PURPOSE OF REVIEW: Astigmatism correction in cataract surgery is a common surgical challenge. Although there are numerous approaches to its treatment during cataract surgery, there remains a lack of consensus on what level of postoperative astigmatism to target. We examine the literature to determine the effect of astigmatism on visual function and provide a recommendation on how much to treat in cataract surgery.

Correction of corneal astigmatism is a key element of cataract surgery, since post-surgical residual astigmatism can compromise the patient's uncorrected visual acuity. A refractive lenticule was created using a femtosecond laser and then extracted through a small arcuate incision. We report a 63-year-old male who had undergone left eye optical penetrating keratoplasty for central leucomatous corneal opacity 10 years earlier. The eye had clear donor graft with residual astigmatism of -6.50 diopter cylinder (DC) at 30°. Validation of surveys is an important step in establishing the usefulness of questionnaires for gathering information in clinical studies. Often in large studies of refractive error, it is not possible to examine and refract all subjects and their relatives. The results of transverse or combined transverse and radial keratotomy in eyes with idiopathic corneal astigmatism (N = 11), astigmatism+myopia (N = 9) or early stages of keratoconus (N = 6) are presented.
