Non-Randomized Pre-Post Testing to Evaluate Knowledge of Community Health Workers on Chronic Diseases After Asynchronous Online Training.

BACKGROUND: Community Health Worker (CHW) training started to be conducted mostly virtually during the 2020 COVID-19 pandemic when there was increased need for CHWs and a restriction on in-person gathering. Synchronous virtual training has become the norm for initial training with a need for continuous training to further improve capabilities found in the literature. In this paper we explore conducting online continuing education on chronic diseases asynchronously for CHWs. METHODS: CHWs in Maryland were sent information on availability of online training for chronic diseases through the netw

Contribution of community health workers to the treatment of common illnesses among under 5-year-olds in rural Uganda.

BACKGROUND: The control of malaria, pneumonia, and diarrhoea is important for the reduction in morbidity and mortality among children under 5 years. Uganda has adopted the Integrated Community Case Management strategy using Community Health Workers (CHWs) to address this challenge. The extent and trend of these three conditions managed by the CHWs are not well documented. This study was done to describe the epidemiology and trends of the three common illnesses treated by the CHWs in Bugoye Sub-County in rural Uganda. METHODS: A retrospective review of monthly morbidity data for children less …

Successful elimination of falciparum malaria following the introduction of community-based health workers in Eastern Myanmar: A retrospective analysis.

BACKGROUND: Myanmar has a large majority of all malaria in the Greater Mekong Subregion. In the past decade, substantial progress was made in malaria control. The residual burden of malaria is in remote areas where currently recommended malaria elimination approaches are generally not feasible. In such hard-to-reach communities in Mon state, East Myanmar, Medical Action Myanmar introduced community health workers (CHWs) to deliver early diagnosis and treatment for malaria. We conducted a retrospective analysis to assess the impact of this intervention. METHODS AND FINDINGS: This retrospective…

Pharmacists and community health workers improve medication-related process outcomes among Cambodian Americans with depression and risk for diabetes.

BACKGROUND: Cambodian Americans have high rates of cardiometabolic and psychiatric disorders and disadvantaged social determinants of health (SDOH). These factors can make it challenging to resolve drug therapy problems (DTPs) and improve medication-related outcomes. This manuscript reports planned analyses from a randomized controlled trial in which participants were randomized to one of 3 treatment arms: (1) community health worker (CHW)-delivered lifestyle intervention called Eat, Walk, sleep (EWS), (2) EWS plus pharmacist/CHW-delivered medication therapy management (EWS + MTM), or (3) soci

Community-directed health (CDH) workers enhance the performance and sustainability of CDH programmes: experience from ivermectin distribution in Uganda.

The performance and 'drop-out' rates of ivermectin (Mectizan) distributors in the Ugandan programme for community-directed treatment with ivermectin (CDTI) were investigated and related to the manner in which the distributors were recruited. Distributors, from randomly selected communities endemic for onchocerciasis in seven of the 10 affected districts, were interviewed. Questionnaires were initially completed for 296 communities (in which ivermectin had been distributed in 1998 but not in 1999) and then extended to another 310 communities (in which ivermectin had been distributed in both stu

Psychological health and motivational factors of health workers and community volunteers in preventive chemotherapy programs for neglected tropical diseases control in Ekiti State, Nigeria.

BACKGROUND: Mass Drug Administration (MDA) remains a key tool for elimination of Neglected Tropical Diseases (NTDs) and relies on the effort of health workers and community volunteers. Little is known about the mental health and motivation of these health workers to support better programming. This study assesses the psychological well-being of these implementers at two time periods (before and during the 2019 COVID-19 pandemic) to inform sustainable program strategies. METHODS: We conducted a cross-sectional study in three pre-selected communities with a history of poor MDA performance in Ek

Effect of a Community Health Worker-Led Intervention on Physical Activity in Adults with Type 2 Diabetes in Primary Health Care in the Brazilian Amazon.

Type 2 diabetes mellitus (T2DM) is a major global public-health problem, and physical inactivity contributes to poor disease control. In settings with limited access to health services, as in the Brazilian Amazon, interventions delivered by Community Health Workers (CHWs) within Primary Health Care (PHC) may offer a pragmatic strategy to increase physical activity (PA). We aimed to evaluate the effect of a CHW-led, theory-based intervention on PA among adults with T2DM in PHC in a cluster-randomized, community-based trial. A total of 274 participants were enrolled (intervention: n = 140, contr…

Using community health workers to refer pregnant women and young children to health care facilities in rural West Bengal, India: A prospective cohort study.

BACKGROUND: Community health workers (CHWs) have been placed in many rural areas in India to increase villagers' connections to basic preventive health care. In this study, we describe how pregnant women and mothers of young children react when CHWs inform them that they, or their child, are at high risk of pregnancy-related complications or early childhood developmental delays, and further screening and health care from a physician is recommended. METHODS: In this longitudinal study in rural villages in West Bengal, India, pregnant mothers, as well as mothers of children aged 12-24 months, w…

Impact of Community Support Workers in Rural Ethiopia on Emotional and Psychosocial Health of Persons Living with HIV: Results of a Three-Year Randomized Community Trial.

People living with HIV face multiple psychosocial challenges. In a large, predominantly rural Ethiopian region, 1799 HIV patients new to care were enrolled from 32 sites in a cluster randomized trial using trained community support workers with HIV to provide individual health education, counseling and social support. Participants received annual surveys through 36 months using items drawn from the Centre for Epidemiologic Studies Depression Scale-10, Medical Outcome Study Social Support Survey, and HIV/AIDS Stigma Instrument-PLWA. At 12 months (using linear mixed effects regression models con

Effects of community health worker-led interventions on cardiometabolic outcomes in vulnerable populations: A systematic review and meta-analysis.

UNLABELLED: Cardiometabolic diseases (CMDs) disproportionately affect vulnerable populations worldwide. Community health worker (CHW)-led interventions may reduce risk and improve preventive care, but their effectiveness remains unclear. This systematic review and meta-analysis evaluated their effectiveness on cardiometabolic outcomes in vulnerable groups. A comprehensive search of PubMed/MEDLINE, Scopus, Embase, ClinicalTrials.gov, and Cochrane Library was performed from January 1, 2000 to December 20, 2025, for randomized trials of CHW-led interventions on cardiometabolic outcomes. Community Health Worker Support for Hispanic and Latino Individuals Receiving Hemodialysis: The Navigate-Kidney Randomized Clinical Trial.

IMPORTANCE: Hispanic and Latino individuals with hemodialysis-dependent kidney failure experience social barriers that may increase their risk for volume-related complications, including fluid accumulation between dialysis sessions or interdialytic weight gain (IDWG). Higher IDWG contributes to cardiovascular complications, hospitalizations, and poor quality of life. Minimizing IDWG is prioritized by patients, clinicians, and payers; yet, high IDWG remains common despite dietary education. OBJECTIVE: To determine if community health worker (CHW) support reduces IDWG among Hispanic and Latino

IMPACT: A Structured Human-Centered Design Process for Integrating Community Health Worker Insights Into Program Policy in Pakistan.

BACKGROUND: Community health workers (CHWs), most of whom are women, are the bedrock of primary health care provision in much of the world. CHWs are often employed at the bottom of health hierarchies where they have little voice; in state programs and vertical initiatives, accountability has generally flowed downwards. Yet many programs might function better if CHWs' needs and ideas were considered in program design. METHODS: From 2020-2022, we implemented a human-centered design process called IMPACT with CHWs working on polio vaccination in one district of Pakistan. The name IMPACT draws fr…
