Planned caesarean section for term breech delivery.

BACKGROUND: Routine use of caesarean section for breech presentation is widespread. However, poor outcomes after breech birth might be the result of underlying conditions causing breech presentation rather than damage during delivery. OBJECTIVES: To assess the effects of planned caesarean section for singleton breech presentation at term on measures of pregnancy outcome. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group trials register (November 2002) and the Cochrane Central Register of Controlled Trials (The Cochrane Library, Issue 1, 2003). SELECTION CRITERIA: Rand…

Differences between breech and cephalic neonates born after planned cesarean section: a systematic review.

BACKGROUND: A growing number of studies have found that breech presentation is related to preexisting diseases and may affect future fetal development. This study aimed to compare the differences between breech presentation and cephalic presentation and explore the underlying mechanisms of adverse outcomes caused by fetal station itself. METHODS: ‘PubMed’, ‘Embase’ and ‘Cochrane Library’ were searched from inception to October 21, 2022. Comparative studies containing ‘prelabor CS’ or ‘elective CS’ of cephalic and breech presentation were included. A total of 585 studies were included, and 5 st…

Undiagnosed term breech: management and outcome.

OBJECTIVE: To assess the antenatal and intra-partum risk factors associated with undiagnosed breech at term and to compare their outcome with those diagnosed before the onset of labor, as well as identifying the determinants for cesarean section in breech presentation. METHODS: Case control study of 183 undiagnosed breech deliveries at term with those diagnosed before labor. RESULTS: There was no significant difference between diagnosed term breech and undiagnosed breech deliveries with regards to maternal, neonatal outcome and incidence of cesarean section. Footling breech presentation, hyp…

Assisted Reproductive Technology and Breech Delivery: A Nationwide Cohort Study in Singleton Pregnancies.

Since essential factors have changed in recent years in assisted reproduction technologies (ARTs), this study reassessed the association between ART and breech presentation. We primarily aimed to estimate the correlation between ART and breech at delivery. Secondary purposes were to evaluate the correlation between other subfertility treatments (OSTs) and breech and to assess possible confounding factors and temporal trends. This study investigated the 31,692,729 live birth certificates from US states and territories in the 2009-2020 period. The inclusion criteria were singleton births reporti…

Association of a low chest-to-head circumference ratio with breech or transverse lie: the Japan Environment and Children's Study.

OBJECTIVE: We examined whether the chest-to-head circumference ratio at birth was associated with breech presentation and transverse lie. We also described the obstetric management of such pregnancies in the Japan Environment and Children's Study (JECS). METHODS: We performed a cross-sectional evaluation of data collected between January 2011 and March 2014 in a nationwide prospective birth cohort study, the JECS. We analyzed 83,822 non-anomalous singletons born at 34-41 weeks' gestation to mothers with no history of previous cesareans or uterine surgery. We defined low, normal (reference gro…

Conservative management in breech deliveries leads to similar results compared with cephalic deliveries.

OBJECTIVE: To compare the difference in neonatal mortality and morbidity between breech and cephalic presentations at term. METHODS: This was a retrospective matched cohort study in two centers between July 1998 and April 2000, including all breech deliveries between 37(+0) and 41(+6) weeks, except cases with multiple gestations and antepartum intrauterine deaths. All breech presentations were matched with two cephalic presentations. Onset of labor and route of delivery were recorded, and neonatal data were categorized into variables belonging to serious morbidity or moderate morbidity. RESU…

Effect of external cephalic version in a resource-limited setting on the Thailand-Myanmar border: a retrospective cohort with propensity score analysis.

BACKGROUND: External cephalic version (ECV) is recommended to reduce the risk of breech presentation at birth. This study analysed the effect of external cephalic version (ECV) or not, on breech presentation at birth in a resource-limited setting. METHODS: Women with ultrasound confirmed breech presentation from 28 weeks gestation at antenatal clinics of the Shoklo Malaria Research Unit (SMRU) and a known pregnancy outcome, from 2008 to 2018 along the Thailand-Myanmar border were included. Propensity score analysis using inverse probability weighting compared breech at birth between women who …

Breech deformation complex in neonates.

OBJECTIVE: To demonstrate the association of minor anomalies and breech-presenting newborns with breech deformation complex. STUDY DESIGN: A total of 3,345 newborns with singleton, term delivery were examined based on a list of 67 items of major and minor anomalies. All infants were divided into two groups: group 1, vertex presentation (3,107 infants); group 2, breech presentation (224 infants). The prevalence of anomalies between both groups was compared. RESULTS: There were 1,495 (44.9%) infants with one or more minor anomalies, which included 1,313 infants (42.3%) in group 1 and 182 (81.3…

Intrapartum management of multiple gestation.

The intrapartum management of multiple gestation continues to challenge the obstetric profession. In general, attempted vaginal delivery is appropriate for vertex-vertex twins. The options of external version, breech delivery, and cesarean delivery are analyzed for vertex-nonvertex twins. Special considerations in the intrapartum management of multiple gestation include monoamniotic twins, conjoined twins, and triplet pregnancies.

Uterine anomalies. Frequency of diagnosis and associated obstetric complications.

Uterine anomalies are associated with an increase in cases of breech presentation, premature labor, abnormal presentation with dystocia, and the necessity for cesarean section. The yearly frequency of diagnosis of an anomaly has varied from 0.06 to 0.48 at this medical center depending on the physician's clinical suspicion. Thus, if the true incidence and the decrease of obstetric complications are to be accomplished, an increased interest in diagnosing uterine anomalies is essential.

Vaginal breech delivery at term and neonatal morbidity and mortality - a population-based cohort study in Sweden.

INTRODUCTION: The routine to deliver almost all term breech cases by elective cesarean section (CS) has continued to be debated due to the risk of maternal and neonatal complications. The aims of the study were (1) to investigate if mode of delivery impacts on the risk of morbidity and mortality among term infants in breech presentation and (2) to compare the rates of severe neonatal complications and mortality in relation to presentation and mode of delivery. METHODS: This population-based cohort study used data from the Swedish Medical Birth Register. All women (and their newborn infants) w…

Recent trends in breech delivery in the Netherlands.

The effect of publication of the results of the Term Breech Trial in October 2000 on the mode of delivery of babies in breech presentation in the Netherlands was subject of this study. The results showed an immediate amplifying impact on top of the already existing trend of rising caesarean section rates for term singleton breech presentations. The willingness of obstetricians in the Netherlands to adopt the conclusions of the Term Breech Trial so quickly originate from scientific, legal, psychological and emotional sources. The same amplifying impact applied for all non-term breeches and bree…

[Neonate--newborn condition and prematurity with breech delivery].

INTRODUCTION: Before birth, most of foetuses located in mother's uterus have head presentation down towards the breech exit. Preterm delivery presents the delivery before the end of the 37th gestational week. Etiology of preterm breech deliveries is the same as of the ones at term, such as uterine malformations and myomas, polyhydramnion, foetal abnormalities, high parity, short umbical cord, placenta previa. The complications include passing of the body through insufficiently dilated cervical channel and "capturing " of the head, umbilical cord prolapse, intrapartal hipoxy, discoordinated del

Incidence and risk factors of caesarean section in preterm breech births: A population-based cohort study.

OBJECTIVES: To describe the incidence of breech presentation at 22-34 weeks' gestation, estimate the incidence of cesarean section delivery by cause of prematurity, and assess the factors associated with caesarean delivery in preterm breech births with preterm labor or preterm premature rupture of membranes. STUDY DESIGN: EPIPAGE 2 is a French national prospective population-based cohort study of preterm births that occurred in 546 maternity units in 2011. We estimated the overall incidence of breech presentation and the incidence of cesarean delivery by cause of prematurity. Among the 579 si…

Cesarean delivery of the breech very-low-birth-weight infant: does it make a difference?

We evaluated the impact of cesarean section (CS) delivery on the outcome of breech very-low-birth-weight (VLBW) (500-1,499 g) infants. This retrospective study used data from a large perinatal data base with 109,428 consecutive deliveries including 1,564 VLBW infants born between 1982 and 1987. The study infants included 982 (62.8%) vertex and 582 (37.2%) breech presentations. The CS rate was higher for the breech group than for the vertex group (72.5% breech, 34.3% vertex, OR 5.1, 95% CI 4.0, 6.4). Breech infants delivered by CS were twice as likely to survive until discharge than those deliv…
