Sticky eye, tricky diagnosis.

BACKGROUND: Infective conjunctivitis is common and mostly responds well to supportive management and/or empiric antibiotic treatment. At times the differential diagnosis includes more serious conditions that potentially threaten vision. Recurrence and treatment failures are relatively common. OBJECTIVE: To outline the usual clinical appearance of conjunctivitis and its management. To explore the clinical appearance (with the help of case studies) of conditions that have been mistaken for conjunctivitis, and to consider some of the conditions that may coexist that contribute to recurrence and

[Clinical and cytological parallels in inflammatory diseases of the conjunctiva].

The conjunctiva was examined in 52 patients with acute and chronic nonspecific conjunctivitis by impression cytology. Clinical course of conjunctivitis of different origin was correlated to the cytopathological changes in the conjunctival epithelium. The authors emphasize the high clinical value of the method in the diagnosis of inflammatory processes in the conjunctiva, monitoring of treatment, and prediction of the final result of therapy.

Bacterial conjunctivitis and of antibiotics in Dhulikhel Hospital-Kathmandu University Hospital.

BACKGROUND: Acute Infective Conjunctivitis may be due to viral or bacterial infection; though it is usually self limiting, topical antibiotics are often prescribed for rapid recovery. OBJECTIVES: This study aims to find out prevalence of bacterial infection among cases of acute infective conjunctivitis and to determine their in-vitro antibacterial susceptibilities to commonly used antibacterial agents and to find out whether the rampant use of topical antibiotics are at all necessary or justified. METHODS: A prospective analytical study conducted over a period of two years on clinically diag

A new diagnostic index for bacterial conjunctivitis in primary care. A re-derivation study.

BACKGROUND: Eighty per cent of primary care patients with infectious conjunctivitis are treated with antibiotics, although in only 30%, there is a bacterial cause. An accurate diagnostic index to distinguish bacterial from viral conjunctivitis may help reduce unnecessary antibiotics. OBJECTIVES: To validate and, if necessary, improve an existing diagnostic index for bacterial conjunctivitis. METHODS: Non-experimental validation and updating study of an existing diagnostic index in Dutch General Practice. We collected 210 adult patients with incident symptoms suggestive for acute infectious c

[From conjunctivitis to glaucoma. When is a red eye an alarm signal?].

A red eye is mostly a relatively harmless sign of simple conjunctivitis. Viral conjunctivitis may be extremely contagious. If no improvement is seen after ten days of treatment, referral to an ophthalmologist is indicated. Local corticosteroids and local anesthetics must not be prescribed. The patient should also be referred to an ophthalmologist when the red eye is chronic or recurrent, and when a unilateral red eye is associated with severe pain and vomiting (suspicious for acute glaucoma!), or generally when severe pain or impairment of sight, indicative of corneal infiltration or a hypopyo

Outbreak of acute hemorrhagic conjunctivitis in Maharashtra and Gujarat states of India, caused by Coxsackie virus A-24 variant.

Acute hemorrhagic conjunctivitis is associated with enteroviruses. Among these, Coxsackie A-24 variant (CA-24) and Enterovirus-70 (EV-70) are known to cause epidemics and pandemics. An outbreak of acute hemorrhagic conjunctivitis occurred in August-September 2003 in Maharashtra and Gujarat states of India. The present investigation was carried out to determine the viral etiological agent associated with the epidemic. Virus isolates were obtained from 11 eye swabs of conjunctivitis patients using HeLa/ Hep-2 cell lines. The isolates were characterized by serological and mouse pathogenecity test

Eikenella corrodens as a causative agent for neonatal conjunctivitis.

Ophthalmia neonatorum is the most common eye infection in the first month of life. The etiologies have been attributed to chemical, bacterial, and viral agents. Neonatal bacterial conjunctivitis is caused by many species of bacteria, the most important being Neisseria gonorrhoea and Chlamydia trachomatis. Eikenella corrodens is a Gram-negative bacillus. It is a fastidious, slow growing, facultative anaerobic bacterium that may be found as the normal flora of the human mouth, nasopharynx, gut, and genitourinary tract. Its role in causing infections in humans has been recognized in the last 2 de

Per-nasal swabbing as an aid to the diagnosis of chlamydial and adenovirus conjunctivitis.

Two hundred and thirty four patients (adults and babies) with conjunctivitis were investigated by taking eye swabs and in addition by taking per-nasal swabs. Chlamydia trachomatis was isolated from 20 patients and adenovirus from 14 patients. Per-nasal swabbing led to a 53% increase in chlamydia diagnosis and a 27% increase in the diagnosis of adenovirus infection. It is suggested that per-nasal swabbing has an important role to play in detecting chlamydial conjunctivitis which itself may be an indicator for high morbidity in patients and their contacts.

Association of individual and environmental factors exposure with asthma among children: a cross-sectional study in Northern and Southern cities, China.

BACKGROUND: Although previous studies have explored many risk factors associated with childhood asthma worldwide, limited studies have examined whether these risk factors differ in different regions. Our study aims to investigate whether there are differences in the factors influencing childhood asthma in Northern and Southern China and to explore possible reasons for this. METHODS: A cross-sectional analysis of 12,771 urban children aged 4-14 in China, we first applied a group Least Absolute Shrinkage and Selection Operator (LASSO) to influencing factors associated with childhood asthma, usi

Chloramphenicol treatment for acute infective conjunctivitis in children in primary care: a randomised double-blind placebo-controlled trial.

BACKGROUND: One in eight schoolchildren have an episode of acute infective conjunctivitis every year. Standard clinical practice is to prescribe a topical antibiotic, although the evidence to support this practice is scarce. We undertook a randomised double-blind trial to compare the effectiveness of chloramphenicol eye drops with placebo in children with infective conjunctivitis in primary care. METHODS: Our study included 326 children aged 6 months to 12 years with a clinical diagnosis of conjunctivitis who were recruited from 12 general medical practices in the UK. We assigned 163 children

Viral and host proteins involved in picornavirus life cycle.

Picornaviruses cause several diseases, not only in humans but also in various animal hosts. For instance, human enteroviruses can cause hand-foot-and-mouth disease, herpangina, myocarditis, acute flaccid paralysis, acute hemorrhagic conjunctivitis, severe neurological complications, including brainstem encephalitis, meningitis and poliomyelitis, and even death. The interaction between the virus and the host is important for viral replication, virulence and pathogenicity. This article reviews studies of the functions of viral and host factors that are involved in the life cycle of picornavirus.
