Cerebral Digital Subtraction Angiography in Acute Intracranial Hemorrhage: Considerations in Critically Ill Children.

Cerebrovascular disorders are an important cause of morbidity and mortality in children. Although minimally invasive, cerebral digital subtraction angiography (DSA) has been shown to be safe in children and is a valuable, and perhaps underutilized, technique for the diagnosis and management of pediatric cerebrovascular disorders in the critical care setting. Through a case-based approach, we explore the utility of DSA in critically ill children with acute intracranial hemorrhage (ICH). We discuss the use of DSA in the acute management of aneurysm and arteriovenous malformation rupture as well

Multiple cerebral arterial variations incidentally detected by magnetic resonance angiography: a case report.

We herein report a case of multiple variations, including (1) anterior communicating artery duplication, (2) accessory anterior cerebral artery (ACA), (3) distal origin right accessory middle cerebral artery (MCA) with bifurcation, (4) proximal origin left accessory MCA with bifurcation, (5) right superior cerebellar artery (SCA) duplication, and (6) left SCA early bifurcation. These variations were found incidentally by magnetic resonance (MR) angiography. Volume-rendering images of MR angiography were more useful than maximum-intensity-projection images for identifying these variations, espe

Anterior Cerebral Artery and Anterior Communicating Artery Variations: Assessment with Magnetic Resonance Angiography.

BACKGROUND: Although anterior cerebral artery (ACA) and anterior communicating artery (AcomA) variations have been frequently investigated by contrast-enhanced angiography, the number of studies performed with magnetic resonance angiography is quite low. Our aim in this study is to investigate ACA and AcomA variations with time of flight magnetic resonance angiography (TOF MRA). METHODS: Source and maximum intensity projection images of 581 adult patients (273 males, 308 females) who were scanned for intracranial arteries with the TOF MRA method were retrospectively analyzed. RESULTS: We fou

Efficacy of dual antiplatelet therapy as premedication before diagnostic cerebral digital subtraction angiography.

OBJECTIVE: Several studies have reported that periprocedural dual antiplatelet therapy lowers the incidence of thromboembolic complications (TEC) associated with coiling of unruptured aneurysms. We hypothesized that preprocedural administration of dual antiplatelet agents (aspirin and cilostazol) for 7days may reduce the risk of complications associated with diagnostic cerebral digital subtraction angiography (DSA). METHODS: We retrospectively reviewed the records of patients who underwent diagnostic cerebral DSA between September 2015 and April 2018. Of the 419 patients included (149 men, 27

Enhanced susceptibility-weighted angiography (ESWAN) of cerebral arteries and veins at 1.5 Tesla.

OBJECTIVE: Enhanced susceptibility-weighted angiography (ESWAN) is a three-dimensional (3D) multi-echo gradient-echo sequence which consists of both magnitude and phase images. This study aims to demonstrate the feasibility of ESWAN for the depiction of both cerebral arteries and veins at 1.5 T by comparing with time-of-flight (TOF) MR angiography (MRA) and MR venography (MRV). METHODS: 13 healthy volunteers underwent both ESWAN and 3D-TOF-MRA examinations. Among them, nine volunteers underwent an additional two-dimensional-TOF-MRV examination. With regard to the ESWAN sequence, both maximum

Insufficient platelet inhibition is related to silent embolic cerebral infarctions after coronary angiography.

BACKGROUND AND PURPOSE: Considering that insufficient platelet inhibition is related to thrombotic complications after coronary angiography, we hypothesized that the extent of platelet inhibition by antiplatelet agents is related to the occurrence of silent embolic cerebral infarction (SECI) after coronary angiography. METHODS: Among the patients scheduled for coronary artery bypass surgery, we retrospectively analyzed the location of SECI on diffusion-weighted imaging of 272 patients, which was performed after coronary angiography, as a presurgical evaluation in Phase 1 study. In Phase 2 stu

Anaesthetic management of total craniopagus twins for magnetic resonance imaging and cerebral angiography.

We describe the anaesthetic management of 4-yr-old total craniopagus twins for radiological investigations. There are some unique anaesthetic problems associated with this condition. These include cross-circulation between the twins that results in induction of both the twins after the administration of i.v. induction agent to one twin and difficulty in mask ventilating both the twins simultaneously due to the angle between the heads; different arterial pressures in the two children complicate pharmacological management and underline the importance of physiological measures to control arterial

Angiography-negative primary central nervous system vasculitis: a syndrome involving small cerebral vessels.

Primary central nervous system vasculitis (PCNSV) is a rare and poorly understood syndrome. We describe the clinical findings in 8 patients who appear to have a distinct subset of PCNSV. We identified 101 consecutive patients with PCNSV who were seen between January 1, 1983, and December 31, 2003. The diagnosis was based on conventional angiography in 70 patients and on central nervous system biopsy in 31 patients. Six of the 31 patients also had angiograms showing changes of vasculitis. Thus, 76 patients of the cohort had abnormal angiograms. Eight of the 101 patients had normal angiograms ("

[Results of cerebral angiography performed after neuroradiological indications in children with CNS diseases].

INTRODUCTION: Despite of the constant progress in the imaging of vessels, cerebral angiography characterises with the best specificity and sensitivity in detection of arterial, arteriovenous malformations and aneurysms. THE AIM: The aim of this study was to compare the results of cerebral angiography and other neuro-radiologic methods in cases suggesting the probability of vascular malformations in children with neurologic syndroms. MATHERIAL AND MEHTODS: In the year 2006 cerebral angiography was performed in 20 children since it was advised after radiological examinations in children hospita…

Our own experience with CT angiography in early diagnosis of cerebral vascular malformations.

Computed tomography (CT) and intraarterial cerebral angiography are essential methods in early diagnosis of cerebral vascular malformations. In recent years however non-invasive or minimally invasive methods like MR angiography and CT angiography (CTA), which could potentially replace angiography, have been developed. The aim of presented study is to demonstrate our own experience in application of CTA in early diagnosis of cerebral vascular malformations. The material consists of 86 CTA examinations performed shortly after non-traumatic intracranial haemorrhage. Angiographic correlation has b…

Right retrograde brachial cerebral angiography with simultaneous compression of the left carotid artery.

Right retrograde brachial angiography with simultaneous compression of the left common carotid artery was performed in 12 patients, invariably resulting in filling of the right vertebral and the basilar artery. In all but one patient, the right carotid artery and its branches were also filled. Retrograde filling of the left internal carotid artery occurred in 8 patients. Furthermore, retrograde filling of the intracranial part of the left vertebral artery was obtained in 5 of 12 patients. A complete four-vessel cranial angiography was thus obtained in one third of the patients. The method may

In-vivo Assessment of the Proximal Branches of the Anterior Cerebral Artery Using Rotational Angiography.

BACKGROUND: The anatomy of the proximal anterior cerebral artery (ACA) and its branches, including the recurrent artery of Heubner (RAH) and medial lenticulostriate arteries (MLSAs), is known for frequent variations. Impairment of these branches can result in severe consequences, including neurological deficits or cognitive impairment. This study aimed to analyze these branches and their variations in vivo, using data from 3D rotational angiographies (3D-RA). MATERIAL AND METHODS: We reviewed 3D-RAs of 209 hemispheres from 191 patients with pathologies remote from the region of interest. The

Importance of cerebral angiography in the evaluation of delayed carotid stent thrombosis: a case report.

BACKGROUND: While noninvasive imaging is typically used during the initial assessment of carotid artery disease, digital subtraction angiography remains the gold standard for evaluating carotid stent thrombosis and stenosis (Krawisz in Cardiol Clin 39:539-549, 2021). This case highlights the importance of digital subtraction angiography for assessing carotid artery stent patency in place of non-invasive imaging. CASE PRESENTATION: We present a 61-year-old African American male patient with a history of right cervical internal carotid artery dissection that was treated with carotid artery sten

Reliability of cerebral radioisotopic angiography and of directional Doppler C.W. in the diagnosis of cerebrovascular insufficiency.

The diagnostic value of radioisotopic cerebral angioscintigraphy (R.A.) and of Doppler CW (D.C.W.) techniques to identify stenosis of the internal carotid artery in its extracranial course was studied in 97 patients with ischaemic lesions (50 T.I.A. and 47 Complete Stroke). The results of R.A. and D.C.W. were compared with those of contrast carotid-angiography (C.A.). C.A. revealed stenosis above 50% or complete occlusion in 22% of cases, whereas D.C.W. and R.A. showed flow reduction in 27% and 48% of the cases respectively. In T.I.A., C.A. positivity went down to 8%; D.C.W. to 16%; and R.A. t

An investigation with serial angiography into the evolution of cerebral arterial spasm following aneurysm surgery.

Serial postoperative angiograms were performed in 28 patients with intracranial aneurysms, 26 of whom had presented with a subarachnoid hemorrhage. The clinical state and intracranial pressure (ICP) were also measured. Angiograms were performed in the ward using a cannula, which was passed into the proximal external carotid artery via the superficial temporal artery. Measurements of the vessel diameters were made, with the preoperative angiogram as a baseline. Patients could be placed into one of five groups depending on the presence or absence of significant arterial spasm, the clinical state
