The Helicobacter pylori breath test: a surrogate marker for peptic ulcer disease in dyspeptic patients.

BACKGROUND: There is interest in noninvasive H pylori testing as a means of predicting diagnosis and determining management in dyspeptic patients. AIMS: To assess the value of the 14C urea breath test as a predictor of peptic ulcer disease in patients presenting with dyspepsia. PATIENTS AND METHODS: 327 consecutive patients referred for investigation of dyspepsia had a 14C urea breath test performed before endoscopy. Patients were not included if they had previously confirmed ulcer disease, previous gastric surgery, or were taking non-steroidal anti-inflammatory drugs. RESULTS: Of the 182 p

Noninvasive detection of Helicobacter pylori infection in clinical practice: the 13C urea breath test.

OBJECTIVES: To validate the 13C urea breath test for the detection of Helicobacter pylori infection both before and after treatment. METHODS: 13C urea breath tests with 125-mg and 250-mg doses were carried out on each of 60 infected and 60 noninfected subjects. Results were compared with histological examination of gastric biopsies to establish detection limits. The best cut-off point was used in a clinical trial of the efficacy of the breath test in duodenal ulcer patients before and after antimicrobial therapy. The incremental increase (percentage, delta over baseline in U of delta/mil) in …

Hydrogen breath test as a simple noninvasive method for evaluation of carbohydrate malabsorption during exercise.

The aim of this study was to examine hydrogen (H2) production with the hydrogen breath test (HBT) after ingesting primarily digestible carbohydrate (CHO) during 3 h of 75% maximal oxygen consumption exercise. This was done to indicate CHO overflow in the colon which may occur when gastric emptying, intestinal transit and CHO absorption are not matched and CHO accumulates in the colon where it is subject to bacterial degradation. Further, this study was designed to assess breath H2 production as a function of the type of CHO ingested and the type of exercise. A group of 32 male triathletes perf

The 13C-urea breath test--detection of Helicobacter pylori infection in patients with partial gastrectomy.

The 13C-urea breath test (13C-UBT) for diagnosis of Helicobacter pylori (Hp) infection was evaluated in 41 patients after partial gastrectomy and was used for determination of the Hp-prevalence after two different procedures of reconstruction of the gastrointestinal tract, i.e. Billroth's II operation and Roux-en-Y anastomosis. Breath samples were taken at various time points within 30 minutes after a motility inhibiting liquid test meal with citric acid followed by 75 mg of 13C-urea. The 13CO2/12CO2-ratio (delta-value) was measured using isotope ratio mass spectrometry and the recovery of tra

Potential usefulness of hydrogen breath test with D-xylose in clinical management of intestinal malabsorption.

Hydrogen breath tests (H2 BT) have been used extensively to investigate intestinal disaccharidase deficiencies. A potentially useful test for assessing intestinal absorptive function, the H2 BT with D-xylose (H2 BT-D-xylose), has received scant attention. We report here the results of our investigation of this test in 45 patients. Fifteen patients had proved malabsorption that was due to nontropical sprue in nine, and to lymphoma, Whipple's disease, or giardiasis in the remainder. Nine patients had small-bowel bacterial overgrowth secondary to either postsurgical sequelae or intestinal dysmoti…

The application of learning theory to driving confidence: the effect of age and the impact of random breath testing.

Despite the dangers, many drivers continue to take risks when driving. This paper outlines an explanation of this risk-taking behavior and the failure of numerous fear-arousing messages to change the manner in which many people drive. Being an occupant in a car and learning to drive may be seen as analogous to systemic desensitization and flooding procedures, in which fear is extinguished and/or a response inconsistent with fear is learned. Once this procedure is complete the fear response is unlikely to be reinstated by messages pointing out possible dangers on the road, because the situation

[Hydrogen (H2) breath test following lactose loading in children with recurrent abdominal pain].

To define the practical diagnostic value of H2 breath testing after lactose load in children with chronic abdominal pain, we measured end-expiratory H2 every 30 minutes for 2 hours after peroral lactose (2 g/kg body weight, Lactoscreen). H2 testing was done in 62 children (28 female, 32 male), aged between 2 and 16 years. Abdominal pain had been observed in these children for more than 2 months. Causes of abdominal pain other than lactose intolerance had been excluded. In 2 out of 62 children, H2 testing was impossible for lack of cooperation. 17 out of the remaining 60 children showed a posit

Assessment of the reproducibility of the lactulose H2 breath test as a measure of mouth to caecum transit time.

The lactulose H2 breath test is in use as a simple non-invasive measurement of mouth to caecum transit time, but its reproducibility has never been assessed. We have examined the reproducibility of mouth to caecum transit time in 21 normal subjects using lactulose 10, 15, and 20 g; seven subjects being studied with 10 g and 12 each with 15 and 20 g doses. Transit time decreased with increasing doses of lactulose although the differences were not significant between or within (n = 5) individuals. Variation in transit times between individuals was considerable with all doses of lactulose (mean c

Dose dependence of the 14C-aminopyrine breath test. Intrasubject comparison of tracer and pharmacological doses.

Although the aminopyrine breath test has received much attention, the question has not yet been settled whether pharmacological or tracer doses of the drug should be used. Nine volunteers were given 14C-aminopyrine 9 mg/kg or a tracer amount, in a randomized sequence and according to a crossover design. The specific activity of 14CO2 in breath was significantly different only during the first hour. Up to the 8th hour the disappearance of 14CO2 from breath was smaller after the pharmacological (28.5 +/- SD 5.4%/h) than after the tracer dose (36.2 +/- 10.6%/h; p less than 0.05). The over-all dis

Extended hydrogen breath test analysis for optimized diagnosis of SIBO-positive IBS patients.

BACKGROUND: Small intestinal bacterial overgrowth (SIBO) is suggested in irritable bowel syndrome (IBS). Optimal hydrogen levels and time frame for diagnosing SIBO are still under discussion. Our primary aim was to consolidate a discriminating threshold for a positive lactulose hydrogen breath test (LHBT) in IBS. As a secondary aim, we optimized the diagnostic time frame for the small bowel. METHODS: LHBT was performed on 503 subjects who met the inclusion criteria. After excluding non-hydrogen producers, the remaining 462 subjects were 92 healthy individuals and 370 IBS patients. Peak hydroge

Same pattern, different result: Algorithmic inconsistency in breath alcohol testing-case reports.

Breath alcohol analyzers employ algorithmic safeguards to identify samples that may be falsely elevated due to mouth alcohol contamination. One such safeguard in the DataMaster DMT (also known as the Intox DMT) is commonly referred to as the 95% rule. Under this criterion, if the final quarter-second average breath alcohol concentration (BrAC) measured during sample delivery is less than 95% of a previous high value recorded during the same sample test, the instrument is expected to generate an Invalid status message. Four cases are presented in which the stated criterion was met, yet the inst

Detecting Bacteria in Their Mammalian Hosts Using Metabolism-Targeted [13C]CO2 Breath Testing.

Infectious diseases are a major cause of morbidity and mortality worldwide. With the increasing frequency of antibiotic resistance, efficient and noninvasive diagnostic methods are more important than ever. In this report, we interrogate the use of several intravenously administered, bacteria-specific, 13C-enriched metabolites whose conversion to [13C]-CO2 can be detected via a portable and inexpensive method, namely nondispersive infrared (NDIR) spectroscopy. The enriched metabolites [U-13C]-maltose, [U-13C]-maltotriose, d-[U-13C]-mannitol, and l-[U-13C]-arabinose were metabolized to [13C]-CO

Spot test with smartphone digital image analysis for determination of methadone in exhaled breath condensate.

In this work, we explored the potential of the spot test combined with image analysis using smartphones as a rapid, simple, low-cost, and environmentally friendly method for identifying methadone concentration. Herein, a carbon-gold nanocomposite has been used to generate color variation at different concentrations of methadone. The data obtained from the digital image colorimetric method was compared with those from the UV-Vis spectroscopy as a standard technique. This method was also utilized for extensive optimization and validation procedures. Through image analysis, it can be obtained wit

Single count breath test for the evaluation of respiratory function in Myasthenia Gravis: A systematic review.

BACKGROUND: Myasthenia gravis (MG) can have a variety of respiratory presentations, ranging from mild symptoms through to respiratory failure. The evaluation of respiratory function in MG can be limited by accessibility to testing facilities, availability of medical equipment, and facial weakness. The single count breath test (SCBT) may be a useful adjunct in the evaluation of respiratory function in MG. METHOD: A systematic review of the databases PubMed, EMBASE, and the Cochrane Library was conducted from inception to October 2022 in accordance with PRISMA guidelines and was registered on P

Lack of almagate interference in breath test results for Helicobacter pylori diagnosis (Almatest study).

OBJECTIVE: The 13C-urea breath test (UBT) is the most widely used non-invasive diagnostic test for Helicobacter pylori. Debate continues to surround the possible interference of antacid intake on its result. This study aims to confirm the non-interference of almagate in the determination of H. pylori by UBT. PATIENTS AND METHODS: Observational, multicentre study in adult patients treated with almagate in whom a UBT (TAUKIT®) was indicated. When the UBT result was negative, use of almagate was stopped for 30 days and the UBT was repeated. When the result was positive, no further determinations

A telephonic single breath count test for screening of exacerbations of myasthenia gravis: A pilot study.

BACKGROUND: Patients with myasthenia gravis (MG) may experience worsening symptoms outside of a clinical setting. A method of diagnosing and triaging such individuals would be valuable. This study gauged the viability of a nurse-administered single breath count test (SBCT) over the telephone for assessing MG exacerbations. METHODS: This was a retrospective, single-center review of a pilot study of 45 telephone calls from patients with MG who had worsening baseline symptoms. SBCTs were administered over the telephone to patients by trained nurses. Patients with a breath count of 25 or less wer

Does a glucose-based hydrogen and methane breath test detect bacterial overgrowth in the jejunum?

BACKGROUND: Direct diagnosis of small intestinal bacterial overgrowth (SIBO) requires the collection and culture of fluid from the jejunal lumen, with a finding of over 105 viable bacteria per mL. More often, SIBO is diagnosed indirectly, using a non-invasive test of the exhaled hydrogen and methane generated by microbial fermentation when ingested glucose reaches the jejunum. Our objective was to determine how well this breath test detects chronic overgrowth of jejunal bacteria that is unrelated to gastrointestinal surgery. METHODS: Eighteen patients reporting symptoms consistent with SIBO r
