Delivery Model Matters for Digital Interventions in Pain Self-Management: Mixed Methods Study.

BACKGROUND: Digital health interventions can improve access to and outreach of evidence-based support for people living with chronic pain. How best to deliver such interventions remains unclear, however, and although guided or blended care models show promise, digital interventions are primarily delivered without support or follow-up. OBJECTIVE: This study aimed to compare the use and effect of the evidence-based digital pain self-management intervention EPIO when (1) delivered through digital download without follow-up (ie, EPIONoFollowUp), compared with (2) delivery through a simple blended

Shared Decision-Making and Collaborative Care Models for Pain Management: A Scoping Review of Existing Evidence.

This article aims to summarize the existing evidence on shared decision-making and collaborative care models for acute and chronic pain management. We searched the PubMed database for articles published between 1980 and 2023 and scanned included articles' references to identify additional sources. Two reviewers independently screened titles and abstracts. Studies met inclusion criteria if they implemented or evaluated shared decision-making or collaborative care interventions in patients with acute or chronic pain. A total of 690 articles were reviewed, with 32 full text articles meeting inclu

Alternative Payment Models and Diabetes Outcomes.

PURPOSE OF REVIEW: Determine whether and how alternative payment models (APMs) impact diabetes care and outcomes. Explain what APMs are and highlight key principles of effective APMs. Provide a conceptual model for how APMs can support transformation to evidence-based population health delivery models that improve diabetes outcomes. RECENT FINDINGS: Advanced APMs with shared savings or global budgets, along with quality incentives, tended to improve diabetes and composite chronic condition outcomes, and can reduce Medicare Part A hospital and Part B outpatient costs. Advanced APMs, particular

Adaption of the Biopsychosocial Model of Chronic Noncancer Pain in Veterans.

POPULATION: Veterans with chronic noncancer pain (CNCP) are a vulnerable population whose care remains a challenge for clinicians, policy-makers, and researchers. As a result of military experience, veterans are exposed to high rates of musculoskeletal injuries, trauma, psychological stressors (e.g., post-traumatic stress disorder, depression, anxiety, substance abuse), and social factors (e.g., homelessness, social isolation, disability, decreased access to medical care) that contribute to the magnitude and impact of CNCP. In the veteran population, sound theoretical models are needed to unde

Factors mediating the impacts of child abuse and intimate partner violence on chronic pain: a cross-sectional study.

BACKGROUND: Most research on the health impacts of intimate partner violence (IPV) and child abuse has been conducted in Western countries and may not be generalizable to women living in different contexts, such as Saudi Arabia. Chronic pain, a disabling health issue associated with experiences of both child abuse and IPV among women, negatively impacts women's well-being, quality of life, and level of functioning. Yet, the psychosocial mechanisms that explain how abuse relates to chronic pain are poorly understood. We developed and tested a theoretical model that explains how both IPV and chi

Designing a provider-led multimorbidity care model for fragmented insurance-based health systems: A mixed-methods study in Colombia.

Multimorbidity the coexistence of two or more chronic conditions is a growing challenge for health systems in low- and middle-income countries (LMICs) structured around single-disease paradigms. In Colombia, insurer-provider fragmentation within the General System of Social Security in Health (SGSSS) compounds this challenge, generating discontinuities in care that disproportionately affect people living with multiple chronic conditions. Despite a national epidemiological characterisation, a World Bank-supported management proposal, and a formal pilot implementation, limited published evidence

Holistic management of symptom burden in fibrosing interstitial lung diseases (F-ILDs).

BACKGROUND: Disease modifying therapies for progressive fibrotic interstitial lung diseases (F-ILDs) slow physiological decline but have not consistently shown to improve patient-reported symptoms or health-related quality of life. Patients with F-ILD experience a substantial symptom burden that necessitates comprehensive supportive care alongside antifibrotic treatment. This review summarises the current evidence for management strategies for F-ILD to be considered in conjunction with disease-modifying therapies. SYMPTOM MANAGEMENT: Dyspnoea, chronic cough, fatigue, anxiety and depression ar

Understanding and treating panic disorder in the primary care setting.

According to studies, the median prevalence of panic disorder in the primary care setting is 4%. Rates are higher among certain patient populations, such as those with cardiac (20% to 50%) or gastrointestinal presentations (28% to 40%). Consequently, patients with panic disorder are high utilizers of medical services and are heavily represented among patients classified as high health care utilizers, compared with other psychiatric or non-psychiatric groups. Despite its frequency in the primary care setting, panic disorder is significantly under-recognized by medical providers. Corresponding w

Development of a Cystic Fibrosis Primary Palliative Care Intervention: Qualitative Analysis of Patient and Family Caregiver Preferences.

To prevent or mitigate chronic illness burden, people with cystic fibrosis (pwCF) and their family caregivers need primary (generalist-level) palliative care from the time of diagnosis forward. We used qualitative methods to explore their preferences about a screening-and-triage model ("Improving Life with CF") developed to standardize this care. We purposively sampled and interviewed 14 pwCF and caregivers from 5 Improving Life with CF study sites. Thematic analysis was guided by a priori codes using the National Consensus Project's Guidelines for Quality Palliative Care. Participants include

Establishment of an innovative specialist cardiac indigenous outreach service in rural and remote Queensland.

Cardiovascular diseases are the leading cause of mortality in Indigenous Australians. Indigneous Australians present at a younger age and have a greater incidence of cardiac risk including smoking and diabetes than non-Indigenous Australians. Access to specialist health services is an important determinant of health care outcomes for these patients. We describe an innovative and successful for model for providing Outreach Cardiac Specialist services to Indigenous communities in rural and remote locations. The approach involves a step-wise process of a) community engagement, b) delivering recov

Machine learning prediction of premature death from multimorbidity among people with inflammatory bowel disease: a population-based retrospective cohort study.

BACKGROUND: Multimorbidity, the co-occurrence of 2 or more chronic conditions, is important in patients with inflammatory bowel disease (IBD) given its association with complex care plans, poor health outcomes, and excess mortality. Our objectives were to describe premature death (age < 75 yr) among people with IBD and to identify patterns between multimorbidity and premature death among decedents with IBD. METHODS: Using the administrative health data of people with IBD who died between 2010 and 2020 in Ontario, Canada, we conducted a population-based, retrospective cohort study. We describe

Vitamin D deficiency in the intensive care unit: an invisible accomplice to morbidity and mortality?

The association between vitamin D deficiency and chronic illness is well-known. Vitamin D deficiency has been associated with increased mortality in the general population. Despite this knowledge, vitamin D insufficiency is seldom considered and rarely replaced adequately, if at all, in critically ill patients in intensive care. We present a hypothetic model demonstrating how vitamin D deficiency may be an unrecognized contributor to adverse outcome in intensive care patients.

Cost savings from primary care clinic continuity among patients with chronic disease in Alberta, Canada: a retrospective population-level cross-sectional study.

BACKGROUND: Continuity of care with a primary care physician (PCP) is well known to improve patient health, result in less health service utilisation (HSU), and lower costs. Less is known about the impact of clinic continuity related to team-based care and shared practice models. Information on the cost impact of such models is needed to guide design and payment policies. AIM: To estimate savings in HSU costs for patients with chronic disease when they are moved from primary care clinics with low and moderate continuity levels to those with high levels. DESIGN AND SETTING: A 3-year retrospec
