Iatrogenic erosion of the septum primum after foramen ovale closure is an anecdotal event. We report the case of a 39-year-old woman admitted to our institution for multifocal cryptogenic cerebral ischemia and a patent foramen ovale (PFO) associated with an aneurysm of the septum primum. Lutembacher's syndrome is a combination of interatrial septal (IAS) defect or patent foramen ovale (PFO), associated with mitral stenosis. High left atrial (LA) pressure in mitral stenosis exaggerates left-to-right shunt in patients with interatrial communication. We present a case of heart failure with preserved ejection fraction. BACKGROUND: Transposition of the great arteries with intact ventricular septum causes severe hypoxia immediately after birth. Balloon atrial septostomy is often required for oxygenation improvement, and severe cases may require veno-venous extracorporeal membrane oxygenation (VV-ECMO). This case report highlights the critical role of balloon atrial septostomy in improving respiratory system compliance and facilitating early ECMO discontinuation in a neonate with transposition of the great arteries.

We encountered a very rare lesion, which included a dissection between a prosthetic patch for repair of an incomplete atrio-ventricular septal defect (AVSD) and a pseudo-intima which had developed. This is the first report of a dissection between an atrial septum patch and pseudo-intima after mitral valve replacement (MVR) in a patient with infective endocarditis following an incomplete AVSD repair. Dissection of the left atrial wall or atrial septum after MVR is extremely rare although some case reports have been published.

The first clinical experience of the successful application of electrostimulation of the atrial septum is described. Sickle-shaped electrodes were used for this purpose. Electrostimulation of the atrial septum was medically indicated in all cases. In 2 cases the electrode was fixed to the atrial septum after open-heart surgery and in 3 cases it was done because of an excessively high threshold of cardiac electrostimulation. Sickle-shaped electrodes are shown to be the most optimal for continuous electrostimulation of the atrial septum.
