Chronic pain, craving, and illicit opioid use among patients receiving opioid agonist therapy.

AIMS: In a sample of patients receiving opioid agonist therapy, we evaluated whether having chronic pain was associated with (a) craving for opioids and (b) illicit opioid use. METHODS: In a cross-sectional study of adults on buprenorphine or methadone maintenance recruited from an urban medical center, we examined any craving for opioids (primary dependent variable) in the past week and recent illicit opioid use (secondary dependent variable). Illicit opioid use was defined as a positive urine drug test (UDT) for opiates and chronic pain was defined as bodily pain that had been present for a

Cannabis use vulnerability among socially anxious users: cannabis craving during a social interaction.

Socially anxious individuals appear especially vulnerable to cannabis-related problems. However, the nature of the social anxiety-cannabis relation remains unclear. The present study examined the timing and specificity of cannabis craving in response to a social anxiety induction task among 82 (71% female) cannabis users randomly assigned to either a social interaction or reading task. Participants completed ratings of substance (cannabis, alcohol, cigarette) craving at baseline (prior to being informed of task assignment), before, during, and after task. The Time × Condition interaction was s

Negative moods correlate with craving in female methamphetamine users enrolled in compulsory detoxification.

BACKGROUND: Methamphetamine (METH) use, especially in females, has become a growing public health concern in China. In this study, we aimed to characterize the factors that contributed to drug craving in female METH users under isolated compulsory detoxification. We characterized factors contributing to craving such as duration of detoxification, history of drug use and self-reported mood state. METHODS: Subjects (N=113) undergoing a 1- to 3-year METH detoxification program were recruited from the Zhejiang Compulsory Detoxification Center for Women. The Questionnaire of METH-use Urge (QMU) wa

Repetitive transcranial magnetic stimulation of the superior frontal gyrus modulates craving for cigarettes.

BACKGROUND: Previous functional magnetic resonance imaging studies have shown strong correlations between cue-elicited craving for cigarettes and activation of the superior frontal gyrus (SFG). Repetitive transcranial magnetic stimulation (rTMS) offers a noninvasive means to reversibly affect brain cortical activity, which can be applied to testing hypotheses about the causal role of SFG in modulating craving. METHODS: Fifteen volunteer smokers were recruited to investigate the effects of rTMS on subjective responses to smoking versus neutral cues and to controlled presentations of cigarette

Why two smoking cessation agents work better than one: role of craving suppression.

OBJECTIVE: This research examined why smokers receiving combination medication for smoking cessation are more likely to quit smoking than are those who receive either single agent (monotherapy) or placebo. METHOD: Data were collected from 1,504 current smokers (58.2% women, 83.9% White; mean age = 44.67 years, SD = 11.08) participating in a cessation clinical trial who were randomized to 1 of 6 cessation pharmacotherapy conditions (placebo, nicotine patch, nicotine lozenge, bupropion, nicotine patch + nicotine lozenge, and bupropion + nicotine lozenge). Participants completed ecological momen

Stress and drug-cue-induced craving in opioid-dependent individuals in naltrexone treatment.

BACKGROUND: Naltrexone is a nonaddictive medication that blocks the euphoric effects of opioids. However, naltrexone treatment is associated with high rates of noncompliance and opioid relapse, possibly because it does not reduce stress and protracted withdrawal symptoms during early recovery. Prior clinical and preclinical research has indicated that both stress and drug-cue-related arousal response is associated with craving and vulnerability to relapse in a range of drug-using populations. AIMS: To examine opioid craving and the subjective and cardiovascular response to stress and drug cue

Repeated-testing of place preference expression for evaluation of anti-craving-drug effects.

In addiction research, the conditioned place preference (CPP) paradigm is a widely used animal model of conditioned reward. Usually, CPP development is studied, while only few studies examine CPP expression. In the present study, the suitability of a schedule allowing repeated testing of CPP expression was evaluated. Two groups of rats were either conditioned with cocaine or morphine then the repeated-testing-schedule was applied. This schedule consisted of four repeated applications of a sequence of drug- (i.e. cocaine or morphine), saline- and anti-craving-drug- (i.e. acamprosate, naloxone,

Regional brain metabolic activation during craving elicited by recall of previous drug experiences.

Cocaine cues elicit craving and physiological responses. The cerebral circuits involved in these are poorly understood. The purpose of this study was to assess the relation between regional brain activation and cocaine cue elicited responses. Thirteen right-handed cocaine abusers were scanned with positron emission tomography (PET) and [F-18] fluorodeoxyglucose (FDG) twice; during an interactive interview about neutral themes and during an interactive interview about cocaine themes designed to elicit cocaine craving. In parallel the behavioral (rated from 0: felt nothing to 10: felt extreme) a

Serum prolactin level, craving, and early discharge from treatment in cocaine-dependent patients.

On admission to inpatient treatment, 13 of 33 (39%) cocaine-dependent patients were found to be hyperprolactinemic. Cocaine craving was significantly correlated with cocaine use during the 30 days prior to admission. Serum prolactin level, however, was not correlated with either cocaine use or craving. This lack of intercorrelation argues against the notion that dopamine depletion is an adequate explanation for both craving and hyperprolactinemia following chronic cocaine use. Hyperprolactinemia was associated with a greater likelihood of early discharge from treatment. Gender differences, rec

Cholinergic-Dependent Metaplasticity in the Amygdala-Insula Pathway During Incubation of Cocaine Craving.

The time-dependent increase in drug seeking during abstinence, termed 'incubation of craving' is believed to play a major role in the persistent risks of relapse. This phenomenon is associated with synaptic plasticity changes across brain structures involved in reward processing, executive functions, emotion regulation and inhibitory control. The basolateral amygdala (BLA) and the anterior insular cortex (aIC) are two interconnected structures involved in processing internal and external drug-related cues and are known to play a role in drug craving. However, whether chronic cocaine alters syn

Individualized prediction of heroin cue-induced craving using task-based EEG functional connectivity.

Cue-induced craving is a core driver of addiction and relapse, and its significant heterogeneity represents a major barrier to precision intervention. Currently, there remains a lack of objective, quantifiable, and individualized neurobiological biomarkers. Here, we employed task-based electroencephalography (EEG) to capture the dynamic neural signatures underlying cue-induced craving in patients with heroin use disorder (HUD) and developed an individualized functional connectivity (FC)-based prediction model. We identified β-band power envelope connectivity (PEC) as a reliable biomarker capab

Comparing neuromodulation targets to reduce cigarette craving and withdrawal: a randomized clinical trial.

Cigarette smoking remains the leading preventable cause of death, emphasizing the need for new therapeutics, such as repetitive transcranial magnetic stimulation (TMS). We tested the hypothesis that TMS to three targets would reduce cigarette craving and withdrawal by modulating connectivity within and between three canonical networks in a randomized clinical trial (ClinicalTrials.gov: NCT03827265). Participants (N = 72; DSM-5 tobacco use disorder, ≥1 year of daily smoking) received one session of TMS to hubs of canonical resting-state networks: the dorsolateral prefrontal cortex (dlPFC), supe

The pathways between abstinence self-efficacy, perceived social support and substance use craving.

Low abstinence self-efficacy, lack of social support, especially from family and friends, low self-esteem, feeling lonely, and lack of self-control have been considered as risk factors for substance use disorders, but the pathways of these risk factors on drug craving have not yet been determined. Therefore, the present study aimed to evaluate the association between abstinence self-efficacy and perceived social support on the one hand to drug craving in patients referred to substance use treatment centers on the other hand, with loneliness, self-control, and self-esteem as mediating variables

Craving Induction and Treatment Response in Transcranial Magnetic Stimulation for Tobacco Use Disorder.

BACKGROUND: Repetitive transcranial magnetic stimulation (rTMS) is an emerging treatment for several mental health conditions including substance use disorders. Symptom provocation before rTMS is hypothesized to activate disease-related neurocircuitry and enhance treatment effects. For addiction, craving induction is frequently used before treatment, but its clinical utility requires further exploration. To assess whether craving induction enhances treatment outcomes, we analyzed data from the pivotal multisite trial of deep TMS (DTMS) for tobacco use disorder (TUD). METHODS: A total of 262 p

Ketogenic diet reduces a neurobiological craving signature in inpatients with alcohol use disorder.

BACKGROUND AND AIMS: Increasing evidence suggests that a ketogenic (high-fat, low-carbohydrate) diet (KD) intervention reduces alcohol withdrawal severity and alcohol craving in individuals with alcohol use disorder (AUD) by shifting brain energetics from glucose to ketones. We hypothesized that the KD would reduce a neurobiological craving signature when individuals undergoing alcohol detoxification treatment were exposed to alcohol cues. METHODS: We performed a secondary analysis of functional magnetic resonance data of 33 adults with an AUD who were randomized to a KD (n = 19) or a standar

Frontal White Matter Changes and Craving Recovery in Inpatients With Heroin Use Disorder.

IMPORTANCE: Amidst an unprecedented opioid epidemic, identifying neurobiological correlates of change with medication-assisted treatment of heroin use disorder is imperative. White matter impairments in individuals with heroin use disorder (HUD) have been associated with drug craving, a reliable predictor of treatment outcomes; however, little is known about structural connectivity changes with inpatient treatment and abstinence in individuals with HUD. OBJECTIVE: To assess white matter microstructure and associations with drug craving changes with inpatient treatment in individuals with HUD

Emotional food craving across the eating disorder spectrum: an ecological momentary assessment study.

PURPOSE: Emotional eating during negative emotions might underlie disordered eating behavior (i.e., binge eating and food restriction). Positive emotions, by contrast, seem to promote healthier eating behavior. Naturalistic research on the links between emotions and eating across individuals with binge-eating disorder (BED), bulimia nervosa (BN), binge-purge anorexia nervosa (AN-BP), and restrictive anorexia nervosa (AN-R) is, however, lacking. METHODS: Individuals without eating disorders (comparison group, CG, n = 85), and patients with BED (n = 41), BN (n = 50), AN-BP (n = 26), and AN-R (n

Preclinical evidence to support repurposing everolimus for craving reduction during protracted drug withdrawal.

Cue-elicited drug-craving is a cardinal feature of addiction that intensifies (incubates) during protracted withdrawal. In a rat model, these addiction-related behavioral pathologies are mediated, respectively, by time-dependent increases in PI3K/Akt1 signaling and reduced Group 1 metabotropic glutamate receptor (mGlu) expression, within the ventromedial prefrontal cortex (vmPFC). Herein, we examined the capacity of single oral dosing with everolimus, an FDA-approved inhibitor of the PI3K/Akt effector mTOR, to reduce incubated cocaine-craving and reverse incubation-associated changes in vmPFC
