Maximizing performance and the prevention of injuries in competitive athletes.

Training athletes to achieve their personal best is an arduous task. The art of coaching athletes toward world-class performances is a blend of science and psychologic motivation. Competitive athletes train 20 to 40 hours per week and push the physiologic limits of the human body. In the past, applying training loads was a best-guess estimate made by the coach and/or athlete. Every athlete is different, and his or her day-to-day exercise and nonexercise stressors vary greatly. Psychophysiologic disturbances precipitated by overtraining can lead to exponential changes in an athlete's ability to

[Muscle injuries in athletes: pathogenesis, diagnosis and treatment].

All muscles can be injured during sports activities, certain specific muscles in the lower extremities are particularly vulnerable. The spectrum of these injuries is wide. Muscle strain injuries frequently involve polyarticular muscles which contain a high percentage of fast-twitch fibres (Type II). Disruption of muscle fibres occurs near the myotendinous junction. In most cases, the diagnosis can be based upon history and physical examination. Ultrasound and Magnetic Resonance Imaging can be useful to determine the severity of a specific injury. The treatment of these injuries is basically co

Periarticular injuries to the hallux metatarsophalangeal joint in athletes.

The magnitude of hallux MTP injuries can range from a mild sprain to a frank dislocation. The importance of coaches, trainers, and physicians recognizing the severity of a turf toe injury cannot be overstated. The late sequelae of hyperextension injuries can lead to retirement from professional athletics. With appropriate conservative treatment, most individuals can return to play, although many have some residual pain. Future study in this area should define the indications for acute repair versus late treatment following a period of conservative modalities. Hyper-plantarflexion injuries also

Magnetic resonance imaging of injuries in the child athlete.

Sports related injuries in children and adolescents are an increasingly frequent clinical problem. Soft tissue injury is relatively less common in children than in adults and radiographs suffice in most situations. However there are specific clinical circumstances where MR imaging is of value. We discuss and demonstrate a variety of acute and chronic sports related conditions where MR imaging has contributed to the diagnosis and management. Some conditions such as apophysitis, osteochondritis and chronic physeal injuries are unique to childhood while others including spondylysis and stress fra

Oral injury and mouthguard usage by athletes in Japan.

The use of mouthguards in contact sports effectively prevents oral injury and preserves oral structures. We investigated oral injuries and awareness concerning mouthguards among Japanese high school soccer and rugby players. Athletes were asked a series of questions concerning their history of oral injury while participating in sports, and their pattern of mouthguard use was determined. The data were evaluated statistically using chi-square analysis. The incidence of oral injuries was 32.3% for soccer athletes and 56.5% for rugby athletes, with 0.8% and 24.1% of the respective groups having mo

Closed tendon injuries of the hand and wrist in athletes.

Closed-tendon injuries of the hand and wrist are very common in the athletic population. Most of these injuries, if seen acutely, can be treated successfully with nonoperative management, although some do require operative treatment. It is those injuries, however, that go undiagnosed until the end of the season, which may result in permanent disability.

Valgus Extension Injuries of the Elbow in the Throwing Athlete.

Valgus extension injuries of the elbow are common among throwing athletes. There is a wide spectrum of these injuries, ranging from early soft-tissue inflammatory changes to attenuation and incompetence of the ulnar collateral ligament, as well as bone changes, including chondromalacia, osteochondritis, and osteophyte formation. Early treatment should be directed toward decreasing pain and inflammation, followed by global strengthening and conditioning of the elbow with special emphasis on plyometric strengthening of the flexor-pronator musculature. In patients who remain symptomatic after an

Anterior cruciate ligament injury. The competitive athlete.

The competitive elite athlete who sustains an anterior cruciate ligament (ACL) rupture has few options for treatment. If they wish to continue to compete at the preinjury level, then the only viable option is to undergo an ACL reconstruction. Otherwise, the athlete is at a substantial risk with a ACL deficient knee of sustaining subsequent degenerative changes in the knee at a young age. The athlete's presentation following injury includes reporting of a contact or noncontact acceleration or deceleration twisting injury to the knee, a 'pop' or the sensation of the joint 'coming apart', haemart

The role of arthroscopy in the treatment of wrist injuries in the athlete.

As a minimally invasive procedure, wrist arthroscopy provides the advantages of accurate, direct visual inspection of intra-articular structures for diagnostic purposes and dramatically reduced morbidity for surgical treatment of certain disorders. For an athlete who has a limited window of opportunity to compete, arthroscopy can offer an earlier, more accurate, more complete diagnosis than other imaging techniques for soft-tissue disorders of the wrist. Arthroscopic surgery can frequently offer an abbreviated recuperation period and earlier return to competition. Arthroscopy offers advantageo

Osteitis pubis in athletes. Infection, inflammation or injury?

Medical records of 59 patients (9 females and 50 males), who presented to sports medicine clinics at the Australian Institute of Sport and the University of British Columbia between 1985 and 1990 and who were diagnosed as suffering osteitis pubis, were reviewed and comparison of data obtained was made with the literature. Women average 35.5 years of age (30 to 59 years) and men 30.3 years (13 to 61 years). Sports most frequently involved were running, soccer, ice hockey and tennis. Clinical presentations of osteitis pubis fell into 4 main groups. 'Mechanical' (sport-related) was the largest gr

The diagnosis and nonoperative treatment of shoulder injuries in athletes.

The specific emphasis in this article has been directed toward the diagnosis of prevalent shoulder pathology in a young athletic population; however, as the interest in sports has blossomed in recent years now encompassing a larger age range, the physician must not neglect common pathologic conditions of the older athletes. Fastidious adherence to complete history, physical examination, and a high level of suspicion for uncommon disorders is paramount. Arthritides such as osteo, rheumatoid, septic, and lyme as well as the hematologic disorders of multiple myeloma, lymphomas, leukemia, hemophil

Roentgen examination of cervical spine injuries in the athlete.

The roentgen evaluation of the cervical spine must be performed immediately following the possibility of injury and in such a manner as not to compromise the neurologic status of the patient. Subtle roentgen findings indicating ligamentous injuries must be recognized so that they can be treated prior to developing cervical spine instability. Occult fractures, which may be difficult to diagnose on plain films and require multiple radiographic modalities, must be diagnosed so as to prevent prolonged intractable neck pain. Most importantly, recognizing the mechanism of injury and prevention of ce

Athlete shoulder injuries: CT arthrographic findings.

Forty-three professional and amateur athletes with persistent shoulder pain that interfered with their sports activities were evaluated by computed tomographic (CT) arthrography. In 19 patients, glenohumeral instability (14 anterior, two posterior, three multidirectional) was diagnosed with CT arthrography based on the simultaneous presence of labral and capsular lesions. The findings were crucial in establishing the diagnosis of instability in six patients in whom the condition was not suggested or could not be confirmed clinically. Another significant injury consisted of labral lesions not a

Bony injuries about the elbow in the throwing athlete.

Tremendous valgus stress during the acceleration phase of pitching is the primary cause of elbow injury in the pitcher. Lateral compression injuries are usually found in pitchers in their early teens, and osteochondritis dissecans is the primary underlying cause. Severe flexion contractures and even partial ankylosis of the elbow may occur. Prognosis after surgery is generally poor. Medial tension injuries with either soft-tissue damage or bony damage usually require surgery. The medial collateral ligament should be considered the primary stabilizer against the valgus-vector overload. The medi

Peripheral nerve injuries in athletes: a case series of over 200 injuries.

We retrospectively reviewed electrodiagnostic (EDX) studies performed on 346 athletes with sports injuries who were referred to our EDX laboratory from 1974 to 1997. These injuries included 216 nerve root, plexus, or peripheral nerve injuries sustained by 180 of the athletes. Eighty-six percent of the injuries were to the upper extremity. Athletes with nerve fiber injuries participated in 27 different sports, but over one third of injuries were sustained playing football. The most common symptomatic upper extremity injury was the "burner" (N=40). Forty-three athletes had median neuropathies, m

On-Field Management of Athletic Head and Neck Injuries: Spinal Motion Restriction, Equipment Removal, Patient Transfer, and Spine Boarding Techniques.

BACKGROUND: Although rare, athletic head and cervical spine injuries can be devastating. All health care professionals involved in the on-field care of athletes with these injuries should be aware of updated national guidelines as well as individual institutional practices regarding head and neck immobilization after on-field injuries. Appropriate prehospital management-including evaluation, equipment removal, spine board immobilization, and patient transfer-can mitigate further risk for these injured athletes. INDICATIONS: The goal of spinal-motion restriction is to reduce spinal instability
