Arthritis is a common condition affecting millions of people in the UK and worldwide. All members of the dental team will be seeing and treating patients with this condition, and it is important to understand the disease, its management and medications used. This article outlines the five most common types of arthritis: osteoarthritis (OA), rheumatoid arthritis (RA), axial spondyloarthritis (sPA), juvenile idiopathic arthritis (JIA), and gout. It further discusses the medications used to treat these conditions and any dental implications. Finally, advice is offered on considerations for the de

Musculoskeletal ultrasonography (US) is an established and validated imaging technique in rheumatology. Ultrasonography is able to directly visualize soft tissue pathologies such as synovial tissue changes. Pathological findings in superficial cartilage, bone lesions and synovial tissue changes in the context of rheumatoid arthritis, spondyloarthritis or crystal arthropathies may only be seen by sonography or detected earlier by ultrasonography compared to conventional imaging techniques. The activity of an inflammatory arthropathy can be visualized using Doppler and power Doppler US. US is he

In the present report the authors describe four cases with megacolon and arthritis. The etiology of these unique associations is known in only one case. The musculoskeletal pictures belong to the group of seronegative spondyloarthritis. A huge resistance with great scibalas was detected in the left iliac region by physical examination in all cases. Surgical procedures of the colon resulted in complete remission of arthritis in one case, in the others, chronic obstipation with intermittent relapse of arthritis persisted.

Obesity affects over 20% of the UK's adult population and its prevalence is rising. Obesity can lead to a variety of musculoskeletal problems and is independently associated with locomotor disability and joint pain. Obesity increases the risk of radiographic knee osteoarthritis (OA), but has a lesser effect on disease progression. The association with hip and hand OA is weaker, but implies that excess adipose tissue produces humoral factors, altering articular cartilage metabolism. It has been postulated that the leptin system could be a link between metabolic abnormalities in obesity and incr

Joint disorders are common in the elderly population. Commonest conditions are rheumatoid arthritis, osteo-arthritis and crystal induced arthritis. Comorbid events in the elderly patients make disease management even more difficult. Some tests are needed to distinguish these conditions from more serious diseases like carcinomatous arthritis or multiple myeloma. Management strategies are usually similar to that of younger onset arthritis with dose adjustment of drugs due to underlying age related hepatic and renal dysfunctions.

Indices of angiogenesis are increased in synovia from patients with arthritis, and vascular proliferation may contribute to the pathogenesis of synovitis, pannus growth, bone and cartilage destruction, and osteophyte formation. Pharmacological inhibition of angiogenesis therefore has potential as a therapeutic strategy in human arthritis. However, vascular growth is also essential for normal development, female reproduction and tissue repair. Selective inhibition of undesirable angiogenesis requires an understanding of the different regulatory mechanisms in pathological and physiological angio

Arthritis has a fascinating relationship with hepatitis of diverse origins. Either condition can predominate and the correct diagnosis may be elusive, especially in the early stages. Humoral autoimmune mechanisms appear to serve as a bridge connecting the two target organs, whether the syndromes are caused by viral infections or unspecified etiologies. Maintaining a keen awareness of and being able to recognize the clinical arthritis-hepatitis syndromes are critical, enabling the physician to establish accurate diagnoses, to prescribe the correct therapies, and to predict outcomes.

Choice of diet is one way in which an individual can influence his/her own health, and it is to be expected that patients will seek their physician's expert opinion regarding dietary matters. Respect for the legitimacy of these enquiries and balanced informed discussion, which includes general advice for a prudent diet, as well as disease-specific recommendations when indicated, can be the key to a productive relationship between patients and physician. The issue of dietary advice has an impact on the management of most forms of arthritis including osteoarthritis (obesity/energy balance), gout

Thirty-four patients (24 females and 10 males) selected from 300 consecutive patients with established systemic sclerosis (SSc), with a current or past history of articular symptoms, were clinically documented and further studied using thermography and bone scan to define the pattern of arthritis. Clinical evidence of synovitis was observed in 30 (88%) and joint inflammation was detected in 31 (91%) by the above-mentioned imaging techniques. A distinctive subset of 10 patients with deforming arthritis was characterized in which seven (70%) patients fulfilled criteria for both rheumatoid arthri

Familial clustering is a common feature of many different arthritic conditions which can be explained by shared genetic or environmental influences or a combination of these. The aetiology of some of these diseases (e.g. multiple epiphyseal dysplasia, Lesch-Nyan syndrome) is clearly monogenic but the majority of common rheumatic conditions, such as osteoarthritis (OA) and rheumatoid arthritis (RA) are complex and multi-factorial with a polygenic component. In the last 10 years there has been spectacular progress in unravelling the underlying genetic mechanisms of many monogenic disorders inclu

An inflammatory polyarthritis has been previously described in leprosy but accounts of the clinical characteristics have varied. All patients with joint symptoms admitted to a leprosy centre over 5 months were examined by a rheumatologist. Of 48 acute admissions, 20 (42%) had a symmetrical polyarthritis affecting the wrists and fingers. This was strikingly similar to RA in appearance. The arthritis occurred exclusively in patients with reactions mainly during anti-mycobacterial treatment. The clinical features of the joint disorder were identical in Type I (associated with alterations in immun

Joint problems in adolescence are often monoarticular. In the acute form the differentiation between trauma and infection is of utmost importance, whereas in chronic joint diseases the differentiation will be mainly between mechanical disorders and true rheumatic diseases. In pauci- or polyarticular arthritis of short duration viral infections have to be considered, since true rheumatic diseases tend to run a more chronic course. In girls true sero-positive juvenile rheumatoid arthritis typically appears in this age group. In adolescent boys seronegative Spondylarthropathies involving the lowe

The term "enteropathic arthritis" describes joint manifestations that occur in conjunction with gastrointestinal disease. Underlying causes include inflammatory bowel disease, reactive arthritis, iatrogenic disease. Whipple's disease, and other gastrointestinal diseases. A possible association between Reiter's syndrome and human immunodeficiency virus infection also has been reported. These arthritic syndromes can be treated symptomatically, but long-term therapy should be directed at the underlying cause. In most cases, prognosis for survival and joint function is good.

Arthritis in children may be caused by many diseases and disorders. Some processes may be aborted by early diagnosis and treatment. Others may not cause true arthritis until the adult years, with a variable latent period. The spectrum includes congenital malformations, neoplasia (benign and malignant), infection, trauma, hematologic/vascular disorders, and connective tissue diseases. In all instances, early diagnosis is important. The growing skeleton has great potential to remodel if it is given adequate time.

The categorization in four classes of association between infection and arthritis (namely infective, post-infective, reactive and idiopathic) seems nowadays to be inadequate to cover the extensive field of interactions between infectious agents and host response resulting in arthritis. This paper is a synthetic review of the subject with particular reference to pathogenetic mechanisms in children. An effort has been accomplished, on the basis of the most recent literature, to define the respective roles of the microbial aggression and the host response in a number of conditions: septic arthrit

Arthritis in children may result from many conditions. The rheumatic diseases, including juvenile rheumatoid arthritis, the spondyloarthropathies, rheumatic fever, lupus erythematosus, dermatomyositis, and the various vasculitis syndromes all can cause arthritis; these diseases are distinguished by their characteristic clinical appearances. Several nonrheumatic disorders such as infections, malignancies, congenital or genetic conditions, orthopedic conditions, and psychological disorders may closely simulate the various rheumatic diseases. The evaluation of children with arthritis rests chiefl
