Oxygen therapy and oximetry in the delivery room.

Pulse oximetry is increasingly being used in the delivery room. Expert recommendations state that oxygen therapy during newborn resuscitation should be guided by pulse oximetry. Obtaining accurate and stable oxygen saturation and heart rate information from a pulse oximeter in the delivery room can be challenging. Understanding the properties of this device is important in overcoming these challenges. This article describes several aspects of pulse oximetry use in the delivery room ranging from technical issues with the device itself to clinical applications of the technology.

Aetiology and types of neonatal seizures presenting at Ayub Teaching Hospital Abbottabad.

BACKGROUND: Neonatal seizures (NS) affect approximately 1% of neonates. Clonic, tonic, myoclonic and subtle seizures are the common types. Birth asphyxia, sepsis, metabolic derangements, intracranial bleed, kernicterus, tetanus and 5th day fits are the common aetiologies. This study was planned to evaluate the types and causes of neonatal seizures. METHODS: It was a descriptive case series conducted at Ayub Teaching Hospital, Abbottabad from 12th December 2006 to 25th September 2007 on neonates having seizures. Serum chemistry, blood counts, cerebrospinal fluid examination and cranial ultraso

Performance of Combined Support Vector Machine and Principal Component Analysis in recognizing infant cry with asphyxia.

Combined Support Vector Machine (SVM) and Principal Component Analysis (PCA) was used to recognize the infant cries with asphyxia. SVM classifier based on features selected by the PCA was trained to differentiate between pathological and healthy cries. The PCA was applied to reduce dimensionality of the vectors that serve as inputs to the SVM. The performance of the SVM utilizing linear and RBF kernel was examined. Experimental results showed that SVM with RBF kernel yields good performance. The classification accuracy in classifying infant cry with asphyxia using the SVM-PCA is 95.86%.

[Cardiac troponin as biochemical marker of perinatal asphyxia and hypoxic myocardial injury].

BACKGROUND/AIM: Myocardial cell lesion in newborns may be clinically occult. In recent years there has been shown growing interest in the use of cardiac troponin-I (cTnI) in relation to perinatal asphyxia and hypoxic myocardial lesion. The aim of this study was to determine a relationship between high cTnI levels and outcome in critically ill newborns with perinatal asphyxia. METHODS: In this study 78 patients were divided into three groups. The group I included 39 newborns (15 term and 24 preterm) with perinatal asphyxia, with no deaths, only full or partial (with some neurological sequels)

Spectral analysis of burst periods in EEG from healthy and post-asphyctic full-term neonates.

OBJECTIVE: To investigate whether the periodic EEG patterns seen in healthy and sick full term neonates (trace alternant and burst suppression, respectively) have different frequency characteristics. METHODS: Burst episodes were selected from the EEGs of 9 healthy and 9 post-asphyctic full-term neonates and subjected to power spectrum analysis. Powers in two bands were estimated; 0-4 and 4-30 Hz, designated low- and high-frequency activity, respectively (LFA, HFA). The spectral edge frequency (SEF) was also assessed. RESULTS: In bursts, the LFA power was lower in periods of burst suppression

[Birth asphyxia as a cause of cerebral palsy].

Birth asphyxia is O2i CO2 exchange disorder during the labour, with consequent hypoxia and ischemia. The term "asphyxia" has been used unprecisely quite often. The most frequently used criteria for birth asphyxia have been: fetal bradycardia, meconium stained amniotic fluid, fetal acido-base status with umbilical artery pH value below 7.10, low Apgar score and need for endotracheal intubation. The correct Apgar score quantification depends on the examiner. Fetal acido-base status measured in umbilical artery could be useful biochemical parameter of birth asphyxia. Only if the fetal oxygen supp

Atrial natriuretic factor in neonatal hypoxic-ischemic encephalopathy.

The influence of perinatal asphyxia in the secretion of atrial natriuretic factor (ANF) during the first 6 days of life, and its renal consequences are discussed. Comparison between 20 healthy term neonates and 19 with first--or second--degree hypoxic-ischemic encephalopathy (HIE) is made. Daily controls were performed on clinical and neurological examinations and administration of sodium and fluids. On the first and sixth days of life, 24 hours urine collection, natremia, natriuresis, fractionated excretion of sodium and creatinine clearance were determined. The ANF was performed at 1, 2, 3 a

Breathing pattern abnormalities in full term asphyxiated newborn infants.

Perinatal asphyxia is a cause of significant morbidity among full term infants, but breathing abnormalities after an asphyxic insult have not been studied. This report details breathing patterns of 16 full term asphyxiated infants, during the first week of life who were studied by transthoracic impedance pneumocardiograms. Pneumocardiograms were abnormal in 69% of infants in the asphyxiated group and 13% of infants in the control group. Significant differences were noted in the incidence of prolonged apnoea, the percentage of periodic breathing, and in apnoea density. These results indicate th

Intrapartum asphyxia in term and post term infants.

Many attempts have been made to identify infants at risk of suffering asphyxial brain damage. In a retrospective review of records at the Rotunda Hospital over a five year period all infants who died or suffered seizures, presumed secondary to asphyxia, were compared with the general hospital population. Out of 28,655 deliveries reviewed, there were 13 deaths in infants at or after term associated with perinatal asphyxia, and 32 surviving infants had asphyxial seizures. Seizures were regarded as asphyxial in origin if they occurred in the first forty eight hours of life and were associated wit

Perspectives on perinatal asphyxia: pathophysiology, presentation, and prognosis.

Perinatal asphyxia is a common and potentially serious problem in the management of labor and delivery. Severe perinatal asphyxia can cause significant short- and long-term sequelae. While the pathophysiology and sequelae of perinatal asphyxia in animals is understood fairly well, asphyxiation cannot be reliably prevented or effectively treated once it occurs in the newborn human. However, with a better understanding of what perinatal asphyxia is and its expected clinical symptoms and outcomes, perinatal nurses can better care for asphyxiated infants.

[Urinary N-acetyl-beta-D-glucosaminidase activity in healthy, polycythemic and hypoxic newborn infants].

Urinary N-acetyl-beta-D-glucosaminidase (NAG) activity was assayed in 20 polycythemic newborns and prematures, together with 50 prematures suffering from hypoxia on the 1st, 2nd, 4th, 14th, and 28th day after birth. The enzyme was also assayed in 101 healthy newborns which provided normal reference values. NAG activity was factored by the creatinine concentration to given an index. There were significant difference in the NAG indices either between full-term and preterm babies or between appropriate for gestational age (AGA) and small for gestational age (SGA) neonates of the normal group. [The role of neonatal electroencephalography in the prognosis of the hypoxic-ischemic syndrome].

One hundred and thirty four newborn children of 37 weeks' gestation age or greater, with evidence of hypoxic-ischemic encephalopathy following perinatal asphyxia and surviving through the neonatal period were prospectively studied. The study involved evaluation of clinical outcome and the presence of epilepsy associated with the EEG pattern in the neonatal period. The results prove that the presence of a burst suppression EEG pattern and a hypoactive/flat EEG are negative prognostic criteria. In presence of a focal or multifocal pattern it is difficult to establish a clear prognostic criteria.

The incidence and severity of post-asphyxial encephalopathy in full-term infants.

The incidence of clinically significant birth asphyxia was assessed over a 4-year period in a busy teaching hospital maternity hospital. The overall incidence was 6.0 per 1000 live-born deliveries of whom 2.1 per 1000 showed severely abnormal features including seizures or coma. There was a marked seasonal distribution (more common in winter than summer months) and intrauterine growth retardation occurred in 25% of asphyxiated infants. Only 1.6% of these infants were large for gestation age. There is some evidence that the incidence of this condition has not changed over a 10-year period.

Neonatal hypoxia and epileptic risk: a clinical prospective study.

A historical cohort study was undertaken to determine the risk of epilepsy in a population of 371 newborns with an acute neurological disorder related to fetal and/or neonatal hypoxia compared with a control population of 362 normal newborns. The results showed that the risk of epilepsy was 5.1 times higher in the group of subjects affected at birth by a hypoxia-related acute neurological syndrome than in the control group. (Although the incidence of epilepsy is higher in the first year of life, epileptic seizures connected to perinatal hypoxia may occur in early childhood or later on.) Also,

[Bilateral pulmonary hypoplasia in neonates: a clinical and pathological review apropos of 17 cases].

17 neonates presenting with bilateral hypoplasia of the lungs are reported. Histologic examination of the 13 neonates who died before the 24th hour of life showed 2 histologic types: a "fetal" type, similar to the pulmonary histology of 16-20 weeks fetuses, and a "miniature" type, with reduced dimensions, of harmonious aspect. The association of pulmonary hypoplasia with other fetal malformations suggests a different pathophysiologic mechanism for each type. Recovery may be possible in children with the "miniature" type.
