Cholangiocarcinoma:Unveiling the cold tumor microenvironment and strategies for immune rejuvenation.

Cholangiocarcinoma (CCA) remains a therapeutic challenge due to its "immune cold" phenotype, defined by a desmoplastic fortress and profound T cell exclusion. This review characterizes the multi layered immune escape mechanisms, ranging from defective antigen presentation to the recruitment of suppressive myeloid cells and fibroblasts. We detail how physical barriers and the gut-liver axis conspire to maintain immune invisibility. Moving beyond mechanism, we critically evaluate emerging strategies designed to breach these defenses and reprogram the tumor microenvironment, including stromal tar

Evolutionary bridges: how factors present in amniotic fluid and human milk help mature the gut.

Necrotizing enterocolitis (NEC) continues to be a leading cause of morbidity and mortality in preterm infants. As modern medicine significantly improves the survival of extremely premature infants, the persistence of NEC underscores our limited understanding of its pathogenesis. Due to early delivery, a preterm infant's exposure to amniotic fluid (AF) is abruptly truncated. Replete with bioactive molecules, AF plays an important role in fetal intestinal maturation and preparation for contact with the environment, thus its absence during development of the intestine may contribute to increased

Macrophage colony-stimulating factor as a weapon against cytomegalovirus.

Cytomegalovirus (CMV) infection is one of the severe opportunistic infections faced by severely immunocompromised patients. High viral loads cause tissue-invasive disease and expose to death or various indirect effects. Substantial progress was made in monitoring active infection, and antiviral drugs were developed. However, dose-limiting toxicities and genotypic resistance limit therapeutic efficacy and vaccine development is hampered by the complex biology of the virus. In this issue of EMBO Molecular Medicine, Kandalla et al (2023) suggest an innovative strategy using the cytokine macrophag

Penile implant infection prevention part II: device coatings have changed the game.

The single most important factor in the reduction of penile implant infections has been the infection retardant coatings. Virtually every inflatable penile prosthesis (IPP) sold for the last 15 years in America has been coated and the device infection rate has dropped over 50% to less than 1% in experienced implanter practices. The vast majority of penile implants are contaminated with bacteria at time of surgery and the bacteria live within the implant spaces in a quiescent fashion protected by a biofilm secreted by the organisms that makes them impermeable to antibiotics or the body's defens

Defensins: natural component of human innate immunity.

The widespread use of antibiotics has contributed to a huge increase in the number of resistant bacteria. New classes of drugs are therefore being developed of which defensins are a potential source. Defensins are a group of antimicrobial peptides found in different living organisms, involved in the first line of defense in their innate immune response against pathogens. This review summarizes the results of studies of this family of human antimicrobial peptides (AMPs). There is a special emphasis on describing the entire group and individual peptides, history of their discovery, their functio

Unforeseen risk: leptospirosis and the u.s. Special operations community.

Leptospirosis is a bacterial zoonosis with worldwide distribution and is potentially lethal to those infected if left untreated. Due to the global posture of the U.S. Special Operations community, there is a seemingly perfect overlap of the regions in which they deploy and areas with concurrent endemic leptospirosis infection. Furthermore, Special Operations Forces (SOF) personel are known for working by, with, and through the host nation defense force, who tend to be commonly affected and often serve as a reservoir for this infection. Other risk factors including the usage of infected militar

A clinical approach to the malingering patient.

Malingering presents special challenges to the practicing clinician, including diagnostic uncertainty, the confrontation of potentially criminal conduct, and countertransference and personal reactions. Maintaining a traditional clinical approach to the patient suspected of malingering enables the therapist to draw analogies to other disorders and utilize customary diagnostic and therapeutic skills. Malingering has long been recognized in the military, and has predictably come to the forefront of clinical practice during a time of war. The art in military medicine is to find a way to make our e

Curcumin from Turmeric Alleviates Lipid Metabolism: Network Pharmacology and Cellular Validation.

Curcumin, the primary active compound in turmeric, a widely used medicinal and edible herb in traditional Chinese medicine, exhibits beneficial effects against lipid metabolic dysfunction (LMD). Nevertheless, its comprehensive regulatory network and precise mechanisms remain unclear. In this study, network pharmacology, molecular docking, and in vitro cellular assays were integrated to elucidate the targets and mechanisms by which curcumin acts against LMD. Seven hub targets were identified: AKT1, ALB, INS, IL6, TP53, TNF, and PPARG. Kyoto Encyclopedia of Genes and Genomes analysis revealed si

A critical analysis on ENaC inhibitors, unravelling molecular medicinal insights and promising research roadmap in the treatment of cystic fibrosis.

Airway surface liquid (ASL) homeostasis is crucial for effective mucociliary clearance (MCC) and innate lung defense, both of which are severely compromised in cystic fibrosis (CF). Although CFTR modulators improved disease management, many patients remain ineligible due to mutations that cannot be targeted pharmacologically, highlighting the need for alternative therapeutic strategies. Among these, direct inhibition of the epithelial Na+ channel (ENaC) has emerged as a pharmacologically attractive approach to counteract airway dehydration. Due to adverse effects caused by systemic ENaC inhibi

Beyond Exposure: Standardizing Buprenorphine Training in Pain Medicine Fellowship.

The American medical landscape is currently defined by a profound paradox. While the ongoing opioid crisis, characterized by a rise in synthetic illicit opioids continues to ravage communities, our primary pharmacological defense for both opioid use disorder (OUD) and chronic pain management remains underutilized in the very training programs designed to produce "experts" in the field. Pain medicine fellowship training frequently emphasizes the technical mastery of interventional procedures and non-opioid pharmacologic strategies. While these approaches are critically important, education in m

Medicinal Plants in the Treatment of Peptic Ulcer Disease: A Review.

Peptic Ulcer Disease (PUD) is the most common disorder of the stomach and duodenum, which is associated with Helicobacter pylori infection. PUD occurs due to an imbalance between offensive and defensive factors and Proton Pump Inhibitors (PPI), Nonsteroidal Anti-Inflammatory Drugs (NSAIDs) and antibiotics are frequently used for the treatment. Recently, medicinal plants have emerged as efficacious, safe and widely available alternative therapies for PUD. The aim of this review was to study the medicinal plants and phytochemicals, which have been used for PUD treatment to evaluate the potential

Biosolid-borne tetracyclines and sulfonamides in plants.

Tetracyclines and sulfonamides used in human and animal medicine are released to terrestrial ecosystems from wastewater treatment plants or by direct manure application. The interactions between plants and these antibiotics are numerous and complex, including uptake and accumulation, phytometabolism, toxicity responses, and degradation in the rhizosphere. Uptake and accumulation of antibiotics have been studied in plants such as wheat, maize, potato, vegetables, and ornamentals. Once accumulated in plant tissue, organic contaminants can be metabolized through a sequential process of transforma

Induction of colony-stimulating factor(s) after administration of a traditional Chinese medicine, xiao-chai-hu-tang (Japanese name: shosaiko-to).

Colony stimulating factor (CSF)-rich serum was obtained from mice injected intraperitoneally (ip) with shosaiko-to, a traditional Chinese herbal medicine. Transfer of the CSF-rich serum into naive mice augmented the resistance against Listeria monocytogenes. A dose-dependent induction of CSF was observed in mice given shosaiko-to via intravenous route as well as via intraperitoneal route. Since the serum CSF induction was observed in both lipopolysaccharide (LPS)-responder C3H/He mice and LPS-non-responder C3H/HeJ mice, the effect of shosaiko-to seemed to be independent of possibly contaminati

Molecular Basis of Phenotypic Plasticity: Understory-Induced DNA Methylation and Phytohormone Shifts Optimize Atractylodes lancea (Thunb.) DC Quality.

Atractylodes lancea (Thunb.) DC. (A. lancea) is a crop with medicinal and food properties. Currently, the majority of A. lancea available on the market is cultivated in extensive agricultural fields. This led to phenotypic variation in the plant and a reduction in the content of active ingredients. However, the cultivation of the understory can facilitate the phenotype of A. lancea. This, in turn, aligns with the characteristics of "excellent shape" and "high quality." Nonetheless, there is a paucity of research focusing on the phenotypic changes and molecular mechanisms underlying quality for

NRF2 as a guardian of health: from an ancient survival pathway to a modern therapeutic target.

NRF2 (Nuclear Factor Erythroid 2-Related Factor 2) has traditionally been viewed as a master regulator of the antioxidant response. However, emerging evidence redefines NRF2 as a pleiotropic and evolutionarily conserved systems-level integrator of redox, metabolic, proteostatic, and inflammatory homeostasis, i.e., an evolutionarily conserved guardian of health. From its origins as an adaptive response to rising atmospheric oxygen in early metazoans to its role in orchestrating complex cytoprotective networks in humans, NRF2 exemplifies the biological transition from stress resistance to adapti

Cell phenotypic conversion and fate change during the host defense response, tissue injury and repair.

In the process of host response to insults of serious injury/intense stressor, many types of somatic cells can undergo phenotypic conversion and fate change. The typical feature of cell conversion in this circumstance is to acquire the status of upstream precursors, which commonly accompanies transcriptional alteration to enhance cell proliferation and increase the plasticity of differentiation toward the lineage(s) needed in urgency for defending host against injury as well as maintaining/restoring the integrity of organ tissue function. The extent of cell conversion varies dependent on cell
