unstable angina. Plasma levels of Lp(a) (mg/dl) were significantly higher in patients with unstable angina after treatment 2 weeks than on admission (19.7 +/- 2.8 vs 14.6 +/- 2.3, p < 0.01). On the other hand, the plasma levels of PAI activity (IU/ml) and t-PA antigen (ng/ml) in patients with unstable angina were significantly higher on admission than after treatment (PAI activity: 11.4 +/- 1.4 v

Thrombolytic therapy in refractory unstable angina: the role of Holter monitoring.

We tested the safety and the usefulness of intravenous urokinase (2 million units administered over 30 min) in 44 patients with refractory unstable angina, defined as persistence of ischemic episodes during 48-h Holter monitoring (Phase 1) despite maximal medical therapy. After thrombolysis, recurrence of ischemia was observed during a week of observation in the CCU, including two 24-h Holter monitorings at the beginning and the end of the week (Phase 2). Seventeen patients completed the observation period without either symptomatic or asymptomatic ischemic episodes (Group A); the remaining 27

A randomized study comparing propranolol and diltiazem in the treatment of unstable angina.

One hundred consecutive patients hospitalized in the coronary care unit for unstable angina, excluding patients with Prinzmetal's variant angina, were randomized within 24 hours of admission to treatment with diltiazem (50 patients) or propranolol (50 patients). Also excluded were patients with previous coronary artery bypass surgery and those receiving a beta-receptor blocking agent at the time of hospital admission. Left ventricular function and the extent of coronary artery disease were similar in the two groups. During the hospital stay, the number of chest pain episodes decreased from a m

Aberrant main left coronary artery in a patient with unstable angina.

A patient with significant obstructive coronary artery disease in an anomalous left main coronary artery arising from the right coronary artery is reported. The combination of this coronary anomaly with main left atherosclerotic disease is noteworthy and presented no surgical technical problem.

Nifedipine in the treatment of unstable angina, coronary spasm and myocardial ischemia.

The effects of nifedipine, a potent calcium antagonist, were studied in patients with unstable angina, coronary spasm and myocardial ischemia. Data from two separate groups of patients studied in the cardiac catheterization laboratory indicate that intracoronary injection of nifedipine promptly reversed coronary spasm--whether provoked or spontaneous--in five of six patients. In other patients, direct intracoronary injection of the drug was compared with intravenous administration. After intracoronary injection, local mechanical cardiac action virtually ceased, and the ventricular wall became
