Inversion of the liver and anomalous positions of the gallbladder are both rare. We report a patient with an anterior suprahepatic midline gallbladder associated with hepatic inversion. This association of these two rare anomalies has not, to our knowledge, been previously reported.

One of the unusual problems associated with gallbladder disease is a polyp discovered by either ultrasonography or by oral cholecystogram. We report a recent experience of two cases of polyps of the gallbladder removed by cholecystectomy. The incidence of gallbladder polyps of any type is quite varied in the medical literature. Eighty percent of polyps occur in females, and these occur after the third decade of life. Emphysematous cholecystitis is an uncommon variant of acute cholecystitis. Pneumobilia with air in the biliary radicles is rarely seen in the disease and has been previously described in only four cases which the author could find in the English literature. Herein, we report a fifth case and a brief review of the literature.

The following protocols should be observed in evaluating a patient with suspected cholecystitis: In evaluating chronic cholecystitis in patients on a diet containing fat, plain films of the abdomen and oral cholecystogram are the procedures of choice. If the gallbladder fails to fill after ingestion of 3 g iopanoic acid, the procedure is repeated with an additional 3 g the following day. Failure of the gallbladder to visualize after a two-day study, equivocal findings, or persistent symptoms despite a normal study should be followed by ultrasound and, if necessary, cholescintigraphy. A case of a four year old girl, first seen at the emergency ward for an abdominal pain, is reported. Exploration revealed a mass at the right hypochondrium which a laparotomy labeled as a dropsy gallbladder with a complete transversal septum at the union of the gallbladder's body and infundibulum. Microscopic examination showed an acute cholecystitis. A classification of septate gallbladders is performed and embryology revised. Microphotographs of human embryo are presented making etiopathogenesis of this case understandable. The scanty bibliography on this topic makes this case extremely rare

A sixth case report of multiseptate gallbladder is presented because of its rarity and because of its association in pregnancy. The anomaly has a diagnostic X-ray appearance. As reported in four previously recorded cases, no inflammation was found in our patient's specimen despite symptoms mimicking cholecystitis. Initial presentation and worsening clinical course during the patient's pregnancy lend credence to the idea that symptoms are caused not by inflammation but by pressure in the chambers, caused by relative delay in emptying. In a 4 year period two hundred and seventy-nine patients were treated for gallstones in a single surgical unit. One hundred and thirteen were treated for stones in the bile duct, of them 49 patients were treated surgically and 64 endoscopically. Twenty-three patients had papillotomy without previous cholecystectomy. There were two deaths, both after surgery following failure of endoscopic treatment. Morbidity was greater in the surgically treated group. It is concluded that a large proportion of patients with bile duct stones can be treated by endoscopic papillotomy.
