With the advent of psychotropic medications and with the deinstitutionalization of psychiatry starting in 1968, patients were prematurely discharged from forensic state hospitals. Due to lack of resources, psychiatric forensic patients ended up in the correctional system or homeless with the reduction of psychiatric beds in forensic and civil state hospitals. Lacking proper training and medication management, the recidivism rate of this population was close to 10% for rearrest and about 35% for revocation of conditional release (CR; Manguno-Mire et al., ). A new treatment modality was created

The burden of severe and persistent mental illness is high. Beside somatic treatment and psychotherapeutic interventions, treatment options for patients with severe mental illness also include psychosocial interventions. This paper summarizes the results of a number of systematic literature searches on psychosocial interventions for people with severe mental illness. Based on this evidence appraisal, recommendations for the treatment of people with severe mental illness were formulated and published in the evidence-based guideline series of the German Society for Psychiatry, Psychotherapy and

The mental health system struggles to engage people with severe mental illnesses (SMI) and substance use disorders (SUD) in care. Certified community behavioral health clinics (CCBHCs) are designed to meet service gaps by providing person-centered, comprehensive, coordinated, accessible, and quality care. This study examines the impact of the CCBHC model on access and engagement of service users utilizing administrative data. There was a significant increase in existing and new service users (χ2 = 5.91; p < .05) after the CCBHC implementation and among existing service users with SMI (χ2 = 43.

This case report describes the clinical course of an adult man followed in forensic and community psychiatry, presenting a complex history of a psychotic disorder, developmental trauma, severe polysubstance use, and prolonged social marginalization. The originality of this case lies in the progressive emergence of atypical obsessive-compulsive phenomena during phases of structured psychosocial reintegration. These phenomena intensify in contexts of multiple relational and institutional demands and attenuate when the patient withdraws from certain commitments or imposed frameworks. Predominant …

The global burden of mental and substance use disorders is rising at an unprecedented rate worldwide, driven by a complex and evolving network of social determinants. Mental health policies are predominantly rooted in biomedical frameworks and interventions, partially considering the crucial social, economic, and political factors that fundamentally shape mental health outcomes. Thus, policy responses often emphasize individualized, symptom-focused treatments, primarily psychopharmacological, while overlooking structural inequities. A paradigm shift is imperative: mental health policies must i

OBJECTIVE: Hoarding behavior is not limited to obsessive-compulsive disorder (OCD) alone. With the objective of highlighting the association between hoarding behavior and common psychiatric disorders, we explored its occurrence in psychiatric inpatients and co-occurrence with Axis I diagnoses.
METHODS: In this prospective cohort study, we enrolled adult psychiatric inpatients by simple random sampling at an urban, community-based, academic medical center. Patients were screened for hoarding using the validated Hoarding Rating Scale-Interview, and those meeting criteria for clinically signific

The objective of this review was to identify the available support systems for family carers of head-injured people within the community. The following databases were searched: MEDLINE, EMBASE PSYCHIATRY, CINAHL and PSYCHLIT (years 1987-1999). Only seven papers were identified. Programmes varied from behavioural to cognitive interventions to help carers adjust to particular problem behaviours in their injured relatives. All programmes were professionally led. Most of the data presented was on primary caregivers related to the head-injured person. Only one study used standard assessment tools.

INTRODUCTION: Many medical disciplines, such as emergency medicine, trauma surgery, dermatology, psychiatry, family practice, and dentistry have documented attempts at assessing the level of bioterrorism preparedness in their communities. Currently, there is neither such an assessment nor an existing review of potential bioterrorism agents as they relate to plastic surgery. Therefore, the purpose of this article is to present plastic surgeons with a review of potential bioterrorism agents.
METHODS: A review of the literature on bioterrorism agents and online resources of the Centers for Disea

AIM: Borderline personality disorder (BPD) is a prevalent condition associated with high rates of hospitalizations. The purpose of this manuscript was to detect the factors associated with duration of untreated illness (DUI) in BPD.
METHODS: Through chart review, we identified 152 patients followed up by community psychiatry services in Milan and Monza, Italy. The association between DUI and socioeconomic and clinical variables was examined using Pearson correlation and analyses of variances. The statistically significant variables from univariate analyses were then inserted in regression mod…

Psychiatric disorders show high rates of comorbidity and nonspecificity of presenting clinical symptoms, while demonstrating substantial heterogeneity within diagnostic categories. Notably, many of these psychiatric disorders first manifest in youth. We review progress and next steps in efforts to parse heterogeneity in psychiatric symptoms in youths by identifying abnormalities within neural circuits. To address this fundamental challenge in psychiatry, a number of methods have been proposed. We provide an overview of these methods, broadly organized into dimensional versus categorical approa

Ehlers-Danlos syndrome is a group of connective tissue disorders that most commonly causes joint hypermobility, skin hyperextensibility, and tissue fragility. Patients diagnosed with Ehlers-Danlos syndrome (EDS) frequently suffer from chronic pain and other comorbid conditions. In addition, there is an increased incidence of psychiatry conditions in this patient population, such as anxiety, depression, and personality disorders. We present the case of a 25-year-old female who presented to a community hospital with frequent thoughts of suicide and major depression. She was previously diagnosed

The prevalence and nature of panic symptoms in the elderly are poorly understood. In a replication and extension of Deer and Calamari [J. Behav. Ther. Exp. Psychiatry 29 (1998) 303], the current study sought to determine which symptoms of anxiety best differentiated elderly endorsers of panic from elderly who do not panic, and to compare these results to a reference younger sample. Based on a sample of 302 community-dwelling older adults and a comparison group of 275 younger adults, non-clinical panic (NCP) was endorsed in 26.2% of older adults, which is less frequent than the younger adults (

Early work in psychiatry research, often involving single sites, small samples, and limited variables, has shifted to contemporary research involving multiple sites, large samples, and many variables. Such research raises important questions, including concerns about data quality and methodological rigor, uncertainty about its key lessons, issues regarding clinical relevance, and questions about how to optimize future advances. Here we consider these questions and concerns against the context of big data work on community and register-based surveys, cohort and biobank studies, electronic healt

BACKGROUND AND OBJECTIVE: Mental health disorders affect over 20% of the elderly, with depression being one of the most prevalent conditions. Loneliness plays a significant role in contributing to depression, leading to negative effects on physical, cognitive, and emotional well-being. It also reduces overall quality of life and increases the risk of mortality. Despite its impact, research on gero-psychiatry remains limited, often focusing on clinical settings rather than community-based studies. This study aimed to assess the prevalence of loneliness among older adults and explore the associa

BACKGROUND: Emotion regulation difficulties have been identified as an underlying mechanism in the development and maintenance of psychopathology. The need to improve our understanding of emotion regulation difficulties to accurately assess and treat adolescents in child and adolescent psychiatric settings is essential.
METHOD: In the first part of the study, the psychometric qualities of the Difficulties in Emotion Regulation Scale, 16-item version (DERS-16) were examined in a clinical child and adolescent psychiatric (CAP) sample. In the second part, the DERS-16 was used to examine emotion
