D-penicillamine for primary sclerosing cholangitis.

BACKGROUND: Primary sclerosing cholangitis is a cholestatic disease. D-penicillamine is suggested as a treatment option due to its copper reducing and immunomodulatory potential. OBJECTIVES: To evaluate the beneficial and harmful effects of D-penicillamine for patients with primary sclerosing cholangitis. SEARCH STRATEGY: Eligible trials were identified through searches of The Cochrane Hepato-Biliary Group Controlled Trials Register (August 2005), The Cochrane Central Register of Controlled Trials in The Cochrane Library (Issue 3, 2005), MEDLINE (1950 to August 2005), EMBASE (1980 to August …

Aggressive systemic mastocytosis mimicking sclerosing cholangitis.

A 43 year-old woman presented with fever, abdominal pain, epato-splenomegaly, ascites, cholestasis, anemia, thrombocytopenia and previous diagnosis of sclerosing cholangitis based on liver biopsy and endoscopic retrograde cholangiopancreatography(ERCP). The bone marrow biopsy and the revision of liver biopsy using antitryptase stain diagnosed systemic mastocytosis. Because of the aggressive course of the disease the patient was treated with an acute myeloid leukaemia chemotherapy regimen without success.

[Choledochal malignant hemorrhagic cyst, with cholangitis].

The increased risk of cholangiocarcinoma in patients with congenital cystic disease of the biliary tree is well documented. The reason for the malignant transformation is taught to de related to chronic inflammation and bacterial exposure within bile ducts. Total excision of the choledochal cyst notably reduces the risk of cancer. Five to twenty percent of adult patients with unexcused cysts or treated with by pass will develop cholangiocarcinoma. We are presenting the case of a young female diagnosed with a choledochal cyst at age of four, treated with by pass procedure. Fourteen years later

[Primary sclerosing cholangitis--diagnosis and therapy].

Primary sclerosing cholangitis (PSC) is a chronic cholestatic liver disease of unknown origin, characterized by inflammation, fibrosis, and obliteration of bile ducts, which ultimately results in biliary cirrhosis. The condition most commonly affects intrahepatic and extrahepatic bile ducts together, but sometimes only intrahepatic or extrahepatic ducts are involved. PSC is often associated with inflammatory bowel disease, especially ulcerative colitis. The majority of patients are initially asymptomatic, and identified on the basis of elevated serum levels of alkaline phosphatase or gamma-glu…

Animal models for primary sclerosing cholangitis.

Since the aetiopathogenesis of primary sclerosing cholangitis (PSC) in humans remains undefined, investigators have studied a variety of animal models to gain insights into immunopathogenetic mechanisms associated with obliterative fibrous cholangitis of intra- and extra-hepatic bile ducts. To date, no animal model has been developed that exhibits all of the attributes of PSC. Rodent models instigated by bacterial cell components or colitis are promising because they may help to explain the strong association between PSC and inflammatory bowel disease (IBD). Other models of direct injury to bi…

Hepatic sickling crisis mimicking recurrent cholangitis.

A 22-year-old man with homozygous sickle cell disease presented with recurrent fever, right upper quadrant pain and jaundice. Liver biopsy confirmed the diagnosis of hepatic sickling crisis; the symptoms responded to hydroxyurea therapy. Hepatic vasocclusive crisis can diagnosed on liver biopsy, and need not be a diagnosis of exclusion.

Primary sclerosing cholangitis: MR imaging features.

OBJECTIVE: The purpose of this study was to evaluate the spectrum of MR imaging features of primary sclerosing cholangitis. MATERIALS AND METHODS: A retrospective review was performed of MR imaging findings including MR cholangiography and multiphasic contrast-enhanced dynamic sequences in 22 patients with primary sclerosing cholangitis. MR imaging analysis included abnormalities of intra- and extrahepatic bile ducts, abnormalities of liver parenchyma, changes in liver morphology, and lymphadenopathy. RESULTS: Abnormal findings of bile ducts were seen in all 22 patients; the most common find…

[Intrahepatic cholangitis with presumably immunologic bases].

An overview is given on three topics of chronic cholangitides, namely 1. chronic non destructive cholangitis, i.e. primary biliary cirrhosis, 2. immunocholangitis, 3. primary sclerosing cholangitis including the liver-colitis-syndrome, each of which will lead into primary biliary cirrhosis within a commonly long period. The histological patterns are well defined in chronic non destructive cholangitis and primary sclerosing cholangitis. Clinical features, laboratory data and histologic pattern concerning the immunocholangitis will have to be evaluated in more precise ways in the future. As with…

Mucosal intussusception to avoid ascending cholangitis.

Many methods have been devised to prevent ascending cholangitis following Kasai's hepatic portoenterostomy for biliary atresia. To investigate the effectiveness of mucosal intussusception in preventing ascending cholangitis, 20 mongrel dogs were randomized to undergo Roux-en-Y cholecystjejunostomy and common bile duct ligation with or without mucosal intussusception. Aeromonas hydrophila was instilled into the gastrointestinal tract. Bacterial cultures were grown from samples taken from the gallbladders and lower jejunum, and blood chemistry and barium studies were performed. The barium studie…

Duodenoscopic sphincterotomy for acute suppurative cholangitis.

Fifteen patients (eight males, seven females; age range: 23-76 years) presenting with acute suppurative cholangitis underwent endoscopic retrograde cholangiography and sphincterotomy within 1-10 days of hospitalization. Cholangitis was due to common duct stones in all patients; all but one of them had their gall-bladders in situ. All of them had fever, jaundice, abdominal pain, leucocytosis and deranged liver function while 26.6% were in shock, 13.3% in coma and 40% in azotaemia. Cardiac or other associated diseases caused 21% of the patients to be high risk candidates for surgery. An adequate…

Recurrent cholangitis secondary to oriental cholangiohepatitis.

A Korean woman with a known mass in the left lobe of the liver and a normal gallbladder by ultrasound suffered recurrent cholangitis, which ultimately proved to be Oriental cholangiohepatitis. This disease, endemic to East Asia, has become widespread in the United States as immigration from that area has increased. The symptoms are identical to recurrent cholangitis associated with gallstones, but the natural history and definitive treatment are entirely different. The diagnosis should be suspected in patients with a history of parasitic infestation with Ascaris lumbricoides or Clonorchis sine…

Radiological features of AIDS related cholangitis.

Cryptosporidial infection is one of the recognised causes of diarrhoea in AIDS patients. It may also produce biliary tract disease. Fifteen out of 250 (6%) AIDS patients seen at our hospital had Cryptosporidial enteritis and five of the 15 (2% total) had clinical evidence of biliary tract disease. The radiological findings in these five patients are presented. Ultrasound examination of all five patients showed abnormalities in the biliary tree; five had dilatation and irregularity of the intra- and extrahepatic bile ducts with focal strictures, four had gall-bladder wall thickening, two had th

Chronic lymphocytic cholangitis in three cats.

Wedge biopsy of the liver during episodic clinical illness in three male cats showed lymphocytic cholangitis. Principal clinical findings were increased serum alkaline phosphatase activity and hepatomegaly (two cats) associated with anorexia, pyrexia. and weight loss; these signs of illness were intermittent with asymptomatic periods. The hepatic lesions were characterized by lymphoid aggregate or follicle formation, diffusely dispersed lymphocytes and plasma cells, and abnormal bile ducts and ductules. Lymphoid aggregates and diffusely scattered lymphocytes were seen in the pancreas also. The…
