BACKGROUND: Choice of sedation of critically ill patients is a core element of intensive care practice. The alpha 2 agonists for sedation to produce better outcomes from critical illness (A2B) trial tested the effectiveness of two alpha agonist sedatives versus propofol in reducing time on mechanical ventilation in 38 intensive care units in the United Kingdom. To evaluate both how this complex trial was implemented and how this may have influenced trial outcomes, an understanding of the contextual and practice variation across multiple sites was required.

Clinical findings in many nail disorders are not usually pathognomonic. An accurate diagnosis therefore relies on inspection of the nail unit from different angles. We review clinical features of different nail disorders that can be observed during frontal examination of the distal edge of the nail plate and the hyponychium and correlate these with features observed when the nail is viewed from above. Frontal examination of the distal nail unit can help establish a clinical diagnosis in routine practice.

BACKGROUND: Japanese cedar (JC) pollen sublingual immunotherapy (SLIT)-tablets (5000 Japanese allergy units [JAU]) are licensed for the treatment of JC-pollinosis with no age restriction on the basis of the results of a 5-year clinical trial. However, there have been no large-scale surveys of 5000 JAU in an actual clinical setting.
METHODS: This was a multicenter observational prospective study. We assessed the safety and effectiveness of the long-term use of 5000 JAU in patients with JC-pollinosis, with an observation period of two seasons of JC pollen dispersal, at clinical sites in Japan.

BACKGROUND/AIMS: Appendectomy for suspected appendicitis cases is a common procedure. Its clinical diagnosis needs to be supported by accurate confirmatory tests. No single paraclinical test with a high degree of sensitivity and specificity is available for its diagnosis. The appendix contains numerous serotonin-producing cells (enterochromaffin cells). In the inflammatory process and subsequent cell injury, serotonin is released and converted to 5-HIAA (5-hydroxy indole acetic acid).
