Update on volume therapy in obstetrics.

Symptomatic hypotension (maternal nausea, vomiting, dizziness and dyspnoea) during spinal anaesthesia for caesarean delivery remains a prevalent clinical problem. Severe and sustained hypotension can lead to impairment of uteroplacental perfusion, foetal hypoxia, acidosis, neonatal depression and further adverse maternal outcomes of unconsciousness, pulmonary aspiration, apnoea and cardiac arrest. Mechanical methods aimed at countering the effects of aortocaval compression do not reliably prevent maternal hypotension. Intravenous crystalloid preloading (given prior to administration of spinal

[Intraoperative volume replacement: crystalloids versus colloids in surgical myocardial revascularization without cardiopulmonary bypass].

BACKGROUND AND OBJECTIVES: The use of crystalloids or colloids for volume replacement in large size surgeries is controversial. The objective of this study was to compare the effects of the intraoperative administration of crystalloids (normal saline - NS) with those of colloids (modified fluid gelatin) for surgical myocardial revascularization (SMR) without cardiopulmonary bypass (CPB). METHODS: Forty patients undergoing SMR without CPB were randomly divided in two similar groups. The first group received modified fluid gelatin and NS and the second group received only NS. Urine output, hemo

Supplemental intravenous crystalloid administration does not reduce the risk of surgical wound infection.

Wound perfusion and oxygenation are important determinants of the development of postoperative wound infections. Supplemental fluid administration significantly increases tissue oxygenation in surrogate wounds in the subcutaneous tissue of the upper arm in perioperative surgical patients. We tested the hypothesis that supplemental fluid administration during and after elective colon resections decreases the incidence of postoperative wound infections. Patients undergoing open colon resection were randomly assigned to small-volume (n = 124, 8 mL.kg(-1).h(-1)) or large-volume (n = 129, 16-18 mL.

[Fluid resuscitation in conditions with disorders of capillary permeability].

Fluid resuscitation remains a cornerstone in the treatment of various types of circulatory failure. Alterations in microvascular permeability are a hallmark of a number of inflammatory conditions including sepsis, septic shock, burns and the acute respiratory distress syndrome. As a result, the loss of plasma fluid into the interstitial space leads to hypovolaemia and tissue hypoperfusion. Administration of large volumes of fluids in often necessary to restore the nutritional blood flow to tissues. This strategy, however, involves the risk of interstitial edema formation, which in turn may fur

Resuscitation in shock associated with burns. Tradition or evidence-based medicine?

OBJECTIVE: To summarize the present standards and guidelines for fluid treatment of shock associated with burns, and to evaluate their scientific support in the literature. DESIGN: Nonsystematic, critical review of the literature regarding the indications for crystalloid and colloid fluid treatment, invasive monitoring and the use of resuscitation end points in shock associated with burns. SUMMARY POINTS: Crystalloid fluid resuscitation of patients with burns is traditionally managed using empirical resuscitation formulae, with the efficacy monitored by vital signs and urinary output The val

Comparison of an ephedrine infusion with crystalloid administration for prevention of hypotension during spinal anesthesia.

This study was designed to compare the efficacy of an ephedrine infusion with crystalloid administration for reducing the incidence of hypotension during spinal anesthesia. Fifty-four ASA I patients scheduled for postpartum tubal ligations under spinal anesthesia were randomly allocated to receive either 15 mL/kg of crystalloid (crystalloid group) or an ephedrine infusion (infusion group). Spinal anesthesia was performed using 70-90 mg of hyperbaric 5% lidocaine. Patients in the infusion group immediately thereafter received an ephedrine infusion at a rate of 5 mg/min for the first 2 min and t

Effect and safety of 4% albumin in the treatment of cardiac surgery patients: study protocol for the randomized, double-blind, clinical ALBICS (ALBumin In Cardiac Surgery) trial.

BACKGROUND: In cardiac surgery with cardiopulmonary bypass (CPB), large amounts of fluids are administered. CPB priming with crystalloid solution causes marked hemodilution and fluid extravasation. Colloid solutions may reduce fluid overload because they have a better volume expansion effect than crystalloids. The European Medicines Agency does not recommend the use of hydroxyethyl starch solutions (HES) due to harmful renal effects. Albumin solution does not impair blood coagulation but the findings on kidney function are conflicting. On the other hand, albumin may reduce endothelial glycocal

Intravenous Fluids in Hospitalized Patients: A Review of Best Practices.

Judicious use of intravenous fluids in hospitalized patients can improve outcomes and prevent adverse events. For many clinical scenarios, balanced crystalloids have some advantages over normal saline, though the differences in outcomes are small. Excessive administration of normal saline can cause metabolic acidosis and kidney injury. For conditions with sodium and/or chloride deficits, normal saline is preferred. Hypertonic saline is used to rapidly correct severe acute hyponatremia, and hypotonic fluids are indicated for replacement of free water deficits. Intravenous albumin is indicated i

Immune response in fluid therapy with crystalloids of different ratios or colloid for rats in haemorrhagic shock.

This study investigated the association between different ratios of balanced salt based-crystalloid (PLASMA SOLUTION-A [CJ HealthCare, Seoul, Korea]) (the ratios of crystalloid for blood loss, 1:1, 1:2 and 1:3) or balanced salt-based colloid (VOLULYTE 6% [Fresenius Kabi, Germany]) (the ratio of colloid for blood loss, 1:1) to restore blood loss and immune response in rats with haemorrhagic shock. About 50% of total estimated blood volume was removed after anaesthesia. The fluid was administered for resuscitation after exsanguination, according to the type of fluid and the ratios of exsanguinat

Large-Volume Crystalloid Fluid Is Associated with Increased Hyaluronan Shedding and Inflammation in a Canine Hemorrhagic Shock Model.

Shedding of the endothelial glycocalyx precedes leukocyte activation and adherence in acute inflammation. Rapid administration of crystalloid or colloid fluids for treating hemorrhagic shock may cause endothelial glycocalyx shedding, thereby increasing inflammation. This study aimed to compare the effect of different fluid treatments in a canine shock model on glycocalyx biomarker, hyaluronan, and inflammatory biomarkers. Greyhound dogs under general anesthesia subject to hemorrhage for 60 min were given 20 mL kg-1 of either fresh whole blood (FWB), hydroxyethyl starch (HES) 130/0.4, 4% succin

Principles of Fluid Management.

Fluid therapy is the most common intervention received by acutely ill hospitalized patients; however, important questions on its optimal use remain. Its prescription should be patient and context specific, with clear indications and contradictions, and have the type, dose, and rate specified. Any fluid therapy, if provided inappropriately, can contribute unnecessary harm to patients. The quantitative toxicity of fluid therapy contributes to worse outcomes; this should prompt greater bedside attention to fluid prescription, fluid balance, development of avoidable complications attributable to f

Volume therapy in trauma and neurotrauma.

Volume therapy in trauma should be directed at the restitution of disordered physiology including volume replacement to re-establishment of tissue perfusion, correction of coagulation deficits and avoidance of fluid overload. Recent literature has emphasised the importance of damage control resuscitation, focussing on the restoration of normal coagulation through increased use of blood products including fresh frozen plasma, platelets and cryoprecipitate. However, once these targets have been met, and in patients not in need of damage control resuscitation, clear fluid volume replacement remai

Adhesion controversies: pelvic pain as a cause of adhesions, crystalloids in preventing them.

OBJECTIVE: To review the clinical data that specifically address the contribution of pelvic adhesive disease to pelvic pain and the use of intraperitoneal crystalloid instillants in the prevention of postoperative pelvic adhesions. STUDY DESIGN: A review of contemporary reports that describe relevant clinical studies. RESULTS: The data on adhesions and pelvic pain suggest that adhesions can cause pelvic pain and that adhesiolysis relieves pain in 60-90% of cases. Adhesions in patients with pelvic pain are associated with restricted motion or expansibility of organs, whereas adhesions in pati
