Sarcoid, amyloid, and acute myocardial failure.

We report the case of a 58-year-old woman, with an 8-year history of pulmonary sarcoidosis, who died in acute myocardial failure after cardiac catheterization. Systemic amyloid, likely of the AL type, was found to involve her myocardial interstitium and intramyocardial coronary arteries, as well as her spleen, thyroid, kidneys, and bone marrow. Her acute myocardial failure was thought to be the result of focal myocardial infarcts secondary to amyloid vasculopathy and of superimposed chronic restrictive myocardial function caused by amyloid. Five previously reported cases of concurrent sarcoido

Alcohol consumption and heart failure in hypertensives.

Sixty hypertensives (30 with and 30 without heart failure, matched for age and sex) were studied and their alcohol consumption and its possible role in heart failure were assessed. The majority in each group belonged to the low socioeconomic class. On presentation, the mean systolic blood pressures were 176.7 +/- 29.7 and 198.8 +/- 29.8 mmHg and diastolic blood pressures 118 +/- 15 and 118.5 +/- 13.6 mmHg, respectively. In those known to be hypertensive before presentation to our unit, hypertension was first detected 4.88 +/- 3.8 and 4.40 +/- 3.3 years earlier in the heart failure and non-hear

Critical mitral stenosis causing ischemic hepatic failure. Successful treatment by percutaneous balloon mitral valvotomy.

We report a 52-year-old patient with severe mitral stenosis who developed new onset atrial fibrillation, low output congestive heart failure and fulminant ischemic hepatic failure with subsequent severe coagulopathy. Percutaneous mitral valvotomy resulted in dramatic clinical improvement with complete resolution of liver function. This case illustrates the potential life-saving role for percutaneous balloon mitral valvotomy in treating critically ill patients who are unable to undergo thoracotomy due to coexisting medical illness.

Planned reduction in Fontan fenestration size using the Atrial Flow Regulator.

Fontan fenestration allows right-to-left shunting increasing cardiac output and oxygen delivery. Increased shunting occurs as cardiac function and ventricular end-diastolic pressures improve, potentially decreasing oxygen saturation. Complete closure may result in impaired Fontan haemodynamics and low cardiac output; however, there are no dedicated devices to reduce fenestration size. We describe Fontan fenestration size reduction using the Atrial Flow Regulator.

Shoshin syndrome: two case reports representing opposite ends of the same disease spectrum.

Thiamine deficiency can have cardiovascular and neurological manifestations. Cardiac beriberi is classically thought to represent a high-output state with oliguria and lactic acidosis. The condition can, however, also present itself with a low cardiac output and fulminant vascular collapse, or as an acute fatal form, causing sudden death, without clear-cut signs of cardiomegaly. In the western society beriberi is mainly encountered in alcoholics. We report on two cases, one with high-output failure and the other with low-output failure and cardiovascular collapse. In both patients the diagnosi

Pulmonary blood flow distribution in lobar hypoxia--influence of cardiac output and nitric oxide inhalation.

Inhaled NO is reported to be less effective in patients with ARDS if cardiac output is high (> 10 L/min). It has also been demonstrated that increased blood flow and increased shear stress cause an enhancement of endogenous NO production. In one-lung ventilation and regional hypoxia, nitric oxide (NO) delivered to the ventilated lung may decrease blood flow to the nonventilated lung and improve arterial oxygenation. So far, however, results have been divergent. The present study was performed with the hypothesis that inhaled NO would be less effective if cardiac output was increased. In the an

[Natural history of primary pulmonary hypertension elucidated by pulmonary hemodynamics].

The present study was undertaken to clarify the natural history of primary pulmonary hypertension (PPH) from a hemodynamic point of view. The subjects consisted of 83 patients (18 men and 65 women), whose ages ranged from 14 to 69 years and averaged 33 years. They were contacted through a nationwide survey. All patients underwent right-sided cardiac catheterization; cardiac output was measured in 52 patients and pulmonary capillary wedge pressure, in 40 patients. The following results were obtained. 1. The patients who died within three months of their cardiac catheterization were in severe ri

Mechanisms of neurohormonal activation in chronic congestive heart failure: pathophysiology and therapeutic implications.

Patients with chronic congestive heart failure have a sequential and incessant activation of those neurohormonal systems, which control body fluids, cardiac output and systemic blood pressure. Neurohormonal activation is initially selective and regional. Generalized activation is a late event in the natural history of congestive heart failure. Although the ultimate stimulus responsible for the activation of these neurohormonal systems is unknown, a decreased cardiac output and diminished effective blood volume have been proposed as the responsible mechanisms. However, extensive clinical and ex

Beyond mitral regurgitation reduction: Divergent hemodynamic and structural responses to transcatheter edge-to-edge repair in primary and secondary mitral regurgitation.

BACKGROUND: Severe mitral regurgitation is associated with reduced forward cardiac output. Transcatheter edge-to-edge repair reduces regurgitation severity, but its effects on resting systemic hemodynamics may differ according to etiology. This study evaluated changes in cardiac structure, cardiac output, and systemic oxygen transport six months after transcatheter edge-to-edge repair in patients with primary or secondary mitral regurgitation. METHODS: We prospectively studied 195 patients with severe mitral regurgitation and low forward cardiac output undergoing successful transcatheter edge

The effect of milrinone on hemodynamic and gas exchange parameters in children.

Milrinone is a drug frequently used for hemodynamic support in children during critical illness. Although the hemodynamic changes induced by milrinone in children may appear similar to those of adults, the physiologic contributors of these changes remain vastly unknown. A systematic review was conducted to identify studies characterising the hemodynamic effects of milrinone in children during critical illness for hemodynamic support for various medical conditions. Studies were assessed for quality and those of satisfactory quality with pre- and post-operative hemodynamics for each patient were

Recurrent acute heart failure caused by sliding hiatus hernia.

The case is reported of a 75 year old woman who presented with recurrent nocturnal episodes of acute pulmonary oedema. The cause was uncertain as she had normal cardiothoracic ratio on chest radiography and normal left ventricular systolic and diastolic function by transthoracic echocardiogram. Another transthoracic echocardiogram was repeated when she was recumbent for an hour and had a full stomach. It showed a striking finding of severe left atrial compression by an external structure. Computed tomography of the thorax showed an intrathoracic mass behind the left atrium causing external com

Accidental tenfold overdose of propofol in a 6-month old infant undergoing elective craniosynostosis repair.

We report a 6-month-old male infant undergoing elective craniosynostosis repair who accidentally received a tenfold dose of propofol over a 4-h operative period. Myocardial dysfunction was observed after nearly 3 h of infusion; this could not solely be explained by the propofol overdose.

The failing human heart does not release nitrogen oxides.

It has been suggested that the myocardial production of nitric oxide, as a consequence of expression of the inducible isoform of nitric oxide synthase (NOS), plays an important role in the pathophysiology of heart failure. We determined the net cardiac production of nitrogen oxides (NOx), as a measure of NOS activity, by performing arterial and coronary sinus sampling in healthy control subjects (n=6) and patients with end-stage heart failure (n=10). The arterial plasma NOx concentration was significantly elevated in heart failure patients (58.4 +/- 7.0 vs 36.9 +/- 4.9 microM, p<0.05). However

Intraaortic balloon pump after treatment of anomalous origin of left coronary artery.

The same system of intraaortic balloon pumping has been used in 2 infants after surgical repair of anomalous origin of the left coronary artery from the main pulmonary artery. Intraaortic balloon pumping was started in the first patient on the second postoperative day, whereas in the second patient it was started before the patient was weaned from cardiopulmonary bypass. Both patients survived. In the first patient intraaortic balloon pumping improved the low cardiac output, which had failed to respond to maximal inotropic support, whereas in the second it maintained hemodynamic stability.

Surgical therapy for heart failure: care of the patient with partial left ventriculectomy.

Partial left ventriculectomy (PLV) surgery has gained significant attention worldwide as a possible alternative to heart transplantation or as a therapy to treat heart failure. This procedure offers an attractive choice to patients and physicians because of the lack of need for immunosuppressive medications and a decrease in the risk of rejection associated with transplantation. PLV surgery is currently under investigation in several centers worldwide, but long-term outcomes have yet to be definitively evaluated. The Cleveland Clinic Foundation began performing PLV surgery in May 1996. Since t

Effects of contrast media on myocardial function during acute left ventricular failure in the dog.

We studied the hemodynamic side effects of intracoronary injection of contrast media during acute ischemic heart failure by using anesthetized dogs. Induction of failure was performed by microembolization of the area supplied by the left main coronary artery. Six ml of iohexol (Omnipaque) increased contractility during the normal state, while this contrast medium induced no alterations in any of the recorded hemodynamic variables during left ventricular failure. Ioxaglate (Hexabrix) was also well tolerated during the normal state, while sodium-meglumine diatrizoate (Renografin) markedly decrea

[Treatment of left ventricular failure following extracorporeal circulation using ventricular assistance with centrifugal pump].

We report the use of left ventricular assistance with a centrifugal pump in a patient with refractory left ventricular failure after aortic valve replacement. Assistance was maintained during the first 24 hours until the hemodynamic status allowed its withdrawal. The used flows ranged from 1.5 to 5 l/min, and there were no complications except hemorrhage (95 ml in 3 hours) that could be stopped without reoperation.
