Unilateral lateral mass compression fractures of the axis.

Lateral mass compression fractures of the axis involve compression of the pedicle and displacement of the overlying facet, with or without an accompanying fracture of the odontoid. These unilateral fractures are not rare. Correlation is made between clinical cases and cadaver material with analysis of microradiographs. Criteria for recognition of the resultant healed fracture deformity include: (1) Tilting of the odontoid; (2) Loss of the odontoid shoulder on the affected side; (3) Increased downward slope of the affected facet; (4) Lateral compression, possibly with buckling; (5) Asymmetrical

Unilateral dysplasia of the superior articular facet of the axis.

A case of unilateral dysplasia of the superior articular facet of the axis due to incomplete or delayed ossification is described in a child. This may result in asymmetry of the articular surfaces of the axis in adult life and be a cause of torticollis. To our knowledge, no similar case has been reported.

A Novel Instrumentation Approach in a Pediatric Patient with Atlanto-Occipital Dislocation and Cervical Fracture: Case Report.

BACKGROUND: Although instrumented stabilization of pediatric atlanto-occipital dislocation (AOD) has been described in the literature, there is little evidence regarding instrumentation techniques in pediatric patients presenting with both AOD and a cervical fracture. We present a case of a 2-year-old male involved in a motor vehicle collision with an unstable C2 fracture and AOD, treated with an occiput-C4 posterior arthrodesis using a rod, crosslink, and cable construct. CASE DESCRIPTION: This patient suffered a type III C2 fracture and AOD with 4 mm craniocaudal and 3 mm anterior displacem

Outcome of Bilateral C1 Laminar Hooks Combined With C2 Pedicle Screw Fixation for the Treatment of C1-C2 Instability: A Report of 18 Cases From a Single Chinese Center.

STUDY DESIGN: A retrospective technical report. OBJECTIVE: To assess the effect of bilateral C1 laminar hooks combined with C2 pedicle screw fixation for the treatment of C1-C2 instability. SUMMARY OF BACKGROUND DATA: Various posterior atlantoaxial fixations for C1-C2 instability have been developed. However, due to anatomic anomalies of the vertebral artery, the smallness of the pedicle, trajectories of broken screws, or a lack of surgical experience, a simple atlantoaxial fixation technique with good safety and effectiveness is urgently needed. MATERIALS AND METHODS: From January 2007 to

Management of odontoid fractures.

Fifty-one adults with odontoid fractures were treated between 1966 and 1979. Approximately 50% had additional trauma; over half of the patients were younger than 40 years old. Three patients presented with myelopathy: 1 was complete at C-2 and the others recovered. Cervical roentgenograms were initially considered normal in 4 patients. The fracture was through the base of the odontoid process (Type 2) in 49 patients and was into the body of C-2 (Type 3) in 2 patients. Thirty-four patients underwent early posterior cervical fusion, whereas the others were treated initially with external immobil

C2 nerve dysfunction associated with C1 lateral mass screw fixation.

The C1 lateral mass screw technique is widely used for atlantoaxial fixation. However, C2 nerve dysfunction may occur as a complication of this procedure, compromising the quality of life of affected patients. This is a review of the topic of C2 nerve dysfunction associated with C1 lateral mass screw fixation and related research developments. The C2 nerve root is located in the space bordered superiorly by the posterior arch of C1 , inferiorly by the C2 lamina, anteriorly by the lateral atlantoaxial joint capsule, and posteriorly by the anterior edge of the ligamentum flavum. Some surgeons su

Axis ring fractures due to simulated head impacts.

BACKGROUND: We investigated mechanisms of axis ring fractures due to simulated head impacts. METHODS: Our model consisted of a human upper cervical spine specimen (occiput through C3) mounted to a surrogate torso mass on a sled and carrying a surrogate head. We divided 13 specimens into 3 groups based upon head impact location: upper forehead in the midline, upper lateral side of the forehead, and upper lateral side of the head. Post-impact fluoroscopy and anatomical dissection documented the injuries. Average occurrence times of the peak loads and accelerations were statistically compared (P

Fibromyxoma of the axis.

Fibromyxoma of bone is a rare benign tumor of fibrous tissue origin. The typical location is the jaws. Sporadic extragnathic cases have been reported, but fibromyxoma of the spine has not been reported. The histological appearance of fibromyxoma is benign and includes abundant extracellular fibrous and myxoid stroma with varying amounts of calcification and ossification. Myxoid changes are usually extensive. Extragnathic fibromyxoma of bone should be distinguished from benign cartilage-forming bone tumors, such as chondromyxoid and myxoid chondrosarcoma and myxoma of bone. It has also been sug

Systemic mastocytosis, an unusual cause of osteoporosis.

A 45-year-old patient is described presenting with severe axial osteoporosis. The presence of maculopapular skin lesions and the combination of osteolytic and osteoblastic bone lesions led to the tentative diagnosis of systemic mastocytosis. This diagnosis was confirmed by measurement of urinary histamine metabolites and by bone biopsy showing increased numbers of mast cells and fibroblast-like cells. The importance of these diagnostic procedures in this rare cause of osteoporosis is discussed.

Odontoid fracture in a nine-month-old infant.

A type 2 odontoid fracture in a nine-month-old infant is presented. To the best of our knowledge, this case has the youngest reported patient with this type of injury having occurred by a mechanism other than direct trauma at birth. The purpose of this communication is to review odontoid fractures in the pediatric age group.

Upward subluxation of the axis in ankylosing spondylitis. A clinical pathologic report.

Upward subluxation of the axis associated with cord compression and death was noted in a patient with a long history of idiopathic ankylosing spondylitis. Upward subluxation of the axis has been recognized in up to 8 per cent of patients with rheumatoid arthritis but it is an exceedingly rare complication of ankylosing spondylitis. In this patient psoriasis and then psoriatic dactylitis developed 26 years after the onset of his ankylosing spondylitis. It is tempting to speculate that the unusual destruction of the joints around the atlas might be due to an added effect of psoriasis on idiopath

Direct and Oblique Approaches to the Craniovertebral Junction: Nuances of Microsurgical and Endoscope-Assisted Techniques Along with a Review of the Literature.

PURPOSE: The aim of this review is to provide an update of the technical nuances of microsurgical and endoscopic-assisted approaches to the craniovertebral junction (transnasal, transoral, and transcervical), and to report on the available clinical results in order to identify the best strategy. METHODS: A nonsystematic update of the reviews and reporting on the anatomical and clinical results of endoscopic-assisted and microsurgical approaches to the craniovertebral junction (CVJ) was performed. RESULTS: Pure endonasal and cervical endoscopic approaches still have some disadvantages, includ
