The Clinical Relevance of Impulsivity in Substance Use Disorder Treatment: Examining Within-Treatment Changes and Relationship to Psychiatric Symptoms and Cravings in a Large Inpatient Sample.

OBJECTIVE: Impulsivity is a multidimensional construct consistently associated with problematic substance use, but less is known about its relevance to clinical outcomes. The current study examined whether impulsivity changed over the course of addiction treatment and whether the changes were associated with changes in other clinical outcomes. METHOD: Participants were patients in a large inpatient addiction medicine program (N = 817; 71.40% male). Impulsivity was assessed using a self-report measure of delay discounting (i.e., overvaluation of smaller immediate rewards) and the UPPS-P Impuls

Early life stress produces compulsive-like, but not impulsive, behavior in females.

Adverse experiences during childhood are associated with the development of psychiatric disorders later in life. In particular, childhood abuse and neglect are risk factors for addictive disorders, such as substance misuse and pathological gambling. Impulsivity and compulsivity are key features of these disorders. Therefore, we investigated whether childhood adversity might increase vulnerability for addictive disorders through promotion of compulsive and impulsive behaviors. Rats were exposed to a brief, variable childhood or prepubertal stress protocol (Postnatal Days 25-27), and their behav

Impulsivity and sexual compulsivity - a comparison of untreated individuals with CSBD, individuals undergoing long-term treatment for CSBD, and a healthy control group.

BACKGROUND: Compulsive Sexual Behavior Disorder (CSBD) has been classified in the eleventh revision of the International Classification of Diseases (ICD-11) as an impulse control disorder. Impulsivity, risk-taking, and compulsivity play an important role in CSBD. AIMS: The present study aims to examine impulsivity and sexual compulsivity by comparing three groups: untreated individuals with CSBD, individuals undergoing long-term treatment for CSBD, and a healthy control group. The study aims to use questionnaires and computerized tasks measuring risk-taking and delay discounting in order to i

Aberrant striatal firing mediates impulsive decision-making in a mouse model of Parkinson's disease.

Parkinson's disease (PD) is characterized by progressive neurodegeneration, which is associated with motor and non-motor symptoms. Dopamine replacement therapy can remediate motor symptoms, but can also cause impulse control disorder (ICD), characterized by pathological gambling, hypersexuality and/or compulsive shopping. Approximately 14%-40% of all medicated PD patients suffer from ICD. Despite the high prevalence of ICD in medicated PD patients, we know little of its mechanisms, and the main therapeutic strategy is reducing or eliminating dopamine agonist medication. Human imaging studies s

Effects of gender and family history of alcohol dependence on a behavioral task of impulsivity in healthy subjects.

OBJECTIVE: Substance misusers are often considered impulsive, but it is unclear whether impulsivity precedes substance misuse or develops as a consequence of it. Because alcohol dependence has a clear familial component, a study comparing impulsivity in nonaffected individuals who differ with respect to paternal history of alcohol dependence may provide evidence of familial vulnerability to impulsivity. METHOD: 122 healthy individuals participated, none of whom misused alcohol or drugs; 58 were paternal history positive (PHP) and 64 were paternal history negative (PHN) for alcohol dependence.

Cocaine-dependent adults and recreational cocaine users are more likely than controls to choose immediate unsafe sex over delayed safer sex.

Cocaine users have a higher incidence of risky sexual behavior and HIV infection than nonusers. Our aim was to measure whether safer sex discount rates-a measure of the likelihood of having immediate unprotected sex versus waiting to have safer sex-differed between controls and cocaine users of varying severity. Of the 162 individuals included in the primary data analyses, 69 met the Diagnostic and Statistical Manual of Mental Disorders (4th ed., text rev.; DSM-IV-TR) criteria for cocaine dependence, 29 were recreational cocaine users who did not meet the dependence criteria, and 64 were contr

I want it now! Neural correlates of hypersensitivity to immediate reward in hypomania.

BACKGROUND: Hypomania is associated with impulsive decision making and risk taking, characteristics that may arise from hypersensitivity to reward. To date, the neural dynamics underlying intertemporal reward processing have neither been characterized clinically nor in the general population. Taking vulnerability to hypomania as a surrogate model of impulsivity, we utilized event-related potentials to study the neural mechanisms of delay discounting. METHODS: In the first experiment, 32 participants completed an established Two Choice Impulsivity Paradigm in which free choice between immediat

Change in Decision-Making Analysis and Preferences in Old Age.

OBJECTIVES: To test the hypotheses that decision making ability declines in old age and that a higher level of cognitive reserve is associated with a reduced rate of decline. METHODS: As part of an ongoing cohort study, 982 older adults without dementia at study enrollment completed measures of purpose in life and cognitive activity which were used as markers of cognitive reserve. At annual intervals thereafter, they completed 6 tests of decision making. RESULTS: In a factor analysis of baseline decision making scores, 3 measures (financial/health literacy, financial/health decision making,

Resting-State Functional Connectivity and Alcohol Use Disorder: A Case-Control Study.

OBJECTIVE: The authors examined differences in resting-state functional connectivity (rsFC) in the brain between nontreatment-seeking adults with alcohol use disorder (case group) and recreational drinkers without alcohol use disorder (control group) and explored behavioral and psychological mechanisms underlying these differences. METHODS: This case-control study included 140 adults (N=71 with alcohol use disorder and N=69 demographically matched control individuals) who completed a 9-minute resting-state functional MRI scan. About 45% were men, and the mean±SD age was 32.7±10.4 years. Seed-

Neuroeconomics and myopia prevention: A framework for public health intervention.

Aims to outline the application of neuroeconomics in paediatric myopia prevention, offering a framework for designing public health interventions that target decision-making mechanisms such as intertemporal choice, social preferences and neurohormonal regulation. We synthesized theories and empirical evidence from neuroeconomics, psychology and neuroscience. Multimodal approaches-including behavioural tasks (e.g. delay discounting, Ultimatum Game), functional near-infrared spectroscopy (fNIRS), pupillometry and salivary assays-were used to evaluate how interventions influence neurocognitive pr

Transcranial direct current stimulation alters the process of reward processing in children with ADHD: Evidence from cognitive modeling.

BACKGROUND: The ventromedial prefrontal cortex (vmPFC) and dorsolateral prefrontal cortex (dlPFC) are the neural underpinnings of reward processing, which is impaired in individuals with attention deficit hyperactivity disorder (ADHD). In the present study, we aimed to explore the impact of the vmPFC and the dlPFC regulation on reward processing. METHODS: Twenty-six children with ADHD performed the balloon analogue risk-taking task (BART) and chocolate delay discounting task (CDDT) during five different sessions of transcranial direct current stimulation (tDCS), separated by a one-week interv

Comparison of impulsivity and working memory in cocaine addiction and pathological gambling: Implications for cocaine-induced neurotoxicity.

BACKGROUND: The aim of this study was to compare the cognitive performance of cocaine dependent individuals (CDI) with that of pathological gamblers (PG). Cocaine dependence and pathological gambling share neurobiological vulnerabilities related to addiction, but PG are relatively free of the toxic consequences, such that any additional deficits observed in CDI may be interpreted as pertaining to specific drug effects. METHODS: We used a case-control observational design contrasting multiple measures of impulsivity (UPPS-P trait impulsivity, delay discounting) and executive measures of respon

Bright and dark sides of impulsivity: performance of women with high and low trait impulsivity on neuropsychological tasks.

We administered a multidimensional measure of trait impulsivity (the UPPS-P impulsivity scale; Cyders et al., 2007) to a nonclinical sample of 155 individuals and selected 32 participants at the two ends of the trait impulsivity continuum: high (HI, n = 15) and low (LI, n = 17) impulsive women. We further tested these extreme groups on neuropsychological measures of motor impulsivity (go/no-go, d2), delay discounting (Now or Later Questionnaire), reflection impulsivity (Matching Familiar Figures Test), self-regulation (Revised-Strategy Application Test), and decision making (Iowa Gambling Task

Discounting Behavior in Problem Gambling.

Problem gamblers discount delayed rewards more rapidly than do non-gambling controls. Understanding this impulsivity is important for developing treatment options. In this article, we seek to make two contributions: First, we ask which of the currently debated economic models of intertemporal choice (exponential versus hyperbolic versus quasi-hyperbolic) provides the best description of gamblers' discounting behavior. Second, we ask how problem gamblers differ from habitual gamblers and non-gambling controls within the most favored parametrization. Our analysis reveals that the quasi-hyperboli

Exploring risky decision-making dynamics during antidepressant treatment in major depressive disorder: a computational modeling approach.

BACKGROUND: Patients with major depressive disorder (MDD) show greater loss sensitivity and higher delayed discounting rates during decision making, but findings regarding their risk preferences are inconsistent. Computational behavioral modeling provides a promising tool for the investigation of these latent factors underlying the decision-making behaviors. OBJECTIVES: Conduct both cross-sectional and longitudinal analyses to identify patterns and changes in risky decision-making over time, examine correlations between traditional CGT performance indicators and cognitive modeling parameters

Temporal reward discounting in attention-deficit/hyperactivity disorder: the contribution of symptom domains, reward magnitude, and session length.

BACKGROUND: Theoretical models have hypothesized that one core problem in attention-deficit/hyperactivity disorder (ADHD) is abnormal reward processing. Temporal reward discounting (decreases in subjective reward value due to prereward delay) is of interest because of its relation with a key symptom of ADHD-impulsivity. This study investigated 1) whether steep temporal reward discounting (TD) is associated with ADHD-combined type (ADHD-C)/symptoms of hyperactivity-impulsivity specifically; 2) the role of reward magnitude in TD in ADHD-C/participants with symptoms of hyperactivity-impulsivity;

Changes in cocaine consumption are associated with fluctuations in self-reported impulsivity and gambling decision-making.

BACKGROUND: In cross-sectional studies, cocaine users generally display elevated levels of self-reported and cognitive impulsivity. To what extent these impairments are stable v. variable markers of cocaine use disorder, and, thus, are pre-existing or drug-induced, has not yet been systematically investigated. METHOD: We conducted a longitudinal study with cocaine users who changed or maintained their consumption intensity, measuring self-reported impulsivity with the Barratt Impulsiveness Scale (BIS-11), and cognitive impulsivity with the Rapid Visual Processing task (RVP), Iowa Gambling tas

Methylphenidate and fluphenazine, but not amphetamine, differentially affect impulsive choice in spontaneously hypertensive, Wistar-Kyoto and Sprague-Dawley rats.

Impulsivity is one of the core symptoms of attention-deficit/hyperactivity disorder (ADHD). The spontaneously hypertensive rat (SHR), a putative animal model of ADHD, has been used to investigate the neurobiology of impulsivity, although this model has been questioned over concerns that use of Wistar-Kyoto rats (WKY) as a comparison strain may exaggerate effects. The present study compared SHR, WKY and standard, outbred Sprague-Dawley (SD) rats on a delay discounting task where the primary measure was mean adjusted delay (MAD), or the indifference point (in sec) between choice of an immediate

Metabolic state and value-based decision-making in acute and recovered female patients with anorexia nervosa.

BACKGROUND: Patients with anorexia nervosa forgo eating despite emaciation and severe health consequences. Such dysfunctional decision-making might be explained by an excessive level of self-control, alterations in homeostatic and hedonic regulation, or an interplay between these processes. We aimed to understand value-based decision-making in anorexia nervosa and its association with the gut hormone ghrelin. Besides its homeostatic function, ghrelin has been implicated in the hedonic regulation of appetite and reward via the modulation of phasic dopamine signalling. METHODS: In a cross-secti

Cognitive dynamics of intertemporal choice in gambling disorder.

Gambling Disorder (GD) is a behavioral addiction characterized by the persistence of recurrent gambling behaviors despite serious adverse consequences. One of the key features of GD is a marked inability to delay gratification and an overall impairment of decision-making mechanisms. Indeed, in intertemporal choice (ITC) tasks, GD patients usually display a marked tendency to prefer smaller-sooner over larger-later rewards (temporal discounting, TD). However, ITC represents a highly verbal/explicit measure, and as such it might not be sensitive to implicit decision biases. Here we sought to unc
