Dose-Dependent Intracranial Response to Sotorasib in KRASG12C-Mutated Lung Cancer Brain Metastases: A Case Report.

INTRODUCTION: KRASG12C inhibitors such as sotorasib have expanded precision-oncology options for non-small cell lung cancer (NSCLC). However, central nervous system (CNS) disease control remains a major clinical challenge. CASE PRESENTATION: We report the case of a female patient with extensively pretreated KRASG12C-mutated lung adenocarcinoma and metachronous progressive brain metastases. Initially, peripheral and cerebral lesions responded to once daily sotorasib 960 mg (standard dose). Following cerebral progression, tolerance-adapted escalation to 1,920 mg achieved renewed intracranial di

How promiscuity promotes tolerance: the case of myasthenia gravis.

Central T cell tolerance in the thymus plays a nonredundant role in preventing organ-specific autoimmunity. This role is largely attributable to promiscuous expression of tissue-restricted auto-antigens in medullary thymic epithelial cells (mTECs). The nascent T cell repertoire is exhaustively screened for potential autoreactivity against these ligands presented by mTECs. Failure of this screening process can result in organ-specific autoimmune diseases affecting single or multiple organs. Here we discuss how promiscuous expression of the acetylcholine receptor alpha-chain in the human thymus

Ecological niche differences between two polyploid cytotypes of Saxifraga rosacea.

PREMISE: Different cytotypes of a species may differ in their morphology, phenology, physiology, and their tolerance of extreme environments. We studied the ecological niches of two subspecies of Saxifraga rosacea with different ploidy levels: the hexaploid Central European endemic subspecies sponhemica and the more widely distributed octoploid subspecies rosacea. METHODS: For both cytotypes, we recorded local environmental conditions and mean plant trait values in populations across their areas of distribution, analyzed their distributions by niche modeling, studied their performance at two

Compromised fidelity of B-cell tolerance checkpoints in AChR and MuSK myasthenia gravis.

OBJECTIVE: Myasthenia gravis (MG) is an autoimmune condition in which neurotransmission is impaired by binding of autoantibodies to acetylcholine receptors (AChR) or, in a minority of patients, to muscle specific kinase (MuSK). There are differences in the dominant IgG subclass, pathogenic mechanisms, and treatment responses between the two MG subtypes (AChR or MuSK). The antibodies are thought to be T-cell dependent, but the mechanisms underlying their production are not well understood. One aspect not previously described is whether defects in central and peripheral tolerance checkpoints, wh

The interaction between electrically induced convulsions and tolerance in the abstinence period after chronic barbital treatments in the rat.

The effect of electrically induced convulsions was tested on the tolerance to hexobarbital after chronic barbital treatment in male rats. In two experiments barbital was given in the drinking water for more than 20 weeks. The dose was around 200 mg/kg/day. Tolerance was tested with an EEG threshold method where hexobarbital is infused intravenously to obtain a criterion of burst suppression. If on the third abstinent day an electrical convulsion was induced 1 h prior to the threshold determination then the hexobarbital thresholds were reduced compared with barbital-treated animals where no con

Autonomic mechanisms associated with heart rate and vasoconstrictor reserves.

INTRODUCTION: Hemorrhage is accompanied by baroreflex-mediated tachycardia and vasoconstriction. The difference between baseline and maximum responses is defined as the heart rate (HR) and vasoconstrictor 'reserve'. OBJECTIVE: To test the hypothesis that higher HR and vasoconstrictor reserves in subjects with high tolerance (HT) to central hypovolemia is associated with greater reserve for sympathoexcitation and vagal withdrawal compared with low tolerant (LT) subjects. METHODS: R-R intervals (RRI), systolic arterial pressure (SAP), estimated stroke volume, and muscle sympathetic nerve activ

Candidate Mechanisms of Cerebral Ischemia/Hypoxia Tolerance: Insights from Hibernating Rodents.

Cerebral ischemia/hypoxia constitutes a central pathological process in ischemic stroke and a range of neurological disorders, yet effective and safe neuroprotective strategies remain scarce in clinical practice. Hibernating rodents repeatedly undergo extreme physiological fluctuations, including marked reductions in cerebral blood flow and subsequent reperfusion, during torpor-arousal cycles, yet they do not exhibit overt neurological deficits upon emergence from hibernation. This unique phenotype renders them a valuable natural model for investigating endogenous tolerance to cerebral ischemi

Cannabinoid tolerance relies on CB1 receptor ubiquitination by NEDD4L.

Cannabinoids, the active components of cannabis, exert numerous acute effects in the brain by engaging cannabinoid CB1 receptors (CB1Rs). However, tolerance emerges rapidly after repeated drug exposure, undermining the efficacy of cannabinoid-based therapies and contributing to cannabis-associated adverse effects. Although the processes of CB1R short-term desensitization (i.e., receptor uncoupling and internalization) are well characterized, the mechanisms underlying CB1R long-term tolerance (i.e., downregulation of receptor protein levels) remain elusive. Here, we identify a ubiquitin-depende

Synergistic venetoclax and anti-CD3 therapy for control of alloreactive response in hematopoietic stem cell transplantation.

T cell-based immunotherapies are effective for treating hematological malignancies and promoting lifelong graft tolerance after solid organ transplantation. Recipient preconditioning resulting in profound lymphodepletion is a key to preventing graft rejection and maximizing T cell engraftment. The 2 standard lymphodepleting agents, antithymocyte globulin and cyclophosphamide chemotherapy, are associated with major adverse effects in patients. This research investigates a new therapeutic approach that combines anti-CD3 (aCD3) therapy with venetoclax, as a BCL-2 inhibitor and T cell immunoglobul

Donor derived hematopoietic stem cell niche transplantation facilitates mixed chimerism mediated donor specific tolerance.

Compelling experimental evidence confirms that the robustness and longevity of mixed chimerism (MC) relies on the persistence and availability of donor-derived hematopoietic stem cell (HSC) niches in recipients. Based on our prior work in rodent vascularized composite allotransplantation (VCA) models, we hypothesize that the vascularized bone components in VCA bearing donor HSC niches, thus may provide a unique biologic opportunity to facilitate stable MC and transplant tolerance. In this study, by utilizing a series of rodent VCA models we demonstrated that donor HSC niches in the vascularize

Repeated Injections of IL-2 Break Renal Allograft Tolerance Induced via Mixed Hematopoietic Chimerism in Monkeys.

Tolerance of allografts achieved in mice via stable mixed hematopoietic chimerism relies essentially on continuous elimination of developing alloreactive T cells in the thymus (central deletion). Conversely, while only transient mixed chimerism is observed in nonhuman primates and patients, it is sufficient to ensure tolerance of kidney allografts. In this setting, it is likely that tolerance depends on peripheral regulatory mechanisms rather than thymic deletion. This implies that, in primates, upsetting the balance between inflammatory and regulatory alloimmunity could abolish tolerance and

Regulatory T cells, leptin and angiogenesis.

Regulatory T cells (Treg cells) are crucial in mediating immune homeostasis and promoting the establishment and maintenance of peripheral tolerance. Excess body weight and obesity are typified by 'low-degree' chronic inflammation and are associated with an increased risk of atherosclerosis, diabetes, fatty liver disease, autoimmune diseases and cancer. All these pathological conditions are characterized by chronic inflammation, abnormal cytokine production, elevated acute-phase reactants, and the activation of several inflammatory signaling pathways. In this context, the discovery of the adipo

Morphine-tolerant mice exhibit a profound and persistent cardioprotective phenotype.

BACKGROUND: Morphine and other opioids continue to be used as the major treatment for acute pain both before and after surgery. In this regard, much research has focused on the mechanisms of morphine tolerance and dependence in the central nervous system; however, few studies have examined the effect of morphine on peripheral organs, such as the heart, in morphine-tolerant animals. Here, we examine the effect of tolerance to the analgesic effect of morphine on ischemic tolerance in mice after prolonged morphine exposure and withdrawal. METHODS AND RESULTS: Male C57/BL6 mice were implanted sub

Role of endothelin in neonatal morphine tolerance.

Management of neonatal opioid tolerance and withdrawal symptoms remains a major challenge in neonatal intensive care units. We provide evidence that central endothelin (ET) mechanisms are involved in the development of morphine tolerance in neonatal rats. Pregnant rats were rendered tolerant to morphine and rat pups were delivered by cesarean section. The effect of morphine tolerance on characteristics of ET receptors in neonatal rats was determined. The affinity (Kd) and density (Bmax) of [I]ET-1 binding in the brain was found to be similar in placebo and morphine-tolerant neonatal rats. Morp

Tolerance to the antinociceptive effect of Crotalus durissus terrificus snake venom in mice is mediated by pharmacodynamic mechanisms.

Crotalus durissus terrificus venom exerts central and peripheral antinociceptive effect mediated by opioid receptors. The present work investigated the tolerance to the antinociceptive effect of the venom and characterised the mechanisms involved in this phenomenon. The hot plate test, applied in mice, was used for pain threshold determination. The venom (200 microg/kg) was administered by oral route, daily, for 14 days, and the nociceptive test was applied before and on days 1, 7 and 14 of the treatment. Prolonged treatment with venom lead to the development of tolerance to the antinociceptiv

Maintenance of immune tolerance to a neo-self acetylcholine receptor antigen with aging: implications for late-onset autoimmunity.

Age-related changes in immune regulation are likely to account for the age-associated increase in serum autoantibody levels and in certain a … the proportion of T regulatory cells; it is speculated that this may compensate for deficiencies in central tolerance that occur owing to thymic involution. In summary, our study reveals, for the first time, that some immune tolerance mechanisms do survive aging; this suggests that certain late-onset autoimmune disorders may be induced by a specific insult that dis …

Thymic epithelial tumors: what's next?

Thymic epithelial tumors (TETs) are rare malignancies arising in an organ central to T cell development and immune tolerance, creating a distinctive interface between tumor biology and immunotherapy. Complete surgical resection remains the mainstay for resectable disease, whereas systemic treatment options for unresectable, recurrent, and metastatic TETs remain limited. The unique immunobiology of thymoma, particularly impaired central immune tolerance, may account for the unusual balance between antitumor activity and severe immune-related adverse events observed with immune checkpoint blocka

An aberrant prostate antigen-specific immune response causes prostatitis in mice and is associated with chronic prostatitis in humans.

Chronic prostatitis is a common disease of unclear etiology and has no specific treatment. Mice deficient in the expression of the autoimmune regulator (Aire) gene, which are defective in thymic expression of self antigens and central tolerance, develop spontaneous prostatitis. In this study, we found that Aire-deficient mice developed spontaneous B and T cell immune responses to a prostate autoantigen, seminal vesicle secretory protein 2 (SVS2), which we believe to be novel. We show that thymic expression of this self antigen was Aire dependent. Moreover, prostatitis was induced in WT mice th…

Interleukin 35 Regulatory B Cells.

B lymphocytes play a central role in host immunity. They orchestrate humoral immune responses that modulate activities of other immune cells and produce neutralizing antibodies that confer lasting immunity to infectious diseases including smallpox, measles and poliomyelitis. In addition to these traditional functions is the recent recognition that B cells also play critical role in maintaining peripheral tolerance and suppressing the development or severity of autoimmune diseases. Their immune suppressive function is attributed to relatively rare populations of regulatory B cells (Bregs) that
