A systematic review of couple- or household-targeted interventions for smoking cessation in pregnancy.

BACKGROUND: Tobacco smoking is a major modifiable risk for poor pregnancy outcomes. Pregnant women whose partner smokes are six times more likely to continue smoking. Typically, interventions target pregnant mothers or smoking fathers to reduce second-hand smoke exposure. Evidence reviews advocate for couple-targeted smoking cessation interventions, however no recent review has evaluated the effectiveness of interventions targeting both pregnant women and their partner/household member. We aimed to systematically review and evaluate the effectiveness of smoking cessation interventions targetin

Early Initiation of Sodium-Glucose Cotransporter 2 Inhibitors in Acute Heart Failure: A Systematic Review and Meta-Analysis.

BACKGROUND: Observational studies and small randomized controlled trials have suggested the benefits of early introduction of sodium-glucose cotransporter 2 inhibitors (SGLT2is) in acute heart failure (AHF). However, current evidence on their efficacy and safety in this clinical setting remains limited. METHODS: We performed a systematic review and meta-analysis to assess efficacy/safety of early use of SGLT2is in AHF. PUBMED/EMBASE/Cochrane were searched from inception to May 31, 2024, for randomized controlled trials evaluating outcomes of SGLT2i early initiation in patients with AHF. Effic

Prophylactic antibiotics before cord clamping in cesarean delivery: a systematic review.

INTRODUCTION: The number of clinical trials investigating the optimal timing of prophylactic antibiotics in cesarean section has increased rapidly over the last few years. We conducted a systematic review to inform up-to-date evidence-based guidelines to prevent postpartum infectious morbidity in the mother and rule out any safety issues related to antepartum antibiotic exposure in infants. MATERIAL AND METHODS: Four bibliographic databases were searched for published reports of trials. Ongoing or unpublished studies were searched in Clinicaltrials.gov and the World Health Organization regist

Interventions for preventing tobacco smoking in public places.

BACKGROUND: Environmental tobacco smoke is a health hazard. Reducing exposure to tobacco smoke in public places is a widespread public health goal. There is, however, considerable variation in the extent to which this goal has been achieved in different settings and societies. There is therefore a need to identify effective strategies for reducing tobacco consumption in public places. OBJECTIVES: To evaluate the effectiveness of interventions to reduce tobacco consumption in public places. SEARCH STRATEGY: We searched the Tobacco Addiction Review Group trials register, Medline, EMBASE, HEALT

Hormone and blood pressure relationships in primary aldosteronism.

We used continuous intra-arterial pressure monitoring and hourly venous hormone sampling over 24 hours in 5 patients with primary aldosteronism to study blood pressure and hormone regulation. Three patients were restudied under identical conditions of controlled diet electrolyte intake and body posture 3-7 months after removal of the aldosterone-secreting adrenal tumor. Prior to surgery there was no positive relationship of arterial pressure to renin or to aldosterone. Norepinephrine fluctuations showed positive correlations with arterial pressure but these 2 indices were more closely related

[Effects of rectal indomethacin in the prevention of post-ERCP acute pancreatitis].

UNLABELLED: Recently non-steroidal anti-inflammatory drugs have seemed to reduce the frequency of post-ERCP pancreatitis in some prospective controlled trials, but the results have to be confirmed by further studies. AIM: To evaluate the efficacy of rectally administered indomethacin for the reduction of incidence of post-ERCP pancreatitis. METHOD: A prospective randomized placebo-controlled study was conducted in 228 patients who underwent ERCP. Patients were randomized to receive a suppository containing 100 mg indomethacin or an inert placebo 10 mins before ERCP. Patients were evaluated c

[Hypothermia after perinatal asphyxia reduces the risk of brain damage. But it's too early to recommend the method for routine treatment].

Two randomised controlled trials evaluating intervention with moderate hypothermia after perinatal asphyxia will probably be published this year. The first study, evaluating head cooling with a "cool cap" is already published, and preliminary data from the other study evaluating body cooling earlier have been presented. Both studies indicate that moderate hypothermia, initiated within 6 h after birth, for 72 h reduces mortality and survival with severe brain injury. Several issues must, however, be solved before postasphyctic cooling can be recommended as clinical routine. There are no data on

Strategies for screening for familial hypercholesterolaemia in primary care and other community settings.

BACKGROUND: Familial hypercholesterolaemia is a common inherited condition that is associated with premature cardiovascular disease. The increased cardiovascular morbidity and mortality, resulting from high levels of cholesterol since birth, can be prevented by starting lipid-lowering therapy. However, the majority of patients in the UK and worldwide remain undiagnosed. Established diagnostic criteria in current clinical practice are the Simon-Broome and Dutch Lipid Clinical network criteria and patients are classified as having probable, possible or definite familial hypercholesterolaemia. Systematic reviews in paediatric multiple sclerosis and Creutzfeldt-Jakob disease exemplify shortcomings in methods used to evaluate therapies in rare conditions.

BACKGROUND: Randomized controlled trials (RCTs) are the gold standard design of clinical research to assess interventions. However, RCTs cannot always be applied for practical or ethical reasons. To investigate the current practices in rare diseases, we review evaluations of therapeutic interventions in paediatric multiple sclerosis (MS) and Creutzfeldt-Jakob disease (CJD). In particular, we shed light on the endpoints used, the study designs implemented and the statistical methodologies applied. METHODS: We conducted literature searches to identify relevant primary studies. Data on study des

Changes in Serum C-Reactive Protein Following Nonsurgical Periodontal Therapy: A Systematic Review and Meta-Analysis.

Background: Periodontitis is associated with elevated circulating C-reactive protein (CRP), but the extent to which CRP changes after nonsurgical periodontal therapy remains uncertain. This systematic review and meta-analysis examined within-participant changes in serum CRP and considered controlled evidence separately. Methods: PubMed/MEDLINE, the Cochrane Library, and SciELO were searched through August 2025. For uncontrolled pre-post studies reporting periodontitis-specific paired data, mean CRP reductions in mg/L were pooled using a random-effects model with restricted maximum likelihood a

Preparing for obstetric anaesthesia-An educational randomised controlled trial comparing e-learning to written course material.

BACKGROUND: E-learning is increasingly used in postgraduate medical education. Its numerous benefits include an enhanced learning experience through learning style individualisation, interactive elements, and assessment through tests and quizzes. Current meta-analyses conclude that the overall effectiveness of e-learning is comparable to traditional teaching modalities. However, studies demonstrating its benefits are heterogeneous and of varying quality. This randomised controlled trial aims to investigate whether contemporary e-learning completed prior to a course in obstetric anaesthesia pre
