BACKGROUND: As many as 1.5 billion people around the world harbour Ascaris lumbricoides in the digestive tract. Gallbladder ascariasis, although less common than bile duct ascariasis, is quite often seen in endemic areas.
METHODS: Some 1300 patients with hepatobiliary ascariasis, admitted between October 1992 and June 1998, were analysed prospectively. The clinical features and outcome of 56 cases of gallbladder ascariasis are presented.
RESULTS: Forty-seven patients were diagnosed by ultrasonography and nine were diagnosed at laparotomy. Only ten patients diagnosed by ultrasonography expell…

Two patients with right upper quadrant pain underwent real time ultrasound examination and biliary ascariasis was diagnosed. One case was confirmed by endoscopic retrograde cholangiopancreatography with removal of the parasites, and the other by intravenous cholangiography. Patients with ascariasis in the bile duct occasionally presenting with atypical right upper quadrant abdominal pain can be diagnosed accurately by real time ultrasound, which shows the movement of the worms.

In a prospective 6-month study, sonographic diagnosis of biliary ascariasis was made in 14 patients; in nine patients, the diagnosis was confirmed by endoscopic retrograde cholangiopancreatography and bile analysis, while in five patients, confirmation was obtained by proven intestinal infestation. No false-positive or false-negative cases were encountered. Ascariasis of the biliary tract was seen on ultrasound as echogenic, non-shadowing images, either as single or multiple strips, with visualization of the digestive tract of the worm as an anechoic inner tube, or as amorphous fragments. Foll…

Ascaris lumbricoides is the most common roundworm in warm and temperate areas. Although radiological features of Ascaris lumbricoides are well defined, there are only a few reports on the ultrasonographic findings of intestinal ascariasis. In our two patients with no acute abdominal symptoms, intestinal ascariasis was initially diagnosed by abdominal ultrasonography. In both patients, tubular structures in the segments of the small intestine were demonstrated by ultrasonography.

Ascaris lumbricoides is a worldwide intestinal infestation that may cause various complications. Biliary ascariasis, however, is a rare condition. We describe a child with biliary ascariasis. The patient's clinical symptoms were pain, vomiting and abdominal tenderness, and she was thought to have acute appendicitis. However, laboratory examination revealed high serum alkaline phosphatase and amylase levels, and ultrasonography and percutaneous cholangiography demonstrated biliary ascariasis. The patient was successfully treated with mebendazole and antispasmolytic drugs.

A 60-year-old male suffered from abdominal pain, chills and fever for two weeks. A vivid Ascaris lumbricoides in the common bile duct and intrahepatic ducts was identified in operation. Chronic cholecystitis was also noted. We discuss the findings of multiple image modalities in biliary ascariasis and review the literature.

A 52-year-old man presented with obstructive jaundice. Sonography of the liver disclosed dilatation of intrahepatic ducts in which multiple elongated intraluminal lesions were contained due to biliary ascariasis. A localized mass projecting into the common bile duct and causing its obstruction proved to be a coexistent cholangiocarcinoma.

A case is presented of persistent gallbladder infestation by Ascaris lumbricoides that was successfully treated by laparoscopic cholecystectomy. The management of this rare manifestation of ascariasis is reviewed.

Eight cases of hepatobiliary invasion of Ascaris lumbricoides into the hepatobiliary tract are presented. In the presence of nonspecific symptoms and biochemical test results, the diagnosis was made in seven patients by sonography. In the remaining patient, in whom the adult roundworm was not visible on sonography owing to the presence of aerobilia, the diagnosis was made at ERCP. Sonography revealed typical tubular, echogenic, filling defects along the long axis of a dilated common bile duct, sometimes exhibiting slow movements and containing a central sonolucent line. Repeated sonographic ex

Ascaris lumbricoides has rarely been implicated in the causation of middle ear disease. An infant who developed acute suppurative otitis media while on antibiotics for an upper respiratory tract infection and who extruded an ascaris worm from the ear is presented. The otitis media subsequently resolved. Our understanding of the pathophysiology of ascariasis in the eustachian tube and middle ear is discussed.

Infection with the roundworm Ascaris lumbricoides is common in children and may mimic an acute abdomen. The cases of two pediatric patients who presented to the emergency department with signs and symptoms of a surgical abdomen are presented to highlight the presentation of this infection. Both cases were diagnosed by physical examination and radiographs and were treated successfully without surgical intervention. These cases illustrate the need for heightened awareness by the emergency physician of ascariasis in the differential diagnosis of acute abdomen.
