Is severe renal osteodystrophy a contraindication for recombinant human growth hormone treatment?

Recent extension of the use of recombinant growth hormone (rhGH) to non-growth hormone-deficient patients necessitates close attention to possible complications in these patients, including effects on bone. Recent studies on the use of rhGH in children with chronic renal failure (CRF) provide some early data. No significant differences in radiographic osteodystrophy scores, serum calcium, phosphorus, or parathyroid hormone (PTH) levels were found between treated and untreated groups. Alkaline phosphatase increased transiently. The effect of renal osteodystrophy on growth response has not yet b

Scaphocapitate Fracture Syndrome in an Adolescent: A Case Report, Diagnosis, and Surgical Treatment.

Scaphocapitate fracture syndrome, or Fenton syndrome, is a rare condition characterized by fractures of the scaphoid and capitate bones, often with a 180-degree rotation of the proximal capitate, and is frequently underdiagnosed due to its rarity and inconclusive initial radiographic findings. We report the case of a 15-year-old boy who sustained a high-energy wrist injury after a three-meter fall. Initial X-ray raised suspicion of a scaphocapitate fracture syndrome; therefore, the patient was referred for a CT scan. He returned with the results two months later, which confirmed scaphocapitate…

Biomechanical comparison of 1-screw and 2-divergent pin internal fixations for treatment of slipped capital femoral epiphysis, using specimens obtained from immature dogs.

Biomechanical strength and stiffness of 2 fixation treatments used to repair acute slipped capital femoral epiphysis were evaluated in bone specimens form immature dogs. A servohydraulic testing machine was used to create slipped capital femoral epiphysis in 7 pairs of femurs by shearing the capital femoral epiphysis along the physis in a craniocaudal direction. The slip was reduced and repaired with one 3.5-mm-diameter screw placed in lag fashion or 2 double-pointed, 1.6-mm (0.062 inch)-diameter smooth pins and retested. Strength and stiffness of each intact femur (which served as the control

The value of scintigraphy in hips with slipped capital femoral epiphysis and the value of radiography and MRI after 10 years.

Preoperative bone scintigraphy of the femoral head in 33 hips with slipped capital femoral epiphysis, showed no relation to duration of symptoms or degree of slip. The preoperative uptake was always normal or increased. Two hips had postoperative femoral head uptake below normal, both had complications affecting the vascular supply, resulting in necrosis of the femoral head and severe arthrosis. At follow-up after 10 (5-15) years of 28 hips, no relation could be demonstrated between Adolescent Hip Questionnaire which included clinical data, and radiography or magnetic resonance imaging. We onl

Multiple fractures in early infancy: report of two unusual cases.

Two cases of multiple fractures, one in a newborn with an unclassified bone dysplasia, another in an infant with recurrent fits of unknown etiology are reported. The second case is the youngest in the literature with bilateral slipped capital femoral epiphyses and bilateral femoral neck fractures, secondary to stress injury.

Delayed diagnosis and surgical management of an isolated capitate fracture: A case report with two-year follow-up.

INTRODUCTION: Isolated capitate fractures are among the rarest carpal injuries and often remain undetected due to subtle radiographic findings and the bone's central location within the carpus. When diagnosis is delayed, complications such as nonunion and avascular necrosis (AVN) may develop, requiring advanced surgical management. CASE PRESENTATION: A 20-year-old male manual laborer presented with persistent wrist pain and restricted motion six months after a motorcycle accident. Initial radiographs failed to detect a fracture. Subsequent imaging, including computed tomography (CT) and magne

A transepiphyseal fracture of the proximal femur combined with a fracture of the mid-shaft of ipsilateral femur in a child: a case report and literature review.

The paper describes a rare fracture of proximal femur, classified by Delbet (Am J Surg 6:793-797, 1929) as type I (transepiphyseal type) combined with a fracture of the midshaft of ipsilateral femur in a 2-year-old child. Immediate operation with open reduction and internal fixation was successful. During the postoperative course, avascular necrosis (AVN) of capital femoral epiphysis was seen by bone scan. Applying an abduction orthosis used for the treatment of Legg-Calvè-Perthes disease, collapse of capital epiphysis was prevented. Although, a minimal area of AVN and coxa vara remained, no c

Evaluating osseointegration, bone remodeling and osteolysis after total wrist arthroplasty using photon-counting detector CT.

OBJECTIVE: To evaluate osseointegration, bone quality, and periprosthetic osteolysis following total wrist arthroplasty using photon-counting detector CT (PCD-CT). MATERIALS AND METHODS: Nineteen patients with total wrist arthroplasty were examined with PCD-CT the day after surgery, at 6 and 12 months postoperatively. CT images were reviewed and osseointegration and periprosthetic osteolysis were evaluated. Cortical thickness and cortical density were measured. The bones around the implant were divided into zones, and the evaluations were made in each zone. RESULTS: Osseointegration, visuali

Abnormal type III fusion between lunate and triquetrum: A case report.

INTRODUCTION: The lunotriquetral coalition is the fusion of the lunate and triquetral bones of the wrist and is the most frequent carpal coalition type. It is frequently asymptomatic and discovered as an incidental due to chronic wrist pain, trauma, or fracture. This case aims to present an unusual unilateral lunotriquetral coalition and clinical significance for diagnosis and treatment. PRESENTATION OF CASE: In this case, we presented a 37-year-old male who exhibited with right-sided trauma and wrist pain. He was diagnosed to had a type III complete osseous lunotriquetral coalition on the ri

A Radiographic Classification of Capitate Morphology and Its Association With Kienböck Disease.

PURPOSE: Kienböck disease (KD), osteonecrosis of the lunate, has an unclear etiology. While there is a strong and clear association between KD and negative ulnar variance (UV), it is likely not the only mechanical factor. The capitate, which has a large joint surface with the lunate, may play a role in KD pathogenesis. This study investigated the association between capitate morphology and KD. We classified the capitate into three types based on the angle and length ratio of the capitotrapezoid (CT) and scaphocapitate joint lines: type I (short, vertical CT facet), type II (pentagonal with a l

Slipped capital femoral epiphyses: A major on-target adverse event associated with FGFR tyrosine kinase inhibitors in pediatric patients.

Fibroblast growth factor receptor (FGFR) tyrosine kinase inhibitors (TKIs) are increasingly being used off label in pediatrics. Long-term safety data are limited, and serious toxicities unique to pediatrics may emerge. In a retrospective analysis of patients less than 18 years of age with recurrent/refractory FGFR altered gliomas treated with FGFR TKIs at MSKCC (n = 7), we observed slipped capital femoral epiphyses in three of seven patients along with increased linear growth velocity. Clinicians should closely monitor bone health and have a low index of suspicion for serious orthopedic compli

Significantly lower femoral neck growth in screw fixation of the asymptomatic contralateral hip in unilateral slipped capital femoral epiphysis.

There is an ongoing debate on which fixation technique should be preferred for the prophylactic fixation of the asymptomatic contralateral hip in slipped capital femoral epiphysis (SCFE). In the case of Kirschner-wire (K-wire) fixation, there is a possibility of secondary loss of fixation because of longitudinal growth of the physis, whereas in screw fixation, physeal growth of the femoral neck might be impaired. The aim of this matched-pair study was to compare the longitudinal growth of the femoral neck in screw fixation versus K-wire fixation of the asymptomatic contralateral hip in SCFE. A

High-grade SCFE: the role of surgical hip dislocation and reduction.

The patient with an unstable slipped capital femoral epiphysis poses a challenging problem to the treating physician to improve the position of the displaced epiphysis to avoid femoroacetabular impingement without developing avascular necrosis (AVN)-a potentially devastating complication. Although the standard operative procedure of in situ pinning following an incidental reduction while positioning the patient on the table, has been the mainstay of treatment in North America, other viable options are available including a surgical dislocation approach to the hip followed by a modified Dunn os

Staging puberty in slipped capital femoral epiphysis: importance of the triradiate cartilage.

In 83 children with slipped capital femoral epiphysis, puberty was staged at the time of diagnosis using bone age, closure of triradiate cartilage, Risser index, and the pubertal diagram of Dimeglio. In boys 95% and in girls 83% of slips occurred during the accelerating phase of puberty (stage 1 and 2), in which mainly the limbs grow. In boys (54%) and in girls (66%), most slips occurred in stage 1. The triradiate cartilage was still open at the time of diagnosis in 65% of boys and 64% of girls. Staging puberty is useful to differentiate the risk for slipped capital femoral epiphysis and to ev

Maturation factors in slipped capital femoral epiphysis.

Several maturation factors relative to growth and epiphyseal development were reviewed retrospectively in 191 patients with slipped capital femoral epiphysis, including bone age, height and weight, thyroid functions, sex hormone levels, and growth hormone levels. Seventy-one percent of 138 patients had weights above the 80th percentile. Active thyroid (T3) was significantly low in 25% of 80 patients studied. Testosterone levels were markedly depressed in 76% of 64 patients tested. In this same group, 87% had low growth hormone levels. The consistently low testosterone and growth hormone levels

Bilateral Slipped Capital Femoral Epiphysis in a Young Girl Treated With Chemotherapy: A Case Report.

CASE: A 1-year-old girl was treated with chemotherapy and hematopoietic stem cell transplantation because of CD40 ligand deficiency. Four years later, she presented with pain in her right leg, diagnosed as atypical acute slipped capital femoral epiphysis, without a clear cause, besides chemotherapy possibly. She was treated with fixation of the epiphysis with a cannulated screw. Two years later, the same diagnosis was made for the left hip and the same surgery was applied. After the 2-year follow-up, clinical outcomes were good. CONCLUSION: Chemotherapy may be a risk factor for atypical slipp

The inclination of the weight bearing surface in the hip joint. The clinical significance of abnormal force.

The mechanical basis responsible for the development of secondary osteoarthritis in adults and slipped capital femoral epiphysis in adolescents is discussed, with the role of the oblique weight-bearing surface in generating abnormal shearing forces in these two conditions emphasized. The same biomechanical principles have been found to apply to the acetabular component in total hip implants. A vertical cup position will lead to abnormal stress concentration in the outer edge of the bone implant interface with an increase in shearing forces. The form of the subchondral bone sclerosis on the rad

[Perilunar dislocation of the corpus in a patient with aplasia of the scaphoid].

Congenital aplasia of the scaphoid is a rare abnormality arising when the distal row of carpal bones is dislocated radially and proximally, the capitate replaces the scaphoid, and the lunate is triangular instead of quadrilateral in shape. The scaphoid bone which makes a functional bridge between the proximal and the distal row is a stabilising element under normal anatomical conditions. If it is absent, the carpus structure is disturbed and its radial part is prone to perilunar dislocation, because stability is provided only by the ligaments. However, a perilunar dislocation associated with a
