Enhancing Cognitive Impairment Assessment and Management in Hip Fracture Patients: A Two-Cycle Clinical Audit.

Cognitive impairment significantly affects elderly hip fracture patients, dramatically worsening their outcomes, yet healthcare teams consistently fail to screen for it. Despite national guidelines mandating routine cognitive assessment, compliance remains alarmingly low. This audit confronted this critical gap between evidence and practice in fragility hip fracture care.  Methods: We conducted a prospective two-cycle audit at Northwick Park Hospital in Harrow, England, to transform cognitive assessment practices. Cycle 1 (July-November 2023) exposed baseline failures in 30 patients aged ≥65 w

Memory-Enhancing and Anxiolytic Effects of the Rose of Jericho on Sleep Deprivation-Related Cognitive and Behavioral Changes.

BACKGROUND: Sleep is a crucial physiological phenomenon that enables the body to engage in restoration and rejuvenation. Remarkably, even limited periods of sleep deprivation (SD) can adversely affect cognitive functions such as memory retention, emotional regulation, data processing, and concentration. The Rose of Jericho (RoJ) has been considered more than a plant and has demonstrated potential therapeutic actions in childbirth, respiratory diseases, gastrointestinal disorders, and cancer. The effect of the RoJ on memory, cognition, and behavior has not yet been well-studied. OBJECTIVES: Th

Abscisic Acid Enhances Motor and Cognitive Function in the 3-Acetylpyridine Mouse Model of Cerebellar Ataxia.

Cerebellar ataxia is a debilitating neurodegenerative disorder characterized by impaired motor coordination and balance with limited treatment options. Abscisic acid (ABA), a phytohormone detected in mammalian brains, has shown neuroprotective properties. This study investigated the effects of ABA on motor, cognitive, and affective deficits in a mouse model of cerebellar ataxia in which male Swiss mice received a single intraperitoneal injection of 3-acetylpyridine (3-AP; 60 mg/kg), which leads to the loss of climbing fiber input to Purkinje neurons leading to cerebellar degeneration. In ABA-t

Prefrontal parvalbumin neurons as a target for enhancing cognition in non-pathological and 22q11.2 microdeletion syndrome mice.

To achieve goals in a dynamic environment, the prefrontal cortex (PFC) facilitates various cognitive functions that allow an organism to detect and discriminate information, identify changes in context, and apply this information to representations of known rules and internal states. A failure of organized communication in the PFC is thought to contribute to the emergence of cognitive impairments in psychiatric diseases, with attentional deficits occurring as a fundamental symptom across various conditions. The 22q11.2 microdeletion syndrome is a rare genetic condition that confers a high risk

OPA1 Enhances Microglial Amyloid-β Clearance and Alleviates Cognitive Impairments in an Alzheimer's Disease Model.

Amyloid deposition is thought to be a pathologic hallmark of Alzheimer disease (AD), which is associated with cognitive decline. Microglia play a crucial role in the pathology of AD, especially in the clearance of Aβ. Optic atrophy 1 (OPA1) is a GTPase primarily on the inner mitochondrial membrane, related to mitochondrial dynamics and cellular energy metabolism. Here, we found that decreased OPA1 expression and defective mitochondrial morphology in microglia during AD. Next, we utilized an OPA1 activator BGP-15, an OPA1 inhibitor myls22 and an OPA1 overexpression virus to investigate the role

Enhancing cognitive-motor recovery in Rett syndrome: effects of integrated intervention on neuropsychological and motor outcome.

Cognitive-motor integration plays a crucial role in the rehabilitation of individuals with complex disabilities, where dissociated impairments in cognition and movement often hinder global functioning. In this study, we investigated the efficacy of an integrated neurorehabilitation program targeting both neuropsychological and motor domains in patients with Rett Syndrome. Baseline assessments included measures of attention, memory, and temporal sequencing, as well as gross, fine, and graphomotor abilities, evaluated using relevant GAIRS subscales (Global Assessment and Intervention Rating Scal

Enhancing identification performance of cognitive impairment high-risk based on a semi-supervised learning method.

BACKGROUND: Cognitive assessment plays a pivotal role in the early detection of cognitive impairment, particularly in the prevention and management of cognitive diseases such as Alzheimer's and Lewy body dementia. Large-scale screening relies heavily on cognitive assessment scales as primary tools, with some low sensitivity and others expensive. Despite significant progress in machine learning for cognitive function assessment, its application in this particular screening domain remains underexplored, often requiring labor-intensive expert annotations. AIMS: This paper introduces a semi-super

Secondary outcomes of enhanced cognitive behavioral therapy (eCBT) for children and adolescents with obsessive-compulsive disorder.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a debilitating mental health condition usually presenting with a high degree of comorbid symptoms in the majority of cases. Although face-to-face cognitive-behavioral therapy (CBT) is considered the therapeutic golden standard for pediatric OCD, its accessibility, availability, and consistency in delivery are still limited. To address some of these challenges, an enhanced CBT (eCBT) package was created and introduced. This study explored eCBT's broad-based impact on OCD-related comorbid symptoms, functional impairment, quality of life and fami

Enhancing cognitive control of our decisions: Making the most of humor during the IGT in females and males.

We studied the impact of humor on the Iowa Gambling Task (IGT) decision-making performance and the cognitive control exerted during this task, considering sex as a moderator, and examined whether cognitive control mediated the influence of humor on decision-making. Sixty participants (30 females) performed an extended version of the IGT (500 trials divided into 20 blocks). We randomly assigned them to either an experimental group (Humor Group; Hg; n = 30), where humorous videos were interspersed in the decision-making trials or a control group (Non-Humor Group; NHg; n = 30), where nonhumorous

Quantitative analysis of drugs of abuse and cognitive enhancers in influent wastewater by means of two chromatographic methods.

Sewage-based epidemiology using influent wastewater is used to estimate the consumption trends of (illicit) drugs over a short or long period of time in a subpopulation. The current study aimed to develop two separate methods for the quantitative analysis of selected drugs of abuse (DOA) and cognitive enhancers in influent wastewater using reversed-phase (RP) or hydrophilic interaction liquid chromatography (HILIC) coupled to high-resolution tandem mass spectrometry (LC-HRMS/MS). The performance of RP and HILIC column was evaluated. A simple solid phase extraction was used for sample preparati

The effects of enhancing angiotensin converting enzyme in myelomonocytes on ameliorating Alzheimer's-related disease and preserving cognition.

This review examines the role of angiotensin-converting enzyme (ACE) in the context of Alzheimer's disease (AD) and its potential therapeutic value. ACE is known to degrade the neurotoxic 42-residue long alloform of amyloid β-protein (Aβ42), a peptide strongly associated with AD. Previous studies in mice, demonstrated that targeted overexpression of ACE in CD115+ myelomonocytic cells (ACE10 models) improved their immune responses to effectively reduce viral and bacterial infection, tumor growth, and atherosclerotic plaque. We further demonstrated that introducing ACE10 myelomonocytes (microgli

Home-Based Transcranial Direct Current Stimulation to Enhance Cognition in Stroke: Randomized Controlled Trial.

BACKGROUND: Transcranial direct current stimulation (tDCS) is a promising tool for improving poststroke cognitive function. Home-based rehabilitation is increasingly required for patients with stroke, and additional benefits are expected if supplemented with remotely supervised tDCS (RS-tDCS). We evaluated the cognitive improvement effect and feasibility of RS-tDCS in patients with chronic stroke. METHODS: Twenty-six patients with chronic stroke and cognitive impairment (Korean version of the Montreal Cognitive Assessment [K-MoCA] score <26) were randomized into real and sham RS-tDCS groups a

TAT-LBD-Ngn2-improved cognitive functions after global cerebral ischemia by enhancing neurogenesis.

BACKGROUND: Stroke is the major cause of adult neurocognitive disorders (NCDs), and presents a significant burden on both of the families and society. To improve the cerebral injury, we generated a blood-brain barrier penetrating peptide TAT-LBD-Ngn2, in which Ngn2 (Neurogenin2) is a classical preneural gene that enhances neurogenesis, and neural precursor cells survival and differentiation. We previously demonstrated that it has a short-term protective effect against cerebral ischemia-reperfusion injury. However, it is uncertain if TAT-LBD-Ngn2 could promote neurogenesis to exhibit long-term

Acceptability and feasibility of enhanced cognitive behavioral therapy (eCBT) for children and adolescents with obsessive-compulsive disorder.

INTRODUCTION: Obsessive-compulsive disorder (OCD) is a disabling mental health disorder affecting 1-3% of children and adolescents. Cognitive behavioral therapy (CBT) is recommended as the first-line treatment, but is limited by accessibility, availability, and, in some cases, response to treatment. Enhancement with Internet technologies may mitigate these challenges. METHODS: We developed an enhanced CBT (eCBT) treatment package for children and adolescents with OCD to improve treatment effect as well as user-friendliness. This study aims to explore the feasibility, acceptability, and prelim

Enhanced diagnostic accuracy for neurocognitive disorders: a revised cut-off approach for the Montreal Cognitive Assessment.

BACKGROUND: The Montreal Cognitive Assessment (MoCA) has good sensitivity for mild cognitive impairment, but specificity is low when the original cut-off (25/26) is used. We aim to revise the cut-off on the German MoCA for its use in clinical routine. METHODS: Data were analyzed from 496 Memory Clinic outpatients (447 individuals with a neurocognitive disorder; 49 with cognitive normal findings) and from 283 normal controls. Cut-offs were identified based on (a) Youden's index and (b) the 10th percentile of the control group. RESULTS: A cut-off of 23/24 on the MoCA had better correct classif
