The eye in sleep apnea syndrome.

Sleep apnea syndrome (SAS) is a disease characterized by recurrent complete or partial upper airway obstructions during sleep. The majority of patients with SAS demonstrate this obstruction either at the nasopharynx or the oropharynx. Risk factors for SAS include obesity, male gender, upper airway abnormalities, alcohol use, snoring, and neck girth of more than 17 in. in men or 16 in. in women. Reported ophthalmic findings in patients with SAS include floppy eyelid syndrome (FES), glaucoma, and non-arteritic anterior ischemic optic neuropathy (NAION).

Control of breathing and neonatal apnea.

Great strides have been made in our understanding of developmental respiratory neurobiology. A clear picture is, therefore, emerging of the physiological mechanisms that underlie apnea of prematurity. The ventral surface of the medulla and adjacent areas play a key integrative function for central CO2 chemosensitivity and modulation of afferent inputs from peripheral chemoreceptors and laryngeal afferents. Maturational change in medullary neurotransmitter function appears to contribute to the physiological events that characterize apnea of prematurity. Despite this greater scientific insight, …

[Sleep apnea syndrome in the elderly].

INTRODUCTION: Sleep apnoea/hypopnoea syndrome (SAHS) is distinct in the elderly as age-related co-morbidity may be aggravated by sleep-related breathing disorders or may mask typical clinical symptoms. STATE OF ART: The SAHS prevalence in the elderly varies from one study to another, with an average rate of 25% that might increase according to the importance of associated pathologies. Clinical symptoms tend to be the neurological manifestations of excessive daytime sleepiness and cognitive deterioration. PERSPECTIVES: Treatment of SAHS should include weight reduction strategies if obesity is

Sleep apnea and quality of life.

OBJECTIVE: To investigate the effects of sleep apnea (SA) on the quality of life (QOL). DESIGN: A prospective study of QOL in patients with and without SA as defined by an apnea-hypopnea index (AHI) >5. SETTING: University-based outpatient clinics. PATIENTS: Primary care patients followed in a general internal medicine clinic as well as those referred to a sleep disorders clinic at the University of Wisconsin Hospital and Clinics were consecutively recruited and classified into 3 groups of subjects: (1) patients without SA (AHI<5) (n=46), (2) patients with mild SA (AHI 5-15) (n=16), and (3)…

[Sleep apnea and the work site].

The sleep apnoea syndrome (SAS) is characterised by somatic, in particular cardiopulmonary and psychosocial, symptoms, the latter severely impairing the patient's social life. Excessive daytime somnolence and the resulting problems severely stress patients with SAS. Among 24 patients with SAS, 79.2% stated that they fell asleep during the day, frequently in 41.8% and occasionally in 37.4%. In addition, 70.8% of the patients with SA did not feel adequately rested on waking in the morning. Of the 24 patients, 17 had a job, and 11 of these experienced difficulty staying awake at the workplace. Su

Evaluation of the adult with sleep apnea.

Twenty-seven cases of adult sleep apnea are reported. The patients were studied with all-night polysomnography. Seven (26%) had predominately obstructive sleep apnea, while another seven (26%) had predominantly central sleep apnea. Thirteen patients (48%) were found to have a mixed pattern. Obesity was not a reliable guideline for predicting differential etiology. We conclude that sleep studies with specific monitoring are essential in the evaluation of the adult with sleep apnea prior to therapeutic intervention.

Astrocytoma in an infant with prolonged apnea.

A 4-week-old infant experienced prolonged central sleep apnea requiring resuscitation. At 6 months of age he developed episodic obstructive apnea (diagnosed as laryngospasm by direct laryngoscopy) with an abnormal EEG and a normal computed tomography (CT) scan, and at 14 months of age he developed hemiparesis due to a gemistocytic astrocytoma grade III-IV. Following removal of the mass, he has had occasional seizure activity, but no further episodes of obstructive apnea.

Treatment of apnea of prematurity with aminophylline.

The effectiveness of aminophylline in the treatment of apnea of prematurity was evaluated in 13 premature infants (mean birthweight, 1.13 kg; mean gestational age, 29 weeks). Apnea was recorded by direct observation in combination with impedance monitoring. Rectal suppositories of aminophylline (5 mg) were given at six-hour intervals. The average dose was 4.1 mg/kg. No toxicity or complications were noted. The parents became free of apneic episodes during therapy. The response for each eight-hour interval of treatment over 72 hours when compared to pretreatment was significant (P less than .01…

Comparative analysis of cardiac autonomic dysfunction between apnea-predominant and hypopnea-predominant obstructive sleep apnea: a study matched for age, sex, and apnea-hypopnea index : Short running title: cardiovascular risk of apnea vs. hypopnea in OSA.

PURPOSE: Comparative knowledge of cardiac autonomic dysfunction between apnea and hypopnea events is limited. This study aimed to investigate the differences in cardiac autonomic dysfunction between apnea-predominant and hypopnea-predominant in patients with obstructive sleep apnea (OSA). METHODS: This was a single-center retrospective study conducted at Kangwon National University Hospital. Patients were selected based on standard polysomnographic diagnostic criteria for OSA. Patients were categorized into apnea- or hypopnea-predominant groups according to the proportion of apneas within apne…

Evaluation of an automated sleep apnea scoring algorithm via the Wesper Lab home sleep apnea test.

This study evaluated the performance of the Wesper Lab home sleep apnea test (HSAT) artificial intelligence (AI) automated scoring algorithm under both in-laboratory and real-world conditions. We conducted a multi-tiered validation using two datasets and three analyses. The primary analysis compared apnea-hypopnea index (AHI) and central apnea index (CAI) from Wesper Lab HSATs with simultaneous polysomnography (PSG) scored by blinded technologists (n = 44). The secondary analysis evaluated blinded scoring of raw Wesper Lab signals from the same 44 patients: first by a single scorer, then by tw…

Impact of mean apnea-hypopnea duration on disease severity in obstructive sleep apnea: A retrospective study.

BACKGROUND: The mean of apnea hypopnea duration (MAD) may reflect obstructive sleep apnea (OSA) severity, but its clinical relevance remains unclear. Our aim was to assess the impact of MAD on OSA severity. METHODS: A retrospective study (2015-2022) was conducted in a pneumology department, including 521 patients diagnosed with OSA by ventilatory polygraphy. Patients were divided by the median MAD into group 1 (≥median, n = 264), and group 2 (<median, n = 257). RESULTS: The frequency of severe OSA was comparable between the two groups (p = 0.1). Patients in G1 had a lower minimum oxygen satu…

Predictors of Obstructive Sleep Apnea on a Home Sleep Apnea Test After a Negative Attended Polysomnography.

STUDY OBJECTIVES: A home sleep apnea test (HSAT) is an acceptable alternative to polysomnography (PSG) for the diagnosis of obstructive sleep apnea (OSA) in patients with high pretest probability without certain comorbidities, such as severe pulmonary disease, congestive heart failure, or neuromuscular weakness. Current guidelines recommend repeat in-laborataory PSG in those with an initial negative PSG and high clinical suspicion for OSA. This retrospective study evaluated predictors of OSA on HSAT in patients who had a negative PSG. METHODS: Electronic medical records were reviewed on 206 p…

Case Study: Imaging of Apnea Termination in a Patient with Obstructive Sleep Apnea during Natural Sleep.

A 55-year-old woman who presented to the sleep clinic with severe sleep apnea (OSA) (apnea-hypopnea index [AHI] 62) and excessive somnolence (Epworth Sleepiness Scale score 18/24), was imaged with MRI using the Spatial Modulation of Magnetization tagging sequence awake and asleep to visualize upper airway tissue movement. Awake quiet breathing resulted in minimal movement of upper airway tissues. Asleep sequences taken during airway opening post-apnea demonstrated neck extension, mandibular advancement, and widespread tongue deformation accompanying contraction of genioglossus. At the end of t…

The emergence of central sleep apnea after surgical relief of nasal obstruction in obstructive sleep apnea.

By the current definition, complex sleep apnea (CompSA) refers to the emergence of central sleep apnea (CSA) during the treatment of obstructive sleep apnea (OSA) with continuous positive airway pressure (CPAP). However, new-onset CSA has been described with use of other treatments for OSA, including tracheostomy, maxillofacial surgery, and mandibular advancement device. We present a patient with CSA beginning after endoscopic sinus and nasal surgery for nasal obstruction in the setting of mild OSA. This case highlights the importance of non-PAP mechanisms in the pathogenesis of CompSA.

The characteristics of five patients with obstructive sleep apnea whose apnea-hypopnea index deteriorated after uvulopalatopharyngoplasty.

The objective of this retrospective, consecutive, case series design study was to determine the number of unselected patients with obstructive sleep apnea (OSA) who deteriorated after uvulopalatopharyngoplasty (UPPP). Sixteen of 27 patients at the Sleep Clinic at Veterans Affairs Medical Center who underwent UPPP for OSA and who completed both a pre- and postpolysomnogram were studied. After comparing the apnea-hypopnea index (AHI) before and after UPPP, three groups of patients were identified: deteriorators, unchanged, and improvers. In five patients (31%), the AHI increased by more than 10%…

[Expert consensus for the use of home sleep apnea test in the diagnosis of obstructive sleep apnea in adults].

With the advancement of home sleep apnea test (HSAT) technology and the increase of encouraging evidence of home-based management approach for obstructive sleep apnea (OSA), HSAT devices using different types of biosignal acquisition technology are more and more widely used for the diagnosis of OSA in adults. HSAT for the diagnosis of OSA should be performed in combination with a comprehensive clinical and sleep evaluation. HSAT can be used as a diagnostic tool for uncomplicated OSA with a high pretest probability of moderate to severe OSA. A negative HSAT result cannot exclude the diagnosis o…
