Listeria-associated arthritis in a patient undergoing etanercept therapy: case report and review of the literature.

Listeriosis can be a cause of infectious arthritis. Here, we present a case of articular listeriosis in a patient with rheumatoid arthritis receiving treatment with etanercept, a tumor necrosis factor antagonist. We review the literature of articular listeriosis and discuss the role of tumor necrosis factor blockade in precipitating listeriosis.

Diagnostic challenges in pediatric arthritis: A case of systemic juvenile idiopathic arthritis misdiagnosed as septic arthritis in a 3-year-old boy.

Arthritis in children has several causes, including infectious, inflammatory, and infiltration of malignant cells, and it is important to differentiate them as soon as possible. We present the case of a 3-year-old boy who was initially diagnosed with septic arthritis secondary to joint pain, swelling, and high fever. Despite appropriate antibiotic therapy, the patient's symptoms persisted, leading to an eventual diagnosis of systemic juvenile idiopathic arthritis (sJIA). This case highlights the importance of distinguishing between septic arthritis and sJIA because early misdiagnosis can lead

Differentiating Transient Synovitis of the Hip from More Urgent Conditions.

A variety of traumatic and infectious processes can cause acute onset of hip pain and difficulty walking. Without a history of trauma, a common cause is transient (or "toxic") synovitis, but serious infectious causes, such as septic arthritis of the hip or osteomyelitis, must be ruled out. Differentiating between septic arthritis of the hip (a true emergency) and transient synovitis of the hip (a much more benign inflammatory process) can be difficult. The Kocher criteria have proven to be helpful in making the distinction between these two conditions, and they are reviewed in this article. In

Familial Mediterranean fever occurring in an elderly Japanese woman with recent-onset rheumatoid arthritis.

A 60-year-old woman with a two-year history of rheumatoid arthritis (RA) developed recurrent two- to three-day attacks of fever (>38 °C) accompanied by monoarthritis of the right hip joint. The first attack occurred two months after beginning anti-tumor necrosis factor-α therapy. Since a diagnosis of infectious arthritis was suspected, the therapy was discontinued. Thereafter, the patient repeated similar episodes; however, oral colchicine effectively controlled the attacks. The patient was diagnosed to have familial Mediterranean fever (FMF). The clinical manifestations of FMF mimic infectiou

Pneumococcal keratitis, bacteremia, and septic arthritis in an asplenic patient.

We report the case of a 66-year-old black woman who presented with concomitant acute infectious keratitis, bacteremia, and septic arthritis caused by Streptococcus pneumonia. The septic arthritis resolved rapidly with surgical drainage and intravenous antibiotics, but despite aggressive topical and intravenous antibiotic therapy for the infectious keratitis, the cornea perforated, the patient developed endophthalmitis, and the eye eventually was eviscerated. To the best of our knowledge this is the first reported case of this nature. This patient had undergone splenectomy > 50 years prior to d

Clinical suspicion is key: an unusual presentation of septic arthritis after distal pancreatectomy.

Septic arthritis is the result of an infectious agent gaining access a sterile joint. This results in a devastating inflammatory response that leads to rapid destruction of intra-articular cartilage and with it significant morbidity. This case study reports an unusual presentation of septic arthritis following abdominal surgery; specifically, a distal pancreatectomy performed for an enlarged, mid-body pancreas mass involving the splenic artery. This is the first reported case of septic arthritis following abdominal surgery, though the exact etiology is unknown.

[Acute monarthritis].

Common causes of acute monarthritis are osteoarthritis, an acute attack of gout or other crystal arthropathies in older patients or a reactive arthritis or gonococcal arthritis in younger patients. The differential diagnosis should primarily rule out septic arthritis. An arthrocentesis with a microbiological work up and a fresh specimen should be prepared for the diagnosis of crystal arthropathies and if an infectious origin is suspected or if nothing remarkable was found during the primary examination. Further diagnosis und therapy should be performed by a specialist.

Isolated sternoclavicular joint arthritis in heroin addicts and/or HIV positive patients: three cases.

The authors describe three patients in whom septic arthritis of the sternoclavicular joint (SCJ) occurred, drug addiction and human immunodeficiency virus (HIV) infection representing the predisposing conditions. Infectious arthritis is well known in intravenous drug users, but it is rare in HIV positive patients, who are prone to bacterial infections from usual or unusual microorganisms. In one case, staphylococcus aureus methicillin sensitive was responsible for septic arthritis. In another case, SCJ infection was associated with pneumonitis.

Escherichia coli Sternoclavicular Septic Arthritis Secondary to Colon Cancer-associated Bacteremia.

Non-traumatic sternoclavicular arthritis is typically caused by either degeneration or non-infectious inflammatory conditions, although septic arthritis can often occur due to Staphylococcus aureus, particularly in immunocompromised individuals. We herein present a case of Escherichia coli (E. coli)-induced sternoclavicular arthritis in a Japanese woman with no underlying disease. As no common source of E. coli bacteremia was identified, colonoscopy revealed early-stage rectal cancer. To our knowledge, this is the first report of septic sternoclavicular arthritis in which colorectal cancer was

Infection versus disease activity in rheumatoid arthritis: eight years' experience.

Septic arthritis is a serious and sometimes fatal complication of rheumatoid arthritis. We have examined the clinical characteristics of 16 patients with infectious arthritis seen during an eight-year period. This represented 0.5% of all admissions to our hospital for patients with rheumatoid arthritis. Although rheumatoid arthritis is considered a predisposing factor for joint sepsis, 15 of our patients had other conditions that most likely increased their susceptibility to infection. Many patients lacked distinctive features of joint sepsis (fever, chills) and only one half had leukocytosis.

Chronic Chikungunya Arthritis and Rheumatoid Arthritis: What They Have in Common.

Chikungunya virus (CHIKV) is a single-stranded RNA virus belonging to the family Togaviridae and genus Alphavirus that causes an acute febrile illness, chikungunya fever, which is transmitted to humans by Aedes species mosquitoes. During acute illness, patients have high fever, polyarthralgias or polyarthritis, maculopapular rash, headache, and myalgia that lasts for days to weeks. Following resolution of acute infection, a significant proportion of patients develop chronic chikungunya arthritis that can resemble rheumatoid arthritis. In this review, we first consider the historical background

A case of hip joint septic arthritis due to haematogenous infection, which was misdiagnosed sternoclavicular joint septic arthritis as SAPHO syndrome.

Anterior chest pain associated with sternoclavicular arthritis has been considered a symptom specific to SAPHO syndrome. Differentiating aseptic arthritis of the SAPHO syndrome from infectious sternoclavicular joint (SCJ) arthritis is often difficult. We reported a 55-year-old woman with left hip joint and right SCJ pain. Diagnosis and treatment were delayed because she was misdiagnosed with SAPHO syndrome, and haematogenous infection spread to the hip joint septic arthritis. She was diagnosed promptly after presenting to our hospital and underwent early debridement, and her hip joint and SCJ

Radiotracers for Imaging of Inflammatory Biomarkers TSPO and COX-2 in the Brain and in the Periphery.

Inflammation involves the activation of innate immune cells and is believed to play an important role in the development and progression of both infectious and non-infectious diseases such as neurodegeneration, autoimmune diseases, pulmonary and cancer. Inflammation in the brain is marked by the upregulation of translocator protein (TSPO) in microglia. High TSPO levels are also found, for example, in macrophages in cases of rheumatoid arthritis and in malignant tumor cells compared to their relatively low physiological expression. The same applies for cyclooxgenase-2 (COX-2), which is constitu

Reactive Arthritis.

Reactive arthritis is classified as a spondyloarthropathy. Current concepts of disease suggest an infectious trigger, followed by inflammatory arthritis. Several mechanisms have been proposed to explain the interaction of host susceptibility and microorganism. Diagnosis relies on a compatible clinical syndrome and microbiologic confirmation of the pathogen. Antibiotic therapy seems useful in Chlamydia-triggered arthritis. The role of antibiotics in arthritis triggered by enteric pathogens is less clear. The role of tumor necrosis factor alpha inhibitors in therapy is evolving. Many patients ha

Acupuncture-associated Listeria monocytogenes arthritis in a patient with rheumatoid arthritis.

Septic arthritis is a rare complication of acupuncture. We present a patient with rheumatoid arthritis who developed septic arthritis of the right knee after consecutive weekly sessions of acupuncture therapy for 3 weeks. The infection was localized by musculoskeletal sonography and magnetic resonance imaging, with culture of the synovial fluid aspirated from the joint yielding Listeria monocytogenes. The patient responded well to antibiotic treatment and regained joint mobility. A high index of suspicion for an infectious process is required for prompt diagnosis and treatment of acupuncture-i
