Severe upper airway obstruction in the tropics requiring intensive care.

BACKGROUND: The clinical profile of severe upper airway obstruction, a challenging acute pediatric emergency, has not been extensively documented in the developing nations of the tropics. METHODS: The diagnostic categories, severity of illness and outcome from 63 episodes of severe upper airway obstruction in 56 children admitted to the Pediatric Intensive Care Unit between January 1994 and December 1999 were reviewed. Outcome variables studied included requirement for ventilation, mortality and complications. Severity of illness was determined with the Pediatric Risk of Mortality (PRISM) II

Flexible bronchoscopy in children at the University of Puerto Rico Pediatric Hospital. Review of 324 consecutive patients.

Described are the findings resulting from 324 bronchoscopies performed with the flexible bronchoscope during the past 8 years at the University of Puerto Rico Pediatric Hospital. Neonates, infants, children and adolescents were included. The most frequent indications for the procedure were stridor, recurrent or persistent pneumonia, atelectasis, recurrent croup and persistent wheezing. An abnormality was detected in 88 per cent of the patients and the finding often resulted in significant modifications of treatment or allowed for reassurance. Complications were minor and there was no mortality

National disease burden of respiratory viruses detected in children by polymerase chain reaction.

BACKGROUND: The epidemiology of community-acquired respiratory viruses at the Children's Hospital of Wisconsin between 1996 and 1998 was examined with molecular [multiplex (M) PCR] and standard virologic methods. METHODS AND RESULTS: A total of 3325 patients with lower respiratory infection (LRI) [bronchiolitis (42%), pneumonia (38%) and croup (12%)] were identified. It is estimated that 545,000 LRI hospitalizations occur each year in the United States in children younger than 18 years old (viral, 428,000; pneumonia, 221,000; bronchiolitis, 222,000; croup, 65,000), including a continued incre

Respiratory syncytial virus--associated lower respiratory diseases in hospitalised pre-school children in Ibadan.

An analysis of the features of lower respiratory diseases (LRD) associated with Respiratory Syncytial Virus (RSV) in 21 children aged 1-23 months, indicated that the clinical features did not differ appreciably from those described for LRD due to other infective agents. It was however, remarkable that none of the children was grossly malnourished, neither was there any death. The final respiratory diagnoses were bronchiolitis alone (6 cases), bronchopneumonia (6 cases), empyema/pleural effusion (3 cases, including one with bronchopneumonia), lobar pneumonia (3 cases, including one with associa

The Relationship Between Eosinophilic Esophagitis and Laryngeal Manifestations: Pathophysiology, Clinical Overlap, and Diagnostic Challenges.

Background: Eosinophilic esophagitis (EoE) is a chronic type 2 immune-mediated esophageal disease defined by esophageal dysfunction and eosinophil-predominant inflammation. Beyond classic gastrointestinal symptoms, patients may present with aerodigestive complaints such as chronic cough, hoarseness, recurrent croup, stridor, vocal fold abnormalities, and subglottic stenosis, creating diagnostic overlap with laryngopharyngeal reflux, vocal fold dysfunction, asthma, allergic rhinitis, and other airway disorders. Methods: This scoping review mapped evidence on the relationship between EoE and lar

Foreign body aspiration in two young infants: The devil in the carpet.

Foreign body aspiration is rare in children below 6 months of age. Very young children presenting with stridor, atypical croup presentation, and not responding accordingly, subglottic foreign body aspiration should be considered. These may not always be visible with bedside flexible endoscopy and may need investigation under anesthesia. We report two cases of devil's thorn aspiration in young infants. These children were left on the floor to play and devils thorn may be a danger lurking as the they have been deposited unknowingly by the shoes people wear and pick up by these young infants.

Don't go barking up the wrong tree: Thinking beyond infection in a toddler with stridor.

A toddler presented to the emergency department with persistent stridor and barky cough for 4 weeks and progressive dysphagia for 1 week. During this time, he had sought medical attention 6 additional times and had been treated for pneumonia, wheezing and croup, receiving antibiotics and several courses of steroids without improvement. On the final presentation, airway imaging did not reveal a foreign body. However, bedside laryngoscopy demonstrated bilateral vocal cord paralysis. Further imaging revealed an intracranial posterior fossa mass which provided the unifying diagnosis for his persis

[Current care guideline: lower respiratory tract infections in children].

All respiratory viruses are capable of causing lower respiratory tract infections. Active testing of influenza viruses during influenza epidemics is recommended. Antitussive medications are ineffective and should not be used. Croup presenting with inspiratory stridor is recommended to be treated with oral corticosteroids and inhaled racemic adrenalin. Corticosteroids and inhaled racemic adrenalin are ineffective for the treatment of bronchiolitis. Inhaled salbutamol administered by a spacer (with a mask) is recommended for wheezy bronchitis. Amoxicillin is recommended for treating pneumonia at

[Diphtheria and Renaissance Spanish physicians].

Along the history of medicine, diphtheria has been the cause of many deadly epidemics. Up to the incorporation of microbiology in medicine and the definition of diphtheria provided by Bretonneau in 1826, this disease was studied with all types of sore throut into the galenic system. Spanish physicians in Renaissance started the way to its systematisation, taking it off the rest of sore throat types, with other laryngeal diseases, as it was known later as "croup", giving it the name of "garrotillo". They were the first to describe some important questions about this disease, as the specificity

Diagnosis of stridor in children.

Stridor is a sign of upper airway obstruction. In children, laryngomalacia is the most common cause of chronic stridor, while croup is the most common cause of acute stridor. Generally, an inspiratory stridor suggests airway obstruction above the glottis while an expiratory stridor is indicative of obstruction in the lower trachea. A biphasic stridor suggests a glottic or subglottic lesion. Laryngeal lesions often result in voice changes. A child with extrinsic airway obstruction usually hyperextends the neck. The airway should be established immediately in children with severe respiratory dis

Update on the utility of corticosteroids in acute pediatric respiratory disorders.

BACKGROUND: Corticosteroids, delivered systemically and by inhalation, are widely used for the treatment of multiple acute respiratory illnesses in children. However, the level of evidence to support the utility of this therapy varies between these different acute respiratory illnesses. OBJECTIVE: To summarize the evidence regarding the utility of corticosteroids in the management of common acute pediatric respiratory conditions and to highlights the controversies regarding their use. METHODS: Literature search of manuscripts describing the evidence regarding the efficacy of corticosteroids

The use of heliox in critical care.

This paper reviews the medical use of helium oxygen mixture in obstructive airway disease in patients with croup, narrow endotracheal tubes (ETTs), respiratory distress syndrome, asthma, bronchiolitis, as well as patients with acute exacerbation of chronic obstructive pulmonary disease (COPD) and acute lung injury. In addition, some other indications of heliox use and some innovative methods of ventilation applied in pediatrics and adults are presented through review of the literature of current decade. Yet, to recommend heliox use seems to require more research based on clinical practice and

Stridor in an 11-year-old child.

Stridor in older children can be due to diverse aetiology that includes infections, anaphylaxis and rarely systemic conditions leading to hypocalcaemia. We report the case of a previously asymptomatic 11-year-old girl who presented to the casualty with stridor due to hypocalcaemia. The aetiological investigations for hypocalcaemia uncovered previously undetected chronic renal failure, possibly due to a rare autosomal recessive condition called nephronophthisis. We report this case to highlight the importance of widening the diagnostic focus for children presenting with stridor, especially when

Steroids in otolaryngology.

Steroids are increasingly being used to treat a wide spectrum of otolaryngological disorders. It is important for ear-nose-throat surgeons to understand the short- and long-term complications associated with steroid use. The aim of this study was to review the role of steroids in common ear-nose-throat disorders. Corticosteroid therapy has been shown to be effective in the management of idiopathic facial nerve palsy, allergic rhinitis, acute sinusitis, sinonasal inflammatory polyposis, and croup. The therapeutic efficacy of steroids in the management of Meniere's disease, sudden idiopathic sen

[Inspiratory stridor in a child with a retropharyngeal abscess instead of the normally expected subglottic laryngitis].

A 6-month-old girl presented with fever, coughing, inspiratory stridor and neck swelling. She was admitted for observation under the suspicion of spasmodic croup combined with a lymphadenitis colli. However, a rapidly progressive respiratory insufficiency developed which was found to be due to a retropharyngeal abscess. The patient was intubated and surgical drainage of the abscess followed. The postoperative clinical progress was uncomplicated. Common presenting signs of a retropharyngeal abscess include: inspiratory stridor, bulging of the pharyngeal wall, limited movement of the neck and pa

[Diphtheria. An illness, which many physicians no longer see in personal experience].

Diphtheria is one of those diseases that the rigorous application of vaccination has rendered very rare in Western countries. However, as a result of being re-introduced from Eastern Europe, it may re-establish itself in the West. The incubation period is two to five days, and in the unprotected, infection may take place via a carrier who himself is not ill. The most feared form is the laryngeal form (croup) and the exotoxin-induced complications: myocarditis with arrhythmias and paresis affecting the muscles of the palate, throat and respiratory system. Once it is clinically suspected, antibi

Bronchial foreign body vs asthma.

Between January 1983 and June 1988, 30 unsuspected foreign bodies were extracted from the respiratory tracts of children. Twelve of them showed no roentgenologic findings and their histories revealed repeated hospitalization due to reactive airways disease (RAD) or croup syndrome. As a test-therapy, theophylline, corticosteroids and epinephrine were administered simultaneously to these patients in appropriate doses. No response was obtained in two patients with subglottic foreign bodies (SFB). In seven of the ten patients with bronchial foreign bodies (BFB), wheezing was either decreased or di

Viruses associated with acute respiratory infections 1961-71.

Influenza-like illness, cold and sore throat was the diagnosis given in over 80% of 5177 acute respiratory illnesses in patients swabbed over a 10-year-period. A pathogenic organism was isolated twice as frequently from patients with a sore throat or an influenza-like illness as from those diagnosed as suffering from croup or laryngitis and bronchitis. A laboratory diagnosis was commoner in school children than in older or younger persons. Most of the organisms isolated were found in association with all types of acute respiratory illness but, with increasing age of the patient, one particular
