BACKGROUND: Immunodeficient children are more prone for invasive cryptococcal infections.
CASE CHARACTERISTICS: A 2-year-old boy with disseminated cryptococcosis was evaluated for underlying immunodeficiency without success.
INTERVENTION/OUTCOME: Child was managed successfully.
MESSAGE: Immunocompetent children with disseminated cryptococcosis can present diagnostic or therapeutic challenge in resource-limited settings.

Discovered more than 100 years ago as a human pathogen, the Cryptococcus neoformans-Cryptococcus gattii (C. neoformans-C. gattii) complex has seen a large global resurgence in its association with clinical disease in the last 30 years. First isolated in fermenting peach juice, and identified as a human pathogen in 1894 in a patient with bone lesions, this environmental pathogen has now found niches in soil, trees, birds, and domestic pets. Cryptococcosis is well recognized as an opportunistic infection and was first noted to be associated with reticuloendothelial cancers in the 1950s. Since th…

Disseminated cryptococcosis is a serious opportunistic fungal infection caused by a yeast-encapsulated fungus of the genus Cryptococcus neoformans. It occurs most often in patients with a significant deficit of cellular immunity and preferentially affects the central nervous system. The skin and the lungs are the most commonly affected sites outside the neuro-subarachnoid location. We report the case of a patient apparently immunocompetent who had a disseminated cryptococcosis. The disease started with the multiple purplish skin lesions, large umbilicated on the face, groin, forearm and leg wi…

Cryptococcosis has emerged as an important public health problem in Africa, Asia and the Americas due to the increasing numbers of persons at risk of this infection and the adaptation of its aetiological agents to new environments. The proper management requires early recognition of Cryptococcus neoformans/C. gattii species complex infection, familiarity with the use and limitations of diagnostic tests and knowledge of the available treatment options. This review will address these issues with the goal of providing sufficient information to suspect, diagnose and treat patients with cryptococco…

Cryptococcosis is a significant invasive fungal infection with noteworthy morbidity and mortality, primarily caused by Cryptococcus neoformans and Cryptococcus gattii. In China, C. neoformans var. grubii (especially molecular type VNI) is the most common variety in the environment and responsible for the majority of cryptococcal infections. C. gattii infections are quite rare in China and the primary molecular type is VGI, which is closely related to C. gattii isolates in Australia. Interestingly, the majority of cryptococcosis in China were reported in the HIV-uninfected patients (especially …

Cryptococcosis is an opportunistic infection caused by Cryptococcus neoformans that typically presents in immunocompromised patients, most commonly in those with human immunodeficiency virus (HIV) infection. It rarely has been described in patients with diabetes mellitus (DM). Defects in the host defense mechanisms due to hyperglycemia predispose diabetic patients to opportunistic infections such as cryptococcosis. We present a rare case of disseminated cryptococcosis in a 48-year-old HIV-negative man with DM.

Primary cutaneous cryptococcosis is characterized by skin lesions confined to one body region, without evidence of simultaneous dissemination. Skin lesions frequently occur in immunocompromised patients. We report a case of primary cutaneous cryptococcosis in an immunocompetent patient affecting genital area successfully treated with oral itraconazole.

This report describes a 25-year-old human immunodeficiency virus- seropositive patient who initially presented with clinical features of a t … of varying shape and size. To the best of our knowledge, this is the first documentation of ovarian cryptococcosis in the English language literature. Despite their rarity in the female genital tract, fungal infections must be considered in the differential diagnosis of patients presenting with pelvic pain of obscure origin and a pelvic mass that is refractory t …

PURPOSE OF REVIEW: Expanding access to antiretroviral treatment has dramatically improved the long-term prognosis of patients with HIV-associated cryptococcal disease who survive the acute infection. However, the incidence and acute mortality of HIV-associated cryptococcal meningitis remain high. In this context, this review summarizes urgently needed recent work aimed at improving the acute management of cryptococcal infection in immunocompromised hosts.
RECENT FINDINGS: Studies have started to optimize antifungal regimens and address the complications of raised cerebrospinal fluid pressure

In two cats, a Persian and a D.S.H. cat, cutaneous Cryptococcosis is described. No signs of involvement of other organ systems nor underlying causes were established. One cat seemed refractory to ketoconazole-therapy. In both cats the skin lesions disappeared within 2-3 months itraconazole treatment. Latex agglutination tests against Cryptococci capsula antigen were performed and stayed longtime elevated while clinical lesions were not present anymore. The test may give an indication about prognosis.

Itraconazole was used in 35 cats with cryptococcosis. Treatment response was determined by comparing clinical signs before, during, and after treatment. It could not be evaluated in 7 cats because they died during treatment from causes unrelated to cryptococcosis. Of the remaining 28 cats, treatment response was classified as success in 16 cats (57%), as improvement in 8 cats (29%), and as a failure in 4 (14%). The failures were due to death or euthanasia from drug toxicity (1 cat), progressive fungal disease (2 cats), and relapse 1 year after treatment (1 cat). The cats that improved did not …

A rare case of cryptococcal infection presenting as a neck mass in an otherwise healthy individual is reported. The mass resulted in lytic d … bridement for complete resolution of the infection. The diagnosis, treatment, and manifestations of cryptococcosis in the head and neck are discussed.
