Recent advances in synthesis and medicinal chemistry of benzodiazepines.

Benzodiazepines (BZDs) represent a diverse class of bicyclic heterocyclic molecules. In the last few years, benzodiazepines have emerged as potential therapeutic agents. As a result, several mild, efficient and high yielding protocols have been developed that offer access to various functionalized benzodiazepines (BZDs). They are known to possess a wide array of biological activities such as anxiolytic, anticancer, anticonvulsant, antipsychotics, muscle relaxant, anti-tuberculosis, and antimicrobial activities. The fascinating spectrum of biological activities exhibited by BZDs in various fiel…

[Addictophobia in modern psychiatry (benzodiazepines and other psychopharmacological drugs)].

The paper analyzes clinical, epidemiological and pharmacological aspects of abuse of psychopharmacological medications (anxiolytics, antidepressants, anticonvulsants) and probability of drug dependence. Emphasis is placed on assessment of addictophobia phenomenon, an unreasonable fear of medication dependence, widespread not only among patients and their relatives but also in medical community. The problem of addictive potential of benzodiazepines, in particular, phenazepam, is specially studied. Based on clinical and pharmacological data and taking into account an elimination half-life, the c…

Inappropriate prescription of benzodiazepines in acutely hospitalized older patients.

Benzodiazepines (BDZs) are widely prescribed in older people. The aims of the study are to assess the prevalence of inappropriate prescription of BZDs and the associated factors in acutely hospitalized older patients. Patients aged 65 years or more hospitalized from 2010 to 2017 in more than 100 Italian internal medicine and geriatric wards in the frame of the REPOSI register were included if prescribed with BDZs at hospital admission or discharge. Appropriateness of prescription was assessed according to the 2015 Beers criteria and their modified French and German versions. Among 4681 patient…

A comparison of opioids and benzodiazepines dispensing in Australia.

BACKGROUND: Inappropriate utilization of prescription opioids and benzodiazepines is a public health problem. This study examined and compared user-types and trends in dispensing of these medicines, and identified associated factors related to the duration of dispensing in Australia. METHODS: A random 10% sample of unit-record data of opioids and benzodiazepines dispensed nationally during 2013-2016 was analyzed. Users were categorized into four types: single-quarter (i.e., three months), medium-episodic (dispensed 2-6 quarters), long-episodic (dispensed 7-11 quarters), chronic (dispensed 12-…

Deprescribing benzodiazepines among hospitalised older adults: quality improvement initiative.

Benzodiazepines are recognised as being potentially inappropriate medications for seniors due to their considerable side-effect profile, yet they are commonly prescribed and infrequently discontinued (deprescribed). The study's primary objective was the deprescription or the dose reduction of benzodiazepines among newly hospitalised seniors using a patient education intervention. A 3-month duration quality improvement study based on the plan-do-study-act model was conducted across two units (3C and 4D) in the Glenrose Rehabilitation Hospital to improve benzodiazepine deprescribing among newly …

Benzodiazepines in anxiety disorders: Reassessment of usefulness and safety.

Importance: Benzodiazepines (BZs) are still widely prescribed for the treatment of anxiety disorders despite many publications in the literature which favour antidepressants (ADs) instead. What is the evidence?Observations: Treatment guidelines favour ADs over BZs for treatment of anxiety disorders without any head-to-head comparison of both drug groups with placebo. BZs are claimed to cause less efficacy and more safety issues than ADs, yet ADs also cause disturbing adverse events and, similar to BZs, discontinuation symptoms. Until evidence-based data become available, a look at two 6-month …

Prenatal stress alters sensitivity to benzodiazepines in adult rats.

In rats, prenatal stress (PS) induces persistent changes in the brain that eventually can be translated in altered behavior leading to a greater consumption of psychostimulants in the offspring during adulthood. Though many studies have been carried on the effects of PS on stimulant drug responsiveness, little is known about susceptibility to benzodiazepines dependence in this animal model. We hereby examined the long-lasting impact of PS exposure during the last 10 days of pregnancy on the vulnerability to benzodiazepine addiction in adult rats. In addition, we also investigated the link betw…

The additive effects of alcohol and benzodiazepines on driving.

OBJECTIVES: To examine the relationship between the combination of alcohol and benzodiazepines and the risk of committing an unsafe driver action. METHODS: We used data from the Fatality Analysis Reporting System (1993-2006) on drivers aged 20 or older who were tested for both alcohol and drugs. Using a case-control design, we compared drivers who had at least one unsafe driver action (UDA; e.g., weaving) recorded in relation to the crash (cases) to drivers who did not (controls). RESULTS: Drivers who tested positive for intermediate- and long-acting benzodiazepines in combination with alcoh…

The lack of priming for same-name pictures suggests that benzodiazepines affect access, retrieval or selection of conceptual/lexical information.

Benzodiazepines as a social problem: the case of halcion.

This paper uses the case study of the crisis surrounding the benzodiazepine sleeping tablet Halcion (triazolam). particularly in the UK and US, to assess the explanatory value of the developmental model of drugs as a social problem first outlined by Gabe and Bury (9). It is argued that the case study supports the value of the model's three stage approach of emergence, legitimation and state response but that further comparative work needs to be undertaken with prescribed drugs other than benzodiazepines in order to substantiate fully the claim that the model represents a "natural history" of d…

Potential prophylactic use of benzodiazepines for hemodialysis-associated seizures.

Hemodialysis-associated seizure (HAS) is a common complication of hemodialysis. The efficiency of anticonvulsant drugs in treating or preventing seizures is poorly defined. In this study, children on long-term hemodialysis were examined for HAS and the effect of diazepam prophylaxis on HAS was investigated. Nine patients with a mean age of 14.1 + 2.8 years had HAS and 4 with a mean age of 13.0 +/- 4.4 years had never experienced HAS. The patients with HAS had tonic-clonic seizures. Four patients had focal slow-wave paroxysms, especially in the parieto-occipital regions; 2 had subcortical epile

Effect of NMDA antagonists on rapid tolerance to benzodiazepines.

We have reexamined the effect of NMDA antagonists [(+)MK-801 and ketamine] on rapid tolerance to chlordiazepoxide. (+)MK-801 and ketamine blocked the development of rapid tolerance to chlordiazepoxide, but this effect was dependent on the dose ratio of the NMDA antagonist to that of the benzodiazepine used to produce rapid tolerance. Furthermore, NMDA antagonists blocked both learned and unlearned tolerance to chlordiazepoxide. It appears that in addition to impairment of memory and learning, NMDA antagonists may also influence some other mechanism involved in the production of drug-tolerance.

[Benzodiazepines in geriatrics--a case report on prolonged action].

Due to the age-dependent changes of pharmacodynamic and pharmcokinetic, the frequency of undesirable side-effects of benzodiazepines is higher in geriatric patients. An increased sensitivity to benzodiazepines and their metabolites can induce extremely prolonged duration of action leading to unconsciousness and respiratory insufficiency. Because of an assumed postoperative "psychosyndrome", a 72-year-old patient was treated with high doses of diazepam combined with single doses of tramal, clonidin and ranitidin. This treatment was followed by the development of respiratory insufficiency requir…

Clinical distinctions between long-acting and short-acting benzodiazepines.

After their clinical introduction in the 1960s, the benzodiazepines rapidly became the most widely prescribed sedative/hypnotics because of their many advantages over barbiturates and other older agents. Along with this popularity came controversy, which has continued to this day. The most recent form this has taken has been the concern that the short-acting benzodiazepines may have a predisposition to induce certain forms of clinical complications. The author reviews the historical framework in which this controversy arose. In the late 1970s and early 1980s, it became increasingly clear that …

Treatment of social phobia with drugs other than benzodiazepines.

Social phobia is emerging as an important cause of psychiatric morbidity. Reasons for this are described, as are clinical issues of importance to social phobia, including the extensive associated distress and disability. The use of phenelzine, atenolol, buspirone, fluoxetine, and moclobemide are described. Diagnostic and transcultural aspects of social phobia are described.

Continuation treatment of panic disorder with high-potency benzodiazepines.

High-potency benzodiazepines such as clonazepam and alprazolam are effective and safe in the short-term treatment of panic disorder, but less is known about their effectiveness and safety over the long term. Further understanding of these issues is important since panic disorder is usually chronic and may call for long-term treatment. Available naturalistic data suggest that tolerance to the antipanic or antiphobic effects of clonazepam and alprazolam does not occur in panic disorder/agoraphobia; doses also tend to decrease over time. Withdrawal symptoms occur after short-term treatment with a…

Use of barbiturates and benzodiazepines in treatment of epilepsy.

Barbiturates and benzodiazepines may be very effective in controlling seizures, especially status epilepticus. However, their chronic use may be associated with serious sedative-hypnotic effects that interfere with cognitive function and behavior. When utilized, patients must be carefully monitored to be certain that these side effects are not occurring.

Pharmacokinetics of benzodiazepines: metabolic pathways and plasma level profiles.

Large differences exist among the various benzodiazepines with regard to their pharmacokinetic properties and metabolism in man. Some are eliminated from the body at a relatively slow rate, e.g. desmethyldiazepam, and others are metabolized rapidly, e.g. midazolam, triazolam. Several benzodiazepines have major active metabolites that are slowly eliminated, e.g. medazepam, halazepam , quazepam and, consequently, should be considered as potentially long-acting. Such differences may be very important clinically because pharmacokinetic data will help to optimize drug therapy with respect to the ch…

Correlations among measures of response to benzodiazepines in man.

A retrospective analysis of five studies using benzodiazepines and one using a benzodiazepine-like drug, zopiclone, and involving 61 subjects in toto was carried out. All used a placebo control and all acute studies incorporated pre-treatment values as well. Eight variables which have previously been shown to be sensitive to benzodiazepine action and which covered a range of physiological, psychological and subjective measures were chosen. Within-subject correlations between drug minus placebo values for these variables were computed to examine relationships between the measures while the subj…
