OBJECTIVE: The main aims of the study were to determine the frequency with which two-week wait (2ww) referrals for colorectal cancer (CRC) could proceed directly to straight to test (STT), and the potential improvement in time to diagnosis.
METHOD: A telephone interview was attempted in all 2ww referrals not requiring an advocate and under 80 years. Data were assessed according to a test protocol, and where indicated a potential slot for the appropriate investigation was recorded (virtual test). All patients proceeded to clinic, following which differences in time from GP referral to virtual

OBJECTIVE: The objective of this study is to assess the sigmoid distensibility during CT colonography (CTC) in patients with diverticular disease.
METHODS: Consecutive patients without a history of pelvic radiation or neoplasms underwent 150 CTC. Three radiologists in consensus evaluated axial images for colonic distention, luminal diameters (mm), diverticula, and muscular thickening.
RESULTS: The minimum colon diameter in patients with muscular thickening was significantly smaller, irrespective of the presence of diverticula (P=.009).
CONCLUSION: Muscular thickening with diverticular disea

Virtual colonoscopy involves a helical CT or MR scan of the abdomen and pelvis to detect colorectal polyps and cancer. Both modalities have shown promising sensitivity in revealing larger polyps, in comparison with colonoscopy. Caution should be exercised in its clinical implementation due to significant interobserver variation and individual learning curves. A Danish study indicates that CT colonography (CTC) can be performed cost-effectively compared to colonoscopy. CTC is recommended in preference to double-contrast barium enema after incomplete colonoscopy.

BACKGROUND & AIMS: In the Dutch colorectal cancer (CRC) screening program, fecal immunochemical test (FIT)-positives are offered computed tomography colonography (CTC) instead of colonoscopy in case of comorbidities or patient refusal. Literature on CTC in FIT-positives is scarce, and the risk of post-CT interval CRC (PCT-CRC) in this population is unknown.
METHODS: In this cohort study, we assessed CTC yield, clinical management, and risk of PCT-CRC in FIT-positives who underwent CTC between 2014 and 2019 in the Dutch CRC screening program. Data were linked with the National Cancer Registry

PURPOSE: To compare MiraLAX, a hypo-osmotic lavage, and magnesium citrate (MgC), a hyper-osmotic agent for bowel preparation at CTC.
METHODS: 398 total screening CTC studies were included in this retrospective, single institution study. 297 underwent preparation with a double-dose MgC regimen (mean age, 61 ± 5.5 years; 142 male/155 female) and 101 with 8.3 oz (equivalent to 238 g PEG) of MiraLAX (mean age, 60 ± 9.6 years; 45 male/56 female). Oral contrast for tagging purposes was utilized in both regimens. Studies were retrospectively analyzed for residual fluid volume and attenuation by auto

OBJECTIVE. Serrated polyps include hyperplastic polyps, sessile serrated polyps, and traditional serrated adenomas (TSAs). Hyperplastic polyps and sessile serrated polyps account for approximately 99% of all serrated lesions; TSAs are rare. However, both sessile serrated polyps and TSAs are now recognized as precursor lesions to carcinogenesis, representing approximately one-fourth of all sporadic colorectal cancers. We report what is, to our knowledge, the first series describing the characteristics of CTAs on CT colonography (CTC). MATERIALS AND METHODS. An international, multicenter, retros

PURPOSE: CT colonography (CTC) is a widely accepted examination tool for detection of colorectal lesions but evidence of the proportions of relevant extracolonic findings (ECF) in a large symptomatic but still relatively low-risk cohort is lacking, as well as their relationship to symptoms, age, and sex.
METHODS: All patients (n = 3208) with colorectal symptoms, imaged between January 2007 and September 2016 with first-time CTC, were retrospectively selected. The majority (96.7%) was examined with low-dose unenhanced protocol. The most relevant ECF and colorectal lesions (≥6 mm) were prospect

Computed tomographic colonography (CTC) is a minimally invasive imaging examination for the colon, and is safe, well tolerated and accurate for the detection of colorectal cancer (CRC) and advanced adenoma. While the role of CTC as a primary test for population screening of CRC is under investigation, the fecal occult blood test (FOBT) has been recommended for population screening of CRC in Europe. Subjects with positive FOBT are invited to undergo total colonoscopy, which has some critical issues, such as suboptimal compliance, contraindications and the possibility of an incomplete exploratio

AIM: Same day CT colonography (CTC) for incomplete colonoscopy should be considered to avoid the need for further bowel preparation. Oral administration of contrast is essential for optimal colonic analysis of the CTC. Our aim was to determine the optimal time for giving contrast to people who underwent same day CTC after failed colonoscopy.
METHODS: Data was extracted from the Trust database to identify the number of CTC referrals following failed or incomplete colonoscopy within the last 5 years, and the reason for their referral. Patients were then excluded if they had not received faecal

BACKGROUND: Prevention programmes should only be recommended if they achieve what they promise to achieve. Therefore, we checked the variation and validity of recommendations for screening for colorectal cancer of nine organisations.
METHODS: We analysed the information concerning recommended screening age, guaiac faecal occult blood test (gFOBT), faecal immunological test (FIT), faecal DNA test, sigmoidoscopy, colonoscopy, double-contrast examination/double-contrast barium enema, and virtual colonoscopy/CT colonography in the following three steps: 1) we gathered the references quoted by the

OBJECTIVE: Prior research indicates CT colonography (CTC) would be a cost-effective colorectal cancer (CRC) screening test if widespread availability were to increase overall CRC screening adherence rates. The primary aims of this multicenter study were to evaluate patient experience and satisfaction with CTC screening and compare preference against screening colonoscopy.
MATERIALS AND METHODS: A 12-question survey instrument measuring pretest choice, experience, and satisfaction was given to a consecutive cohort of adults undergoing CTC screening in three disparate screening settings: univer

OBJECTIVES: Evidence has shown that computerized tomographic colonography (CTC) and optical colonoscopy (OC) can detect advanced adenomas at an equal rate; however, a comparison of the subcentimeter adenoma detection has not been performed. The objective of this study is to compare CTC and OC screening programs, with a focus on the detection and recovery of subcentimeter adenomas.
METHODS: In all, 1,700 screening OC examinations in average-risk patients were compared with 1,307 CTC examinations in similar patients drawn from the same referral pool completed in 2006-2008. The detection rate fo

This paper aims to quantify changes in colonic length and positional change between supine and prone CT colonography (CTC) studies in order to aid development of image registration techniques. CTC studies in 20 patients (10 men and 10 women) with technically adequate distension were analysed using an image analysis workstation. Spatial co-ordinates of colonic landmarks were determined in both prone and supine orientations using a three-dimensional colon model view and centreline positions. Change in the co-ordinate position of colonic segments between supine and prone scans was calculated usin

A 74-year-old female had repeated relapses of right lower abdominal pain; a colonoscopic examination showed a submucosal tumor of the appendix. Under provisional diagnosis of appendicitis caused by a submucosal tumor, a laparoscopic operation was performed. The laparoscopic observation did not reveal the typical finding of appendicitis. The tumor was thought to be a malignant neoplasm. The ileocecal region was excised with lymph node dissection. Macroscopically, the mass was localized in the appendix and did not infiltrate to the cecum. Histologically, the marginal zone consisted of lymphoid c

We report a case of colonic perforation in CT colonography, which was observed in a sigmoid colon segment contained within an inguinal hernia. At surgery, apart from the perforation, a normal large-bowel wall was found. Although rare, perforation may occur in patients with normal bowel wall, possibly resulting from a mechanical strain caused by gaseous overdistention. Radiologists performing the procedure must be aware of this possibility.

AIM: To assess whether computed tomography colonography (CTC) is a viable alternative to colonoscopy or double contrast barium enema in the follow-up of patients after diverticulitis.
MATERIAL AND METHODS: Fifty patients underwent CTC followed immediately by colonoscopy. Results were blinded to the examiners. Findings of diverticular disease and patient acceptance were evaluated.
RESULTS: Bowel preparation and distension were good in the majority of CTC and colonoscopy examinations. Diverticular disease was found in 96% of patients at CTC and in 90% at colonoscopy. The rate of agreement betw

When used appropriately, screening for colorectal cancer (CRC) can reduce disease-related morbidity and mortality. Current methods include fecal occult blood testing (FOBT), flexible sigmoidoscopy [FS], barium enema, and colonoscopy; all are cost-effective techniques. Unfortunately, offering an array of options has not increased screening utilization, which continues to lag behind that of other common cancers. Newer techniques, particularly virtual colonoscopy (VC) and stool DNA testing, may offer attractive alternatives for healthcare provider recommendation and patient use.
