Exposure to ionizing radiation and risk of cutaneous malignant melanoma. Search for error and bias.

In a 1984 case-control study of cutaneous malignant melanoma (CMM) among workers at the Lawrence Livermore National Laboratory, Austin and Reynolds found an association between reported employment in proximity to ionizing radiation and CMM (odds ratio (OR) = 5.4; 95% confidence interval (CI): 1.4, 20.7). But in a preliminary study in 1981, they found no association between individual radiation dosimetry readings and CMM. We reanalyzed the 1984 Austin-Reynolds case-control data to determine whether error or bias explain the inconsistency between reported employment in proximity to radiation and

Subject-domain approach to the study of air pollution effects on schoolchildren's illness absence.

In this paper, the authors propose a new statistical modeling technique, the subject-domain approach, which is theoretically proven to be equivalent to the time-domain approach in detecting an association between exposure and response with time trends. The authors use an empirical data set from a school absence monitoring study conducted during the 1994-1995 school year in Taiwan to demonstrate this subject-domain approach's application to environmental epidemiologic studies. Because the subject-domain models can control the influential personal confounding factors in the models, they show gre

Inverse relationship between central obesity and osteoporosis in osteoporotic drug naive elderly females: The Tianliao Old People (TOP) Study.

To examine the relationship between central obesity and osteoporosis in elderly females in a rural community, a total of 368 ambulatory elderly women were recruited by random sampling during July 2009. Structured questionnaires were completed to survey possible osteoporosis-related risk factors. Subjects were dichotomized into either noncentral obese (waist circumference [WC]<80cm) or central obese subgroups (WC≥80cm) for further analysis. Bone mineral densities were scanned by dual-energy X-ray absorptiometry installed in a mobile bus. Thoracolumbar spine X-ray examination was interpreted by

Assessing the risk of stroke from neck manipulation: a systematic review.

BACKGROUND: Strokes, typically involving vertebral artery dissection, can follow cervical spinal manipulative therapy, and these types of stroke occur rarely. There is disagreement about whether a strong association between neck manipulation and stroke exists. An earlier systematic review found two relevant studies of association that used controls, which also discussed the limitations of the two papers. Our systematic review updates the earlier review, and aims to determine whether conclusive evidence of a strong association exists. METHODS: PRISMA guidelines for systematic reviews were foll

Short sleep duration and increased risk of hypertension: a primary care medicine investigation.

OBJECTIVES: Compelling evidence from laboratory-based and population-based studies link sleep loss to negative cardiovascular health outcomes. However, little is known about the association between sleep duration and hypertension in primary care health settings, independently of other well controlled clinical and biochemical characteristics. We investigated the association between sleep duration and the prevalence of hypertension adjusting for 21 potential confounding factors in a noncontrolled primary care sample. METHODS: The sample included 1046 French adults older than 40 years (mean age,

The birth weight "paradox" uncovered?

Low birth weight (LBW) infants have lower infant mortality in groups in which LBW is most frequent. For example, in 1991, US infants born to smokers had higher risks of both LBW and infant mortality than infants born to nonsmokers. However, among LBW infants, infant mortality was lower for infants born to smokers (relative rate = 0.79). There are competing theories regarding this so-called "paradox." One is that maternal smoking is beneficial for LBW infants. The authors use causal diagrams to show that, even in the absence of any beneficial effect of smoking, an inverse association due to str

Increased total mortality and cancer mortality in men with Dupuytren's disease: a 15-year follow-up study.

The aim of the present study was to evaluate the mortality rate and causes of death of individuals with Dupuytren's disease. In 1981/82, as part of The Reykjavík Study, a general health survey, 1297 males were examined for clinical signs of Dupuytren's disease. Based on the clinical evaluation the participants were classified into three groups: (1) those with no signs of Dupuytren's disease were referred to as the reference cohort; (2) those with palpable nodules in the palmar fascia were classified as having stage 1; and (3) those who had contracted fingers or had been operated on due to cont

Risk of lung cancer from environmental exposures to tobacco smoke.

Epidemiologic evidence on the relation between environmental tobacco smoke and cancer is reviewed. The labeling of tobacco smoke as an environmental cause of lung cancer has been challenged based on allegations of bias in the epidemiologic data. However, tobacco smoke has been shown to increase the risk of lung cancer down to the lowest exposure levels. Environmental tobacco smoke contains the same carcinogenic compounds as those found in the tobacco smoke inhaled directly by the smoker. Nonsmokers environmentally exposed have elevated levels of tobacco smoke byproducts in biological samples.

Effects of ambient particulate matter and ozone on daily mortality in Rotterdam, The Netherlands.

The association between daily variations in all-cause mortality from 1983-1991 in Rotterdam, the Netherlands, and ambient air pollution was investigated. Twenty-four-hour average concentrations of total suspended particulates, Black Smoke, ozone, sulfur dioxide, and carbon monoxide were available on a daily basis. Every other day, total iron content in total suspended particulates samples was available. Poisson regression analysis was used to study associations between air pollution and mortality; generalized additive models were used to adjust for confounders (e.g., seasonal trends, weather).

Recurrent respiratory distress syndrome in successive preterm pregnancies.

BACKGROUND: Earlier studies suggesting an increased recurrence risk of respiratory distress syndrome (RDS) among the subsequent infants of women with a previously affected infant were based on low birth weight inclusion criteria that did not differentiate between preterm and growth-retarded infants. METHODS: We therefore carried out two cohort studies of women who delivered two singleton preterm (gestational age < 37 completed weeks) infants: 1978 to 1989 at the Royal Victoria Hospital (RVH) in Montreal and 1959 to 1966 in the United States Collaborative Perinatal Project (CPP). We compared t

Smoking and risk of Graves' disease.

OBJECTIVE: To assess if smoking is associated with Graves' disease and, if so, to ascertain whether this association persists when controlling for confounding factors. DESIGN: Consecutive entry case-control study with two age- and sex-matched control subjects from two different populations per case patient. SETTING: University hospital. PATIENTS: Five groups were studied: (1) Graves' ophthalmopathy and Graves' hyperthyroidism (n = 100; divided in four subgroups according to the severity of the eye disease); (2) Graves' hyperthyroidism without clinical eye involvement (n = 100); (3) sporadic

How epidemiology has contributed to a better understanding of skin disease.

Epidemiology literally means "the study of what is upon the people." It puts the individual's condition in a population context and is the path to disease prevention. In the first part of this review, important aspects of epidemiology are discussed. Fundamentals of epidemiologic research include the measurement of occurrence of an event (prevalence and incidence) and the identification of factors that are associated with this event. The main study designs in observational studies are cohort, case-control, and cross-sectional studies, all of which have intrinsic strengths and limitations. These

Effects of bisphenol A on male and couple reproductive health: a review.

Bisphenol A (BPA) is a ubiquitous environmental toxicant with endocrine-disrupting properties and is suspected to affect human reproduction. The objective of this review was to summarize the potential effects of male exposure to BPA on markers of testicular function and couple reproductive outcomes. Five epidemiologic studies on BPA and reproductive hormones all found significant associations with at least one reproductive hormone; however, no consistent relationships were observed across studies. Six epidemiologic studies evaluated the relation between BPA and semen parameters, and although t

Familial confounding and causal inference in child and adolescent neurodevelopment and mental health.

Recent announcements by the US Government linking paracetamol use during pregnancy to autism in offspring highlight the risks of misinterpreting observational research to inform policy; this is a clear example of the principle that association does not equal causation. Unmeasured familial confounding is a common bias in epidemiological studies, whereby shared genetic or environmental factors within families produce spurious associations between risk factors and outcomes. In this Viewpoint, aimed at clinicians from a range of disciplines working with children and young people with neurodevelopm

Applying two approaches to detect unmeasured confounding due to time-varying variables in a self-controlled risk interval design evaluating COVID-19 vaccine safety signals, using myocarditis as a case example.

We test the robustness of the self-controlled risk interval (SCRI) design in a setting where time between doses may introduce time-varying confounding, using both negative control outcomes (NCOs) and quantitative bias analysis (QBA). All vaccinated cases identified from 5 European databases between September 1, 2020, and end of data availability were included. Exposures were doses 1-3 of the Pfizer, Moderna, AstraZeneca, and Janssen COVID-19 vaccines; outcomes were myocarditis and, as the NCO, otitis externa. The SCRI used a 60-day control window and dose-specific 28-day risk windows, stratifi

Oral Health-Head and Neck Cancers: Addressing Confounding Through Negative Control and Quantitative Bias Analyses.

OBJECTIVES: While there are plausible biological explanations for the association between oral health and head and neck cancers (HNC), existing studies have yielded conflicting results. A key concern is that these associations are influenced by mediators, unmeasured risk factors, and biases. To address this, a negative control exposure was used to evaluate whether the associations between oral health and HNC risk could be attributed to unmeasured confounding. Additionally, quantitative bias analysis (QBA) was performed to estimate the extent of non-differential misclassification of exposure.

Sensitivity analyses for average treatment effects when outcome is censored by death in instrumental variable models.

Two problems that arise in making causal inferences for nonmortality outcomes such as bronchopulmonary dysplasia (BPD) are unmeasured confounding and censoring by death, ie, the outcome is observed only when subjects survive. In randomized experiments with noncompliance and no censoring by death, instrumental variable (IV) methods can be used to control for the unmeasured confounding. But, when there is censoring by death, the average causal treatment effect cannot be identified under usual assumptions but can be studied for a specific subpopulation by using sensitivity analysis with additiona

Association of obesity and insulin resistance with asthma and aeroallergen sensitization.

BACKGROUND: It has been hypothesized that obesity and insulin resistance may play a role in the development of asthma and allergy. The aim of the study was to examine the association of obesity and insulin resistance with asthma and aeroallergen sensitization. METHODS: Cross-sectional population-based study of 3609 Danish men and women aged 30-60 years. Aeroallergen sensitization was defined as positive levels of specific IgE against a panel of inhalant allergens. Asthma was defined as self-reported physician diagnosed asthma. Allergic asthma was defined as the presence of both asthma and aer

BMI: new aspects of a classical risk factor for hypertensive disorders in pregnancy.

HDP (hypertensive diseases in pregnancy) are one of the leading causes of maternal and fetal mortality and morbidity. BMI (body mass index) is an established risk factor for pre-eclampsia, but its role in HELLP syndrome is unknown. We therefore investigated BMI as a risk factor in the development of HELLP syndrome. At the beginning of pregnancy, BMI was measured in 1067 women with a history of HDP and 1063 controls. Diagnoses of HDP were classified according to ISSHP (International Society for the Study of Hypertension in Pregnancy) and BMI according to WHO (World Health Organization) criteria
