Clinical and biochemical evaluation of Cushing's syndrome.

Over the past decade, several advances have been made in our understanding of the molecular pathogenesis of pituitary adenomas, and novel diagnostic tests for the diagnosis and differential diagnosis of Cushing's syndrome have been developed. Although established in the late 1970s, measurement of UFC has emerged as the most sensitive and specific test to screen for and confirm the presence of Cushing's syndrome. The combined CRH/DST is potentially a useful adjunct in patients with probable pseudo-Cushing's syndrome and borderline elevated urinary cortisol levels. Improved assays for circulatin

Adrenal incidentaloma: subclinical Cushing's syndrome.

Adrenal incidentalomas present a significant differential diagnostic challenge. All patients with an incidentally discovered adrenal lesion should be carefully considered and re-evaluated to exclude extra-adrenal malignancy. Tumours with hypersecretory syndromes require excision, whilst those tumours more than 6 cm in size, particularly if they exhibit other features of malignancy on computed tomography (CT), magnetic resonance imaging (MRI) or scintigraphy, should also be excised. In all cases screening for phaeochromocytomas should be performed, and in the context of hypertension and spontan

Cushing's syndrome in medullary thyroid carcinoma.

Ectopic ACTH secretion is a rare cause of hypercortisolism. Induced metabolic disturbances are often serious, and the management of such patients may be difficult. We report here our experience with four medullary thyroid carcinoma (MTC) patients with distant metastases in whom an ectopic ACTH syndrome occurred. The clinical presentation was significant by the severity and the rapidity of the hypercortisolism. Diagnosis and follow-up were realized by measurements of plasma cortisol, urinary free cortisol, urinary 17-hydroxycorticosteroid, plasma ACTH, plasma LPH, serum calcitonin and carcino-e

Cushing's syndrome secondary to olfactory neuroblastoma.

A case of olfactory neuroblastoma in a 36-year-old woman who presented with florid Cushing's syndrome is reported. A nasal polyp, which proved to be an olfactory neuroblastoma, was resected. The procedure was followed by complete remission from the endocrinologic abnormalities. Postoperatively, the patient was well for 5 years until recurrence of both Cushing's syndrome and the nasal polyp was noted. Following combined transnasal-transcranial resection of the tumor, which extended into the anterior cranial fossa, the patient again experienced complete remission of Cushing's syndrome. Immunohis

Case report: hypertension in Cushing's syndrome.

Pathogenesis of hypertension in Cushing's syndrome has remained controversial. A 56-year-old Japanese man with Cushing's syndrome due to adrenal carcinoma has been followed up for more than 6 years. During the followup period, left adrenalectomy and hemihepatectomy due to metastatic lesion were performed. Blood pressure, serum cortisol, and urinary excretion of 17-OHCS, as well as other routine biochemicals, have been measured periodically. These data revealed that there is a marked correlation between the levels of blood pressure and serum cortisol or urinary excretion of 17-OHCS. This findin

Pre-Cushing's syndrome: a case report.

A 67-year-old man affected by prostate cancer was incidentally found to have a nodular enlargement of the left adrenal gland without apparent changes in hormonal status. The adrenal mass was found to be scintigraphically active, the radiolabelled compound being concentrated in its context with a consensual suppression of the contralateral uptake. The patient underwent a resection of the adrenal tumor. Histologically and biochemically, the adrenal mass was found to be a non-functioning adenoma. The radioisotopic uptake along with the non-hormonal activity prompted us to call this tumor "Pre-Cus

Cushing's syndrome associated with lung tumors.

The production of corticotropin or corticotropin-releasing factor by non-pituitary, non-adrenal tumors may rarely be associated with an overt clinical expression of hypercortisolism. Recent studies have emphasized the importance of careful thoracic evaluation when such ectopic hormone secretion is suspected.

Bilateral adrenal myelolipomas with Cushing's syndrome.

A 24-year-old woman with non-pituitary dependent Cushing's syndrome was found to have bilateral adrenal myelolipomas at surgery. These benign tumors consist of bone marrow and fat and are uncommon incidental findings that are discovered by computed tomography. Myelolipomas have rarely been associated with Cushing's syndrome, but the cause of the syndrome has not always been defined, because the reports preceded modern diagnostic methods. Careful examination of the adrenal glands from our patient showed a fusion of myelolipoma elements and adrenal cells without distinct adenomas or the typical

Cushing's syndrome: update of diagnosis and management.

Although considerable advances have been made in the understanding of Cushing's syndrome in the recent past, many difficulties persist in the diagnosis and management of patients with hypercortisolism. Precision in the diagnosis of Cushing's syndrome and the differentiation of its various forms have gradually improved, but a substantial number of cases have laboratory or radiologic findings that can be misleading or at least difficult to interpret. Furthermore, other conditions may mimic Cushing's syndrome and add to the diagnostic difficulties. Surgical extirpation of primary adrenal lesions

Aminoglutethimide and metyrapone in the management of Cushing's syndrome.

Fifteen patients with endogenous Cushing's syndrome were treated with metyrapone and/or amino-glutethimide. The duration of the therapy varied from 19 up to 365 days. In patients with Cushing's disease, metyrapone (0.5-2.5 g/day) and aminoglutethimide (0.5-1.5 g/day) seemed equally effective in reducing the cortisol excretion (54 +/- 9 vs 40 +/- 7%). The majority of these patients also showed a clinical improvement. In 1 patient with adrenal adenoma, metyrapone induced a remission. In another patient with adrenocortical cancer, and in 2 with the ectopic ACTH syndrome, the cortisol excretion wa

Cushing's syndrome and bronchial carcinoid tumour.

Adrenal function test results in a girl with Cushing's syndrome and a bronchial carcinoid tumour suggested pituitary dependent hypercortisolism. Resolution after excision of the tumour indicated that her condition had been caused by ectopic adrenocorticotrophic hormone secretion. Conventional tests of adrenal function may be misleading in cases of adrenocorticotrophic hormone secreting bronchial carcinoid tumours.

Hyperprolactinemia in Cushing's disease and Nelson's syndrome.

In an attempt to clarify the pathogenesis of hyperprolactinemia associated with Cushing's disease, PRL secretion was evaluated in 42 patients with hyperadrenocorticism (Cushing's disease, 26; and adrenocortical adenoma, 16) and in 10 patients with Nelson's syndrome. Mild hyperprolactinemia was found in 6 patients with Cushing's disease (23%) and in 5 patients with Nelson's syndrome (50%). In contrast, basal plasma levels of PRL were normal in all of the patients with Cushing's syndrome secondary to adrenocortical adenoma. Injection of TRH unequivocally induced PRL release; however, the respons

Invasive aspergillosis complicating Cushing's syndrome.

Patients with Cushing's syndrome are susceptible to opportunistic infections. Invasive fungal infections in patients with Cushing's syndrome caused by endogenous glucocorticoid excess rarely are reported, and aspergillosis occurring in this setting, to our knowledge, has not been described. Two patients with Cushing's syndrome and notably elevated levels of circulating cortisol had invasive aspergillosis develop. A patient with endogenous hypercortisolism caused by adrenal cortical carcinoma suffered a subarachnoid hemorrhage from rupture of an Aspergillus mycotic aneurysm. The other patient,

Hypoglycemia by adrenocortical carcinoma with Cushing's syndrome.

A case of adrenocortical carcinoma, a 18-year-old female with Cushing's syndrome and later hypoglycemia, was reported. Cushing's syndrome was corroborated by clinical signs of moon face, obesity, hirsutism and amenorrhea as well as by elevated urinary steroid levels. A huge tumor in the right adrenal region weighing 171 g was removed and histologically diagnosed as adrenocortical carcinoma. Shortly after the surgery, urinary levels of steroid excretion became within normal ranges. However, hypoglycemia with elevated levels of urinary steroid appeared in 6 months postoperatively. She died of ma

Plasma testosterone profiles in Cushing's syndrome.

Elevated plasma testosterone levels were found in 8 women with Cushing's disease and oligo-or amenorrhea and/or hirsutism. In 4 men with Cushing's syndrome either due to adrenal hyperplasia or adenoma, plasma testosterone levels were lowered. Three of these 4 men complained of impotence or loss of libodo. Evidence for a major adrenal origin of the elevated testosterone values in the women with Cushing's disease was derived from the parallel suppression of cortisol and testosterone during dexamethasone administration, the testosterone responsiveness to ACTH and its dramatic fall after adrenalec

Alcohol-induced pseudo-Cushing's syndrome.

In 4 patients corticosteroid hypersecretion associated with excessive intake of alcohol led initially to an erroneous diagnosis of Cushing's syndrome. If diagnostic errors are to be avoided, a history of alcohol intake should always be sought in a patient in whom the diagnosis of Cushing's syndrome is entertained. The abnormalities of pseudo-Cushing's syndrome revert to normal on ethanol withdrawal.

Paracardiac lipomatosis in exogenous Cushing's syndrome.

van de Putte, L. B. A., Wagenaar, J. P. M., and San, K. H. (1973).Thorax, 28, 653-656. Paracardiac lipomatosis in exogenous Cushing's syndrome. Histologically proven lipomatosis presenting as paracardiac masses on the chest radiograph is described in a patient with a renal graft. This lesion was caused by the prednisone therapy and diminished after lowering the dosage. Review of 90 renal transplant patients revealed one likely example of this syndrome and four patients had other radiographic abnormalities suggestive of intrathoracic lipomatosis. It is pointed out that intrathoracic lipomatosis
