Ultrasonography is the method of choice for prenatal malformation screening, but it does not always provide sufficient information for correct diagnosis or adequate abnormality evaluation. Fetal magnetic resonance imaging (MRI) is considered a valuable second line imaging tool for confirmation, completion and correction of complex fetal ultrasonography findings. Fetal MRI has proven its value in pathologies of the central nervous system. MR imaging's role includes nor only confirming or excluding possible lesions, but also defining their full extent, aiding in their characterization, and demon…

BACKGROUND: Tuberculosis (TB) of the central nervous system (CNSTB) is associated with higher mortality rates than other forms of TB. Epidemiologic associations with and prognostic indicators of CNSTB have not been assessed in a large US population-based study.
METHODS: Between 1995 and 2004 and using a population-based active surveillance study, we compared patients with CNSTB to patients with TB affecting sites other than CNS (non-CNSTB) with respect to sociodemographic, clinical and Mycobacterium tuberculosis genotype variables. Risk factors associated with mortality at 180 days were compa…

Intracranial and spinal haemangiopericytomas are uncommon, durally based tumours. They macroscopically resemble meningiomas but are distinct histologically, have a more aggressive natural history and require different management. We present a pictorial review illustrating the radiological manifestations of these tumours that will aid in their preoperative identification.

Wegener granulomatosis (WG) is an antineutrophil cytoplasmic antibody (ANCA)-associated granulomatous vasculitis of small and medium-sized vessels. This vasculitis involves mainly the upper and lower respiratory tracts and kidneys, although WG may affect any organ. Central nervous system (CNS) involvement is an uncommon manifestation of WG, reported in 7%-11% of patients. Three major mechanisms have been incriminated as causing CNS disease in WG: contiguous invasion of granuloma from extracranial sites, remote intracranial granuloma, and CNS vasculitis. Herein we describe 6 patients with WG-re…

OBJECTIVES: Toxoplasmosis is identified as one of the major central nervous system (CNS) opportunistic infections in AIDS patients. Sero-epidemiological surveys done among Ethiopian patients indicated a very high prevalence rate. This study was conducted to describe the clinical course of the disease in Ethiopians with AIDS.
PATIENTS AND METHODS: Three hundred and twenty three patients with AIDS and CNS Toxoplasmosis that were consecutively admitted and treated with Sulfadoxine Pyrimethamine (SP) at the Tikur Anbessa Specialized Referral Hospital were reviewed. The diagnosis CNS Toxoplasmosis…

Ehrlichiosis in the United States is caused by three closely related bacterial species (Ehrlichia chaffeensis, Ehrlichia ewingii, and Anaplasma phagocytophilum), all transmitted through tick bite. Although there is variation with respect to geography and tick vector, the clinical manifestations are similar, and treatment of these infections is identical. Ehrlichiosis can present with a spectrum of neurologic manifestations, ranging in severity from headache to meningoencephalitis. Treatment is straightforward if the diagnosis is suspected, but antibiotic therapy should not be delayed pending l

Central nervous system (CNS) neoplasms can be diagnostically challenging, due to remarkably wide ranges in histologic appearance, biologic behavior, and therapeutic approach. Nevertheless, accurate diagnosis is the critical first step in providing optimal patient care. As with other oncology-based specialties, there is a rapidly expanding interest and enthusiasm for identifying and utilizing new biomarkers to enhance the day-to-day practice of surgical neuropathology. In this regard, the field is primed by recent advances in basic research, elucidating the molecular mechanisms of tumorigenesis…

Hypertension, proteinuria and edema, a syndrome called as EPH gestosis or preeclampsia is one of the most common causes of mortality of pregnant women and fetus. The etiology of the disease is not fully understood, the genetic factors are thought to play the main role. Changes in the central nervous system are dangerous complications of the gestosis which could cause convulsive eclampsia and stroke. The neuroimaging techniques, the neurophysiological and ultrasonographic investigations allow to diagnose the neurological complications of gestosis and to begin the treatment (symptomatic, because…

A woman with chronic systemic lupus erythematosus (SLE) was treated with prednisone, and developed an acute febrile neuroinfection. The magnetic resonance images (MRI) showed miliary micronodular lesions distributed diffusely within the central nervous system and lungs. Lumbar puncture showed pleocytosis with predominance of polymorphonuclear leukocytes, hypoglycorrhachia, elevated proteins and smears positive for acid-fast bacilli. The diagnosis was confirmed by culture of Mycobacterium tuberculosis in the cerebrospinal fluid and bronchial biopsy tissue. She was treated with ethambutol, rifam…

With improved therapies, patients with cancer survive longer. However, both the acute complications of intensive therapies and the risks of chronic immunosuppression have led to an increased incidence of central nervous system (CNS) infections. The presentation and course of common infections may be different from those in patients without cancer, and new syndromes related both to the underlying diseases and to their treatment have complicated the differential diagnosis. Noninfectious disorders such as drug treatment complications, vascular lesions, and radiation effects can mimic CNS infectio…

Raloxifene is a selective estrogen-receptor modulator recently launched in Japan as a new medicine for treatment of postmenopausal osteoporosis. Raloxifene stimulates neurite outgrowth in cultured cells and increases hippocampal cholineacethyl trasferase activity and serotonin receptor in the brain of ovariectomized rats. As a part of the Multiple Outcomes of Raloxifene Evaluation trial, 7,478 postmenopausal women with osteoporosis were studied by using six tests of cognitive function. There was some evidence that Raloxifene may lower the risk of a decline in memory and attention. It is expect

INTRODUCTION: Each of the processes involved in the development of the central nervous system is under genetic control and some mutations are responsible for severe malformations. For example, holoprosencephaly is associated with HPE genes, hydrocephalus with L1-CAM and lissencephaly with LIS-1.
DISCUSSION: Molecular genetics has brought new insights into the etiology and pathogenesis of nervous system malformations and has provided new tools for accurate prenatal diagnosis.

Human immunodeficiency virus (HIV) may be the cause of both primary and secondary brain diseases. In this review general features of HIV-associated neuropathology are discussed. Up to 90% of patients with AIDS have a variety of HIV-related brain diseases. Primary brain diseases including lymphocytic meningitis and HIV encephalitis are attributed directly to the effect of the virus on the brain. Secondary diseases including toxoplasmosis, cryptococcosis, primary leukoencephalopathies and lymphomas result from these patients' immunodeficiency status.

Central nervous system (CNS) complications occurring early and late after acute measles are serious and often fatal. In spite of functional cell-mediated immunity and high antiviral antibody titers, an immunological control of the CNS infection is not achieved in patients suffering from subacute sclerosing panencephalitis (SSPE). The known cellular receptors for measle virus (MV) in humans, CD46 and CD150 (signaling lymphocyte activation molecule, SLAM), are important components of the viral tropism by mediating binding and entry to peripheral cells. Because neural cells do not express SLAM an…
