Incidence Rate and Factors Associated with Delirium and Subsyndromal Delirium in Patients with COVID-19 in an Intensive Care Unit.

BACKGROUND: Delirium subsyndrome (SSD) and delirium (DL) are known complications in the intensive care unit (ICU) and are associated with worse clinical outcomes. The aim of this study was to screen for SSD and DL in individuals with COVID-19 admitted to the ICU and to study the associated factors and clinical outcomes. METHOD: An observational, longitudinal study was conducted in the reference ICU for COVID-19. All admitted individuals with COVID-19 were screened for SSD and DL during their ICU stay using the Intensive Care Delirium Screening Checklist (ICDSC). Individuals with SSD and/or DL…

Underdiagnosis of delirium on admission and prediction of patients who will develop delirium during their inpatient stay: a pilot study.

INTRODUCTION: The study aimed to determine the prevalence and documentation of delirium among the elderly and if the Clock Drawing Test (CDT) can be used to predict which patients had delirium on admission and those who may develop delirium during their stay in acute medical wards. METHODS: A single researcher performed the Mini-Mental State Examination (MMSE) and CDT on admission and discharge of 57 elderly adults at the National University Hospital, Singapore. Delirium was defined as a ≥ 3-point improvement or ≥ 2-point decline in MMSE scores from admission to discharge, where a fall denote…

Delirium superimposed on dementia: a survey of delirium specialists shows a lack of consensus in clinical practice and research studies.

BACKGROUND: Despite advances in delirium knowledge and the publication of best practice guidelines, uncertainties exist regarding assessment of Delirium Superimposed on Dementia (DSD). An international survey of delirium specialists was undertaken to evaluate current practice. METHODS: Invitations to participate in an online survey were distributed by email among members of four international delirium associations with additional publication on their websites. The survey covered the assessment and diagnosis of DSD in clinical practice and research studies. Questions were structured around cur…

[When caregivers become partners : preventing delirium].

Delirium is a common and potentially preventable complication in hospitalized older adults. Early detection requires interprofessional vigilance and caregiver involvement, as relatives are often the first to notice behavioral change. As part of the preparatory phase of an implementation project in an acute geriatric unit, this article presents a straightforward approach: educating caregivers about early signs, triggers, and practical prevention strategies. Delivered as a brief conversation, tailored to the caregiver's level of understanding and documented in the medical record, this educationa…

Acute kidney injury and postoperative delirium.

Postoperative acute kidney injury (AKI) and postoperative delirium (POD) are two of the most common complications after surgery. It is essential to understand their common mechanisms not only to prevent and predict the occurrence of these complications, but also to guide perioperative management and improve surgical outcomes. This review summarizes the recent literature focused on these postoperative complications and explores the relationship between AKI and POD. Numerous clinical studies have used AKI and POD as primary outcomes and have found that AKI and POD often occur simultaneously in t…

[Off Course with Delirium - Medication Challenges].

A 75-year-old patient admitted to geriatric rehabilitation after a fall showed symptoms initially misinterpreted as hyperactive delirium. After he experienced tonic-clonic seizures, it was discovered he had been taking high doses of amitriptyline, which was missed during the medication history. The diagnosis was revised to antidepressant withdrawal syndrome upon review.

Improving Hospital Delirium Screening and Documentation.

BACKGROUND: Appropriate delirium screening and documentation are important for hospitalized patients to ensure positive patient outcomes and patient safety. A preliminary process review at Mayo Clinic in Rochester, Minnesota, identified low adherence (25.9%) to delirium screening and documentation requirements. PURPOSE: The aim of this quality improvement (QI) project was to identify gaps in delirium screening and documentation, determine possible causes of these gaps, and design interventions to help improve the completion of required delirium screening and documentation over one year withou…

Preventing delirium in post-acute care.

Older adults admitted to post-acute care often are frail with multiple comorbidities, making them particularly vulnerable to developing delirium. The purpose of this quality improvement project was to create, implement, and evaluate an NP-led, evidence-based delirium prevention protocol in a skilled nursing facility (SNF). The incidence of new-onset delirium was assessed pre- and postimplementation; following implementation, no patients were diagnosed with new-onset delirium, and nurses and certified nursing assistants reported changes to their practice. Based on project design, the approximat…

Vulval leiomyoma with acute postoperative delirium.

Leiomyomas are benign, well-circumscribed mesenchymal tumours. While uterine leiomyomas are common, vulval leiomyomas are rare. We report the challenging case of vulval leiomyoma in a multiparous woman, who also developed acute postoperative delirium. A 41-year-old multiparous woman presented with a mass in the perineum of one year duration and discomfort in a sitting position since a month ago. Her menstrual history, family history, and personal history were unremarkable. On examination of the vulva, there was a firm mass of size 10 x 8 cm on the left side, involving the labium majus and minu…

Sepsis-Associated Delirium: A Narrative Review.

Delirium is characterized by an acutely altered mental status accompanied by reductions in cognitive function and attention. Delirium in septic patients, termed sepsis-associated delirium (SAD), differs in several specific aspects from the other types of delirium that are typically encountered in intensive care units. Since sepsis and delirium are both closely associated with increased morbidity and mortality, it is important to not only prevent but also promptly diagnose and treat SAD. We herein reviewed the etiology, pathogenesis, risk factors, prevention, diagnosis, treatment, and prognosis…

Factors Affecting Delirium in ICU Patients.

This study examined delirium severity using a delirium screening tool and analyzed the predictors, including pain, acuity, level of consciousness, fall risk, and pain score, to increase understanding of delirium and present foundational data for developing nursing interventions for delirium prevention. This was a retrospective study of 165 patients admitted to three intensive care units (ICUs). the Nursing Delirium Screening Scale (Nu-DESC) was used as a research tool to screen for delirium and measure the degree of delirium. The incidence of delirium in patients was 53.3%, and the average del…

Twenty-Eight-Day-Long Delirium Tremens.

Refractory alcohol withdrawal delirium is uncommon in day-to-day clinical practice. This case report presents a rare case of delirium tremens of unusually long duration that was complicated by the difficulty in tapering down benzodiazepines despite adding midazolam drip as well as phenobarbitone to the management regimen and excluding other possible diagnoses.
