Cardiac surgery 2016 reviewed.

For the year 2016, more than 20,000 published references can be found in Pubmed when entering the search term "cardiac surgery". Publications last year have helped to more clearly delineate the fields where classic surgery and modern interventional techniques overlap. The field of coronary bypass surgery (partially compared to percutaneous coronary intervention) was enriched by five large prospective randomized trials. The value of CABG for complex coronary disease was reconfirmed and for less complex main stem lesions, PCI was found potentially equal. For aortic valve treatment, more evidence

An alternative "no resection" technique for posterior mitral leaflet prolapse repair: reverse T-plasty.

Reverse T-plasty is an alternative "no resection" technique for posterior mitral leaflet prolapse repair that is inspired by butterfly resection. It combines mediolateral and anteroposterior plane foldings of the posterior leaflet without any resection and shortens cardiopulmonary bypass and cross-clamping time.

[Conservative aortic valve surgery].

Conservative aortic valve surgery is an attractive approach for the correction of the aortic insufficiency and the dilation of the aortic root. Despite being a surgery made in a minority of the patients it's receiving increased attention in the last decade because it can minimize the complications and risks that are inherent to the use of a mechanic or biologic valve. In the following article we propose ourselves to review the actual state of knowledge regarding conservative aortic valve surgery and the path that this type of surgical procedure must have in order to become the gold standard ap

Multiple triangular resection: a reliable technique for correction of multiple prolapse of the mitral valve.

Quadrangular resection of the posterior leaflet of the mitral valve is a well-established technique for the treatment of mitral regurgitation from prolapse of P2. Recently, Suri described triangular resection of the prolapsing scallop, a technique that, avoiding the plication of the annulus corresponding to the resected leaflet, maintains the geometry of the mitral annulus, allowing a more physiologic function of the mitral valve. In this paper, we report multiple triangular resection for the treatment of multiple prolapse of the posterior leaflet.

Potential implications of the helical heart in congenital heart defects.

The anatomic and functional observations made by Francisco Torrent-Guasp, in particular his discovery of the helical ventricular myocardial band (HVMB), have challenged what has been taught to cardiologists and cardiac surgeons over centuries. A literature debate is ongoing, with interdependent articles and comments from supporters and critics. Adequate understanding of heart structure and function is obviously indispensable for the decision-making process in congenital heart defects. The HVMB described by Torrent-Guasp and the potential impact on the understanding and treatment of congenital

[Mitral valve repair today].

Valve reconstruction, whenever technically feasible, is currently the treatment of choice for mitral valve regurgitation. Preoperative transesophageal echocardiography discloses the precise location and extent of these lesions, making it possible for the surgeon to determine the repairability of the valve and to devise a roadmap for the most appropriate surgical maneuvers. Today, the aim of surgery should not be limited to abolishing regurgitation, but should also comprise restoration of the normal valve geometry in order to ensure durability of the repair. This geometric redress involves a) l

[Conotruncal repair of tetralogy of Fallot under 2 years old].

From October 1985 to September 1990, a repair within the conotruncal portion of the right ventricle-Conotruncal repair-was used for the primary repair of consecutive 34 patients with tetralogy of Fallot under the age of 2 years. There were three summarized points of this maneuver: the first was non-use of the tricuspid septal leaflet for closure of the ventricular septal defect, the second was short patch infundibuloplasty with a large monocusp, and the last was total resection of the infundibular septum. Length of the patch infundibuloplasty was just 30% of the right ventricle length accordin

Determinants of survival following cardiac operations in elderly patients.

Ninety-four cardiac operations were performed on 92 patients 65 years of age and over at the Peter Bent Brigham Hospital, Boston, from July 1970 to July 1974. There were 39 aortic valve replacements, 11 mitral valve replacements, 14 double valve operations, 16 coronary revascularizations, five repairs of aortic dissections, and one atrial septal defect closure. The operative mortality was 11 percent in elective cases and 40 percent in the emergency cases, an overall mortality of 17 percent. There were no hospital deaths following elective double valve operations or coronary bypass procedures.
