Stepwise Increase in Concentrations of Atropine Treatment for Myopia Control Study.

PURPOSE: To explore a stepwise atropine treatment strategy for myopic control: beginning with 0.01% atropine for 6 months, followed by adjustment to higher concentrations based on treatment response. DESIGN: Prospective study. PARTICIPANTS: Total of 150 children aged 8 to 14 years with spherical equivalent refractive error (SER) range of -1.00 D to -6.00 D in both eyes were enrolled. METHODS: The study was divided into 2 phases. In the initial 6-month period, children received 0.01% atropine eye drops daily in both eyes. In the subsequent 12-month switchover period, children with less than …

Contribution of the serotonergic system to myopia and the therapeutic effects of atropine and pirenzepine.

Muscarinic-cholinergic antagonists, such as atropine, are widely used to treat myopia, yet their mechanism of action remains unclear. Recent chick studies suggest atropine may act via a non-muscarinic pathway, specifically antagonism of serotonergic signalling. This study investigates the conservation of this mechanism across different models of myopia (form-deprivation myopia (FDM) and lens-induced myopia (LIM)), and across different muscarinic agents (i.e., pirenzepine). We also examined the role (and ocular distribution) of serotonergic signalling in myopia onset. Chicks undergoing FDM or L…

Designing a contact lens with atropine base using a microemulsion technique.

PURPOSE: Myopia or near-sightedness is a global vision problem. Atropine eye drops and myopia-controlling contact lenses can help to slow down its progression, but neither is sufficient alone. The present research work was conducted to design a contact lens embedded with an atropine base within a microemulsion system. The goal was to improve the stability of atropine base and facilitate its release from the lens, preventing the rapid clearance observed with atropine eye drops. METHODS: Two microemulsions, one with a pH of 7.4 and the other with a pH of 6.5, were developed using the surfactant…

Use of Atropine, Ondansetron, and Ketorolac in Suspected Amniotic Fluid Embolism.

Amniotic fluid embolism (AFE) is a rare, life-threatening obstetric event associated with extremely high maternal and perinatal morbidity an … scitation of patients with presumed AFE with a protocol that includes intravenous administration of atropine (1 mg), ondansetron (8 mg), and ketorolac (30 mg). The currently available evidence for the use of atropine, ondansetron, and ketorolac to treat AFE is limited, consisting of only case reports. As a result, the potential harms, including worsening o …

Analysis of the reports of low-concentration atropine in controlling myopia in children.

PURPOSE: To evaluate the efficacy of 0.01% atropine to slow the progression of myopia in children. METHODS: We searched PubMed, Embase, ClinicalTrials.gov, CNKI, Cqvip and Wan fang databases from inception to January 2022, including RCTs (randomized controlled trials) and non-RCTs (non-randomized controlled trials). The search strategy was 'myopia' OR 'refractive error' and 'atropine.' Two researchers independently reviewed the articles, and stata12.0 was used for meta-analysis. The Jadad score was used to assess the quality of RCT, and the Newcastle-Ottawa scale was used for non-RCTs. RESUL…

[[A MULTICENTER SPANISH STUDY OF ATROPINE 0.01% IN CHILDHOOD MYOPIA PROGRESSION]].

Myopia, short-sightedness, is the most frequent cause of vision difficulty and its prevalence is expected to rise. Nowadays, the usage of atropine drops is effective for myopia progression control. Studies on the Asian population showed that atropine 0.01% is more efficient and safer than solutions that have a higher concentration. In this study, we report findings from a multicenter Spanish study that showed that atropine 0.01% decreases the progression of myopia in a pediatric population, with relatively few adverse effects. The results of this study may be relevant to doctors who treat chil…

Use of atropine penalization to treat amblyopia in UK orthoptic practice.

PURPOSE: To compare clinical practice patterns regarding atropine penalization use by UK orthoptists to the current evidence base and identify any existing barriers against use of AP as first-line treatment. METHODS: An online survey was designed to assess current practice patterns of UK orthoptists using atropine penalization. They were asked to identify issues limiting their use of atropine penalization and give opinions on its effectiveness compared to occlusion. Descriptive statistics and content analysis were applied to the results. RESULTS: Responses were obtained from 151 orthoptists …

Scleral changes induced by atropine in chicks as an experimental model of myopia.

PURPOSE: To determine the effects of intravitreal atropine on scleral growth in the form-deprived chick as an experimental model of myopia. METHODS: Five groups of five chicks were studied from day 0-12 post-hatching. One group remained untreated (C), and four were form-deprived by monocular light diffusers to induce myopia. Two groups (RL and A) wore diffusers for 9 days, and the other two groups (D and D + A) wore diffusers throughout the study. Group D received no further treatment (myopia positive control). Groups A and D + A received intravitreal injections of atropine for days 9-12. Mea…

[Emergency axillary brachial plexus block in a patient allergic to atropine].

Peripheral nerve blocks can provide a valid alternative for anesthetizing patients with allergy to drugs such as atropine that are often used for general anesthesia. We report the case of a 7-year-old boy (physical status ASA II, weight 28 Kg). Relevant medical history included generalized skin rash and breathing difficulties due to bronchospasm after administration of atropine eyedrops. The boy had never undergone skin testing for allergy. With a diagnosis of distal metaphyseal fracture of the right radius, he was proposed for urgent closed reduction and osteosynthesis of the lesion. After pr…

Scavenging of photogenerated oxidative species by antimuscarinic drugs: atropine and derivatives.

The quenching ability of photogenerated oxidative species by some antimuscarinic drugs generically named atropines (e.g. atropine [I] eucatropine [II], homatropine [III] and scopolamine [IV]) have been investigated employing stationary photolysis, polarographic detection of dissolved oxygen, stationary and time-resolved fluorescence spectroscopy, and laser flash photolysis. Using Rose Bengal as a dye sensitiser for singlet molecular oxygen, O(2)((1)Delta(g)), generation, compounds I-IV behave as moderate chemical plus physical quenchers of the oxidative species. Correlation between kinetic and…

[The development of occlusion amblyopia following atropine therapy for strabismic amblyopia].

For the treatment of strabismic amblyopia the authors have used atropine in the sound eye under usual optical correction as soon as central fixation of the amblyopic eye is secured by means of proceeding occlusion therapy. Although there may be little effect of occlusion in atropine therapy, six children under five years of age developed amblyopia in their sound eyes. In every case, the first sign of developing amblyopia was the decrease of fixation ability in the sound eye in a binocular fixation test prior to the reduction of visual acuity. On the other hand, fixation ability of the original…

Stimulation of pyloric contractions by intraduodenal triglyceride is persistent and sensitive to atropine.

Intraduodenal lipid infusion stimulates phasic and tonic pyloric contractions and suppresses antral contractions. This study determined: (i) whether this response is sustained over 90 min; and (ii) the role of muscarinic mediation of this response. Antropyloroduodenal motility was recorded in 17 healthy volunteers with a sleeve/sidehole manometric assembly. Subjects received either a 90 min intraduodenal infusion of saline or triglyceride (20% Intralipid) at a rate of 1 mL/min; 30 min after the start of this infusion, the eight subjects who received triglyceride were given intravenous atropine…
