INTRODUCTION: In vestibular schwannoma (VS) surgery, the arachnoidal duplication, based on an epiarachnoidal origin of the tumor, is reputedly induced by medial growth of tumor and helpful in atraumatic dissection. This study was intended to verify the epiarachnoidal origin of VS.
MATERIALS AND METHODS: We studied 49 human temporal bones (TBs) specimens. Twenty-two TBs from 18 patients with VS were selected. An additional series of 27 TBs without any tumor within the internal auditory meatus were also included. We identified the location of the meninges and the position of the transition zone…

BACKGROUND AND PURPOSE: Most arachnoid cysts (ACs) are asymptomatic, but some of them cause mass effect, mainly in adults. Surgery of ACs is controversial, especially regarding the choice of the best method of treatment. The authors present short- and long-term outcome of endoscopic surgery of ACs.
MATERIALS AND METHODS: There were 19 adults with symptomatic ACs treated surgically in the last 6 years. 16 cysts were located supratentorially, and 3 infratentorially. Cystocisternostomy was carried out in 7 patients, cystocisternostomy with catheter implantation was performed in 5 patients, cysto…

An understanding of the microsurgical anatomy of the arachnoid membranes and the subarachnoid cisterns is important in minimally invasive neurosurgery. But the topography of the arachnoid membranes has not been completely elucidated. The description of the distribution and the configuration of the intracranial arachnoid membranes is still a subject of controversy. In order to clarify this we examined eight Han Chinese adult human cadavers under an operating microscope. The dissections were performed with microsurgical instruments and techniques without staining of the intracranial structures n…

BACKGROUND: To report a case of an arachnoid cyst of the optic nerve with homonymous optic atrophy.
HISTORY AND SIGNS: A 17-year-old female patient presented with severe visual loss, limited to light perception, of the left eye. She underwent complete ophthalmologic examination, color fundus photography and electrophysiological study. Magnetic resonance imaging (MRI) revealed the presence of a fusiform structure involving the left optic nerve from the orbital apex to the posterior surface of the globe. The clinical and radiographic features are consistent with an arachnoid cyst of the optic n…

Spinal arachnoiditis is one of the common and disabling complication of tuberculous meningitis (TBM). We focused on early diagnosis of spinal arachnoiditis by spinal MRI in asymptomatic patients in whom neurological examination was normal. We studied 16 patients with a diagnosis of probable or highly probable TBM with symptoms for less than 1 month; three had radiological evidence of spinal arachnoiditis. High cerebrospinal fluid protein appeared to be a risk factor for development of spinal arachnoiditis. MRI is sensitive to detect early spinal arachnoiditis. Earlier diagnosis may be helpful …

The paper presents the clinical picture, diagnostic and therapeutic management of suprasellar arachnoid cysts that result in isosexual precocious puberty in children. The authors stress the importance of physical examinations involving routine assessment of growth and sexual development as well as neurological evaluation in early diagnosis of suprasellar arachnoid cysts and other central causes of precocious puberty. The role of early diagnosis in preventing irreversible somatic and mental changes is emphasized.

The aim of the present study was to investigate whether data on location and distribution of intracranial cysts in a large patient population may explain why and how such cysts are formed. We investigated 123 patients with 129 intracranial cysts, consecutively admitted to the Department of Neurosurgery in Bergen 1988-97. Data were analyzed with regard to intracranial location and gender distribution. Cysts were much more commonly located in the temporal fossae than one would expect if the distribution were random; 68.1% of patients had temporal cysts. We suggest a theory that may explain how m

The incidence of interhemispheric cyst is rare. There have been only 12 cases reported in adults since the advent of CT. We encountered a case of an interhemispheric arachnoid cyst in a 54-year-old patient who had developed paraparesis. Excision of the cystic wall produced a satisfactory result. The CT and MRI scans were not contributory to histological differentiation of the lesions. According to the literature, however, it is highly likely that these patients are suffering from an arachnoid cyst, when the interhemispheric cyst without agenesis of the corpus callosum occurs in adults.

BACKGROUND: Arachnoid cysts located within the interhemispheric fissure are a matter of discussion as to the surgical indication and the choice of the most appropriate surgical procedure. In fact, in spite of the large dimensions that they can reach, the clinical manifestations may remain subtle and apparently stable over the years. On the other hand, the larger utilization of prenatal ultrasound investigations has increased the number of lesions detected before they can provoke clinical manifestations. As far as surgery is concerned, two main options are available-an extrathecal shunting proc…

We describe a 5-month-old boy with bilateral optic nerve hypoplasia associated with visual impairment, growth hormone deficiency and an arachnoid cyst in the right temporal lobe on MRI. Optic nerve hypoplasia is sometimes associated with agenesis of the septum pellucidum, commonly known as septo-optic dysplasia, and/or hypopituitarism. There have been a few reports about its association with other abnormalities of the central nervous system, but none about that with an arachnoid cyst.

A patient is reported with a painless, progressive cauda equina lesion due to arachnoiditis, the result of the contrast medium 'Thorotrast' (thorium dioxide) introduced into the brain over 30 years previously. Contrast medium introduced into the lumbar spine can give rise to aseptic adhesive inflammation (arachnoiditis). It rarely gives rise to clinical problems but, when it does so, is usually associated with back pain and only very rarely with progressive neurological deficit.

An ultrastructural comparison of arachnoid villi and meningiomas was made in man. Human arachnoid villi basically consisted of four portions: fibrous capsule; arachnoid cell layer; cap cell cluster; and central core. Arachnoid cells had many characteristic structures in common with meningioma cells: plasmalemmal interdigitations; desmosomes; hemidesmosome-like junctions; and cytoplasmic filaments. Both arachnoid villi and meningiomas were characterized by the numerous extracellular cisterns which appeared to form outflow channels of cerebrospinal or serum fluid. Differences between arachnoid v…

A case of optochiasmatic arachnoiditis mimicking Foster-Kennedy syndrome is reported. The difficulties in establishing diagnosis are considered, and the literature of optochiasmatic arachnoiditis presenting in this unusual manner is reviewed.

The authors report five patients with spinal stenosis who had a total myelographic block at the level of the obliterated subarachnoid space. Arachnoiditis had not been considered as a primary diagnosis until laminectomy revealed a non-pulsating, thickened dural sac that conformed to the internal configuration of the involved spinal canal. Two patients had stenosis complicated by spondyloarthrosis over multiple lumbar levels, one had a previous spinal fusion, another had degenerative spondylolisthesis, and the fifth had a large midline extruded disc at L2-3 that completely blocked the spinal ca…
