Research progress on risk factors related to intracranial artery, carotid artery, and coronary artery stenosis.

In recent decades, with the rapid development of economy, the acceleration of social aging and urbanization, and the prevalence of unhealthy lifestyles, the number of patients with cardiovascular and cerebrovascular diseases has shown an increasing trend year by year. It has also become one of the important causes of disability and death in all ages and groups. Atherosclerosis is the main pathological change of ischemic cardiovascular and cerebrovascular diseases, which mainly invades the large and medium arteries of the body circulation. In particular, cerebral artery and coronary artery lesi

[A case of aortitis syndrome with coronary steal syndrome due to collateral circulation from the right coronary artery to intracranial vessels].

A 44-year old female with aortitis syndrome complained of precordial pain on effort. Exercise electrocardiograms revealed significant ST segment depression in leads II, III, aVF and V. Coronary arteriograms demonstrated no stenosis. However, the right coronary arteriogram revealed collateral circulation arising from the sinus node artery to the bilateral vertebral arteries and the left internal carotid artery. Collateral vessels in aortitis.syndrome arising from the coronary artery to the lung have been reported sporadically. However, to our knowledge, the collateral circulation from the coron

Cerebrovascular injuries in traumatic brain injury.

Traumatic brain injury is a complex and highly heterogeneous disease due to the host of concomitant injuries that may accompany the initial insult. Due to the dynamic interplay between the injuries that may arise, the management of these injuries is challenging. In a small subset of patients with traumatic brain injury, cerebral vascular injury may occur, which presents its own diagnostic and therapeutic challenges. These vascular injuries often present in a delayed fashion, thereby going unnoticed by clinicians. Early recognition and treatment of these injuries is crucial, given their high mo

Involvement of cerebrovascular abnormalities in the pathogenesis and progression of Alzheimer's disease: an adrenergic approach.

BACKGROUND: Until recently, treatments for older patients with AML ineligible to receive intensive chemotherapies were limited to hypomethylating agents, low-dose cytarabine (LDAC), or clinical trials. In 2018, the FDA approved combination glasdegib (GLAS) plus LDAC based on Phase II results demonstrating improved overall survival (OS) versus LDAC alone in previously untreated AML. However, no randomized clinical trials have directly compared GLAS + LDAC with other AML treatments. OBJECTIVE: Using both indirect treatment comparison (ITC) and simulated treatment comparison (STC), which adjusts

[A role of the autonomic nervous system in cerebro-cardiac disorders].

The authors consider anatomical/physiological characteristics and a role of different autonomic CNS regions, including insula cortex, amygdala complex, anterior cingulate cortex, ventral medial prefrontal cortex, hypothalamus and epiphysis, involved in the regulation of cardiovascular activity. The damage of these structures, e.g., due to the acute disturbance of cerebral blood circulation, led to arrhythmia, including fatal arrhythmia, in previously intact myocardium; systolic and diastolic dysfunction, ischemic changes considered in the frames of cerebro-cardial syndrome. FLAIR vascular hyperintensities and dynamic 4D angiograms for the estimation of collateral blood flow in posterior circulation occlusion.

INTRODUCTION: The objectives of this paper are to assess collateral blood flow in posterior circulation occlusion by MRI-based approaches (fluid-attenuated inversion recovery (FLAIR) vascular hyperintensities (FVHs), collateralization on dynamic 4D angiograms) and investigate its relation to ischemic lesion size and growth. METHODS: In 28 patients with posterior cerebral artery (PCA) and 10 patients with basilar artery (BA) occlusion, MRI findings were analyzed, with emphasis on distal FVH and collateralization on dynamic 4D angiograms. RESULTS: In PCA occlusion, distal FVH was observed in 1

The axial imaging plane--the main domain of the transcranial color-coded duplex ultrasonography?

Transcranial color-coded duplex ultrasonography (TCCS) makes possible the visualization of basal cerebral arteries through color-coding the flow velocity information. This method is well established in the clinical routine for the diagnostics of pathological processes in cerebrovascular disease. The present review describes the examination technique, normal and pathological findings, such as stenosis and occlusion of intracranial arteries, as well as intracranial vascular malformations focussing on the advantages of the examination in the axial imaging planes.

Brain damage during cardiopulmonary bypass.

The development of systems that allow cardiopulmonary bypass have been responsible for the growth of our specialty. In recent years continuing reduction in the mortality associated with cardiac operations has reinforced our confidence in the reliability and safety of perfusion equipment. Cardiac surgeons are aware that the mortality for most cardiac surgical procedures has decreased dramatically and overall morbidity has been reduced significantly. However, it is still clear that cardiopulmonary bypass techniques are not perfect. Indeed, it is fair to say that the period of bypass still contri

Transcranial Doppler ultrasonography: clinical applications in cerebrovascular disease.

Transcranial Doppler ultrasonography was introduced in 1982 as a noninvasive procedure for assessment of the intracranial cerebral circulation. The lightweight and portable equipment used for transcranial Doppler examination facilitates its use in the bedside assessment of critically ill hospitalized patients and outpatients. Clinical applications include the diagnosis of vasospasm in patients with subarachnoid hemorrhage, assessment of intracranial collateral flow in patients with extracranial arterial occlusive disease, detection of intracranial arterial stenosis, identification of the feedi

[Comparison of Doppler velocimetry of the common carotid artery with cerebral blood flow measured by PET].

The cervical carotid Doppler velocity is determined by the cardiovascular function, hemodynamic characteristics of proximal vascular beds, the carotid artery, and the distal vascular beds, i.e., cerebral circulation. If the former three factors remain constant, cervical carotid Doppler velocimetry reflects the cerebral circulation. The purpose of this report is to study whether the cervical carotid Doppler velocimetry reflects cerebral circulation or not. Parameters measured by the common carotid Doppler velocimetry were compared with cerebral blood flow (CBF) in the internal carotid arterial

[The hematocrit and cerebrovascular accidents].

That an elevated haemoglobin or haematocrit is a risk factor in stroke is suggested by the prospective epidemiological evidence from Framingham. Our own study of a series of patients with transient ischaemic attacks revealed that even when due allowance was made for interaction between haematocrit, hypertension and smoking, there was still evidence that both male and female patients had a higher haematocrit than age and sex matched controls. Toghi's autopsy study further shows that cerebral infarction is more frequently found in the elderly population in subjects with a high haematocrit, the e

Comparison of continuous-wave Doppler imaging, oculopneumoplethysmography, and the cerebrovascular Doppler examination.

Previous reports of continuous-wave Doppler imaging have combined that technique with periorbital Doppler studies. In the present report, 75 patients with findings suggestive of cerebrovascular insufficiency were evaluated using continuous-wave Doppler imaging, oculopneumoplethysmography (OPG-G) and the cerebrovascular Doppler examination (CDE). Each test was interpreted independently. Doppler imaging had an overall accuracy of 85%, with a 68% accuracy in identifying 50-70% stenoses and a 69% accuracy in identifying high-grade stenoses (greater than 70%) and occlusion. OPG-G had an overall acc

[Changes in psychological performance parameters of concentration and critical flicker-fusion frequency with advancing age and in relation to diffuse cerebral arterial affections of the circulation].

Psychometric performance parameters and the critical flicker-fusion frequency are studied in various groups and in patients with chronic cerebral circulation trouble. Test d2 proves to be effective in the measuring of such impaired performance. There is a higher degree of correlation with Paul's addition test. A correlation between the performance parameters and the patient's age can be statistically substantiated. Among patients up to one third in the age group 55 and over show and affection of the cerebral performance turbances in spite of the absence of pathological clinical findings. The f

The "hot stroke" and transient vascular occlusions.

Two patients with left middle cerebral artery (MCA) distribution infarctions fulfilled the "hot stroke" criteria on sequential 99m Tc pertechnetate flow and static gamma camera studies. The radioisotopic finding of a paradoxical relative increased flow to the affected hemisphere correlated with serial angiography showing multiple MCA branch occlusions becoming patent with a vascular blush and early venous drainage. The angiographical finding of transient vascular occlusions suggests embolism. Using the present cases together with prior observations, some transient vascular occlusions can be di

Endovascular treatment of basilar artery stenosis due to cerebral vasculopathy related to neurofibromatosis (NF1).

BACKGROUND: Cerebrovascular lesions are uncommon in neurofibromatosis type 1 (NF1). CASE DESCRIPTION: We report a case of 34-year-old man with NF1 who developed posterior circulation stroke. Diffusion-weighted imaging showed acute infarcts in the right vertebra basilar artery territory. Digital subtraction angiography demonstrated significant stenosis of the basilar artery in the mid segment that was identified as the etiology of the symptoms. The vertebral arteries were tortuous and the basilar artery was ectatic. Subsequently endeavour resolute stent was placed across the lesion and post-pr

Usefulness of transcranial Doppler ultrasound in evaluating cervical-cranial collateral circulations.

Transcranial Doppler (TCD) ultrasound is a noninvasive, safe and cost-effective bedside test for evaluating cerebrovascular circulation in real time. It has been rapidly evolving from a simple noninvasive diagnostic tool to an imaging model with a broad spectrum of clinical applications. TCD can show the spectral flow waveforms, blood flow direction, velocities and intensity in the intracerebral vessels, adding physiologic information to other imaging models. TCD can also detect collateral channels through the anterior communicating artery, posterior communicating arteries, reversed ophthalmic…

Vertebral artery stenting following percutaneous transluminal angioplasty. Technical note.

The authors report initial results and follow up using stent placement to treat atherosclerotic stenosis in vertebral arteries. Three patients with severe atherosclerotic vascular disease underwent vertebral artery stent placement using a balloon expandable stent. Medical therapy (aspirin and warfarin) and conventional percutaneous angioplasty failed to resolve the disease and the patients developed symptomatic restenosis within 3 months of angioplasty. Two patients had symptoms of anterior circulation ischemia with carotid artery occlusions and reduced supply to the anterior circulation from
