Infection of hemodialysis catheters: incidence and mechanisms.

Fifty-three consecutive subclavian or jugular hemodialysis catheters inserted into 41 patients were prospectively studied over a period of 8 months in order to determine the incidence of infection and its mechanisms. The intravascular, intradermal and the Y catheter segments as well as both connections were cultured using a quantitative technique for the intraluminal surface. In addition, the intravascular and intradermal portions of the catheter were cultured using a semiquantitative technique for the external surface. Skin smears of the catheter entry site were also cultured, and blood cultu

[Thromboembolic complications and permanent endocavitary stimulation catheters].

Based on four cases of phlebitis associated with permanent intravenous cardiac stimulation catheters, the authors review the pathophysiology, clinical signs, diagnosis and treatment of this disease. The insertion of the catheter causes varying degrees of radiologically detectable thrombosis in 30 to 50 per cent of cases. The most frequent clinical presentation is the typical picture of phlebitis of the upper limb due to axillo-subclavian thrombosis. However, clinical signs are only present in 0.5 to 5 per cent of cases (1.7 per cent of cases in the present series). The risk of this complicatio

Thromboembolic complications with silicone elastomer subclavian catheters.

Central venous thrombosis is a well-recognized complication of plastic catheters used during parenteral nutrition and occurs in up to 33% of cases if examined venographically. Silicone elastomer catheters, in addition to their favorable handling properties, are less thrombogenic than plastic varieties and are now increasingly used for parenteral nutrition. However, their use does not prevent the possibility of catheter-related thrombosis as this study demonstrates. In a consecutive series of 118 silicone elastomer subclavian catheters, 83% of which were used for parenteral nutrition, two insta

Prophylaxis against microbial colonization of venous catheters.

A total of 85 central venous catheterizations in 73 patients were subjected to a prospective study on prophylaxis against infectious complications. Bacterial growth was found in blood cultures in five cases (6 per cent) and in cultures from catheter tips in 23 cases (27 per cent). The most important route of infections was shown to be the catheter connection. Flaming of the connection when changing the giving set did not reduce the risk of catheter tip colonization. Catheter-related thrombosis was found in 33 of 73 phlebographs (45 per cent). There was a significant relationship (p less than 0

Major complications of percutaneous subclavian vein catheters.

New and reportedly safet techniques for subclavian venipuncture with the passage of central venous catheters appear regularly in the surgical literature [55--59]; yet reports of major complications continue to appear as well. We have reported on eight patients with major complications of percutaneous subclavian vein catheters, two of whom died. In our own hospital an improved educational program for junior house staff and nurses has been instituted. Better supervision of junior house staff when performing this potentially lethal technique is necessary. Daily inspection of catheters, early remo

Comparison of Balloon Guide Catheters and Standard Guide Catheters for Acute Ischemic Stroke: A Systematic Review and Meta-Analysis.

BACKGROUND: Balloon guide catheters (BGCs) are designed to induce flow arrest during mechanical thrombectomy procedures for acute ischemic stroke due to large-vessel occlusion and have been associated with improved clinical and angiographic outcomes. We conducted a systematic review and meta-analysis evaluating the relative technical and clinical outcomes associated with BGC versus non-BGC approaches. METHODS: A systematic review of clinical literature using the PubMed database was undertaken to identify multiarm studies published between 2010 and 2021 reporting the use of BGC versus non-BGC

Comparison of procedural duration among ultrasound-guided IVs, mini-midline catheters, and central venous catheters inserted in the emergency department.

BACKGROUND: Patients with difficult intravenous (IV) access in emergency departments (EDs) often require invasive vascular access techniques, including ultrasound-guided peripheral intravenous catheters (USGIVs), mini-midline catheters (MMCs), and central venous catheters (CVCs). Although prior research has explored the safety and survival time of these vascular access devices (VADs), there is little available literature in comparing their procedural duration and clinical utility. METHODS: A prospective observational study was conducted to evaluate the procedural duration, catheter utilizatio

Intermittent Catheters with Integrated Amphiphilic Surfactant Reduce Urethral Microtrauma in an Ex Vivo Model Compared with Polyvinylpyrrolidone-Coated Intermittent Catheters.

Intermittent catheterization mitigates urinary retention for over 300,000 people in the US every year, but can cause microtrauma in the urothelium, compromising its barrier function and increasing the risk of pathogen entry, which may affect user health. To reduce adverse effects, intermittent catheters (ICs) with increased lubricity are used. A common strategy to enhance IC lubricity is to apply a polyvinylpyrrolidone (PVP) coating to ICs; however, this coating can become adhesive upon drying, potentially leading to microtrauma. An alternative approach for lubricity is the migration of integr

Impact of urinary catheters including reusable catheters on quality of life and work productivity in persons with spinal cord lesion.

While preventing renal damage and symptomatic urinary tract infection are of paramount importance to persons with a spinal cord lesion (SCL), patient-reported outcomes (PROs) are receiving much more attention than ever before. We performed a cross-sectional internet survey of 282 adult members of SCL patient advocacy groups and investigated, for the first time, the impact of different types of urinary catheter, including a Japanese reusable silicone catheter and a reusable intermittent balloon catheter, on PROs of persons with SCL. Intermittent self-catheterization (ISC) did not surpass contin

Monitoring quality of care for peripheral intravenous catheters; feasibility and reliability of the peripheral intravenous catheters mini questionnaire (PIVC-miniQ).

BACKGROUND: Peripheral intravenous catheters (PIVCs) account for a mean of 38% of catheter associated bloodstream infections (CABSI) with Staphylococcus aureus, which are preventable if deficiencies in best practice are addressed. There exists no feasible and reliable quality surveillance tool assessing all important areas related to PIVC quality. Thus, we aimed to develop and test feasibility and reliability for an efficient quality assessment tool of overall PIVC quality. METHODS: The Peripheral Intravenous Catheter- mini Questionnaire, PIVC-miniQ, consists of 16 items calculated as a sum s

Non-central peripherally inserted central catheters in neonatal intensive care: complication rates and longevity of catheters relative to tip position.

BACKGROUND: Peripherally inserted central catheters (PICCs) represent a mainstay of intravascular access in the neonatal intensive care setting when long-term vascular access is needed. Ideally, PICCs should be inserted and maintained in a central position with the tip ending in the superior or inferior vena cava. This is not always achievable, and sometimes the tip remains in a peripheral location. Higher complication rates have been reported with non-central PICCs; however these findings have not been confirmed in a solely neonatal series and PICCs with tips in peripheral veins have not been

In vitro contact force evaluation of robotic magnetic navigation catheters; Head to head comparison of different catheters and different settings.

BACKGROUND: The Stereotaxis Robotic Magnetic Navigation (RMN) system uses an external magnetic field to navigate catheters inside the heart during arrhythmia ablation. This study compared in vitro maximal contact force (CF) achieved with different irrigated RMN ablation catheters under various conditions. METHODS: An in vitro model was created to measure CF using a high-precision goldsmith scale mounted in a dedicated frame between the permanent magnets of the Stereotaxis RMN system. Four irrigated RMN catheters (Navistar RMT, Celsius RMT, Magnoflush, MAGiC™) were evaluated at three magnetic

A Novel Approach to Repositioning Peritoneal Dialysis Catheters.

INTRODUCTION: Peritoneal dialysis (PD) is a crucial kidney replacement therapy for patients with end-stage kidney disease. Despite its advantages over hemodialysis (HD), long-term success can be hindered by catheter dysfunction, which often necessitates revision. Currently, surgical treatment methods for PD catheter malfunction include fluoroscopy-guided procedures and laparoscopic or open surgery to salvage or replace the catheter. Here, we introduce the first novel, minimally invasive surgery for repositioning PD catheters. METHODS: From November 2021 to May 2024, 8 patients with PD cathete
