Associations of plasma Neurofilament light chain with sarcopenia and physical capacity in patients with Alzheimer's diseases.

BACKGROUND: Despite the close association of Alzheimer's disease (AD) with muscle decline, the biomarkers of age-related muscle loss, termed sarcopenia, in AD remain elusive. OBJECTIVES: We investigated the plasma neurofilament light (NfL) chain levels as potential biomarkers of sarcopenia in AD patients. DESIGN SETTING, PARTICIPANTS, MEASUREMENTS: We conducted a cross-sectional, observational study on older adults, including controls and patients with AD (n = 38-44/group). We measured the frequency of sarcopenia, body composition, handgrip strength (HGS), gait speed, and short physical perfo

Sex differences in acute health service contact after release from prison in Australia: a data linkage study.

OBJECTIVES: Women released from prison typically experience worse health outcomes than their male counterparts. We examined sex differences in the patterns, characteristics, and predictors of acute health service contact (AHSC) (i.e. ambulance and/or emergency department use) after release from prison. STUDY DESIGN: Data linkage study. METHODS: Baseline survey data from 1307 adults (21% women) within six weeks of expected release from prisons in Queensland, Australia (2008-2010) were linked prospectively with state-wide ambulance and emergency department, correctional, mental health, and dea

Pulmonary dysfunction in children with Duchenne muscular dystrophy may occur earlier than we thought - analysis using novel methodology based on z-scores.

INTRODUCTION: Respiratory status is one of the main factors affecting the length of survival in patients with Duchenne muscular dystrophy (DMD) - the most common, severe, progressive muscular dystrophy. Aim of the study was (1) to assess pulmonary function in DMD patients using the z-score method and (2) to identify factors affecting it, irrespective of disease progression. MATERIAL AND METHODS: We evaluated 55 boys (aged 5-18 years) with DMD. The spirometry was performed with: forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1), peak expiratory flow (PEF) analysis as absolute

Current and emerging treatment strategies for Duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD) is the most common form of muscular dystrophy in childhood. It is caused by mutations of the DMD gene, leading to progressive muscle weakness, loss of independent ambulation by early teens, and premature death due to cardiorespiratory complications. The diagnosis can usually be made after careful review of the history and examination of affected boys presenting with developmental delay, proximal weakness, and elevated serum creatine kinase, plus confirmation by muscle biopsy or genetic testing. Precise characterization of the DMD mutation is important for gene…

Effects of olanzapine on anxiety-related behaviour in male and female rats assessed after 21-24 and 42-45 days of chronic treatment.

Olanzapine can decrease anxiety and impair memory, but there is limited information about length of treatment or sex differences in its effectiveness. Therefore, effects of 21-24 and 41-45 days of treatment and sex differences on anxiety-related behaviour and spatial memory were assessed in PVG/c (PVG/c is the internationally recognised way of referring to this rat strain) male and female rats. From 70 days after birth (PND70), all rats received no drug or 6, 11 or 15 mg/kg/day olanzapine via drinking water. From PND91, they were given four daily tests in an open field, light-dark box, zero ma

Individual differences in the forced swimming test and the effect of environmental enrichment: searching for an interaction.

Animals with low and high immobility in the forced swimming test (FST) differ in a number of neurobehavioral factors. A growing body of evidence suggests that the exposure to enriched environments mediates a number of changes in the brain. Therefore, we studied if animals' individuality can somehow modulate the response to environmental stimuli. Male rats were classified according to their immobility time scores in the FST test session as animals with low, medium or high immobility. Then, rats from groups with low and high immobility were randomly distributed in two groups to be reared in diff

Multimodal therapy as an algorithm to limb salvage in diabetic patients with large heel ulcers.

In many series of diabetic foot ulcer care, heel ulcers greater than 4 cm across have been identified as an independent predictor of limb loss. Therefore, we set out to pursue the most aggressive limb salvage algorithm in patients with heel ulcers greater than 4 cm in diameter. Over 5 years, we identified 21 patients, 39-84 years of age, all with diabetes mellitus, with heel ulcers greater than 4 cm in diameter and had magnetic resonance imaging or bone scan evidence of osteomyelitis. Seven of the 21 patients had end-stage renal disease defined as being haemodialysis dependent. All patients ha

Impact of Early Mobilization and Rehabilitation on Functional Outcomes Following Craniocerebral Gunshot Injuries.

Craniocerebral gunshot injury (CGI) is a severe form of traumatic brain injury associated with profound neurological, cognitive, and functional impairment, often requiring prolonged rehabilitation. We report the case of a 17-year-old boy with a penetrating CGI who presented with a Glasgow Coma Scale score of 3/15, intracranial hemorrhage, a depressed frontal skull fracture, retained bullet fragments, and complete dependence in activities of daily living. Following neurosurgical stabilization, the patient underwent a structured, multidisciplinary neurorehabilitation program over 12 weeks (48 se

[Long terme oxygen therapy in chronic respiratory diseases].

LONG TERM OXYGEN THERAPY IN CHRONIC RESPIRATORY DISEASES. Survival of severe chronic respiratory failure with chronic obstructive pulmonary disease (COPD) is improved by long-term oxygen therapy. Other benefits exist for COPD and other causes of chronic respiratory failure. The indications for this restrictive (more 15 hours per day) treatment require measurements of arterial blood gases in adults. Several actors are involved: the specialist for the prescription, the service provider for supplying and maintaining the equipment, the patient and his entourage, the referring doctor to ensure that

X-Linked Myotubular Myopathy: A Novel Mutation Expanding the Genotypic Spectrum of a Phenotypically Heterogeneous Myopathy.

X-linked myotubular myopathy (XLMTM), a centronuclear congenital myopathy secondary to pathogenic variants in the MTM1 gene encoding myotubularin, is typically recognized for its classic and severe phenotype which includes neonatal hypotonia, severe muscle weakness, long-term ventilator dependence, markedly delayed gross motor milestones with inability to independently ambulate, and a high neonatal and childhood mortality. However, milder congenital forms of the condition and other phenotypes are recognized. We describe a 6-year-old boy with a mild XLMTM phenotype with independent gait and no

The clinical presentation caused by truncating CHD8 variants.

Variants in the chromodomain helicase DNA-binding protein 8 (CHD8) have been associated with intellectual disability (ID), autism spectrum disorders (ASDs) and overgrowth and CHD8 is one of the causative genes for OGID (overgrowth and ID). We investigated 25 individuals with CHD8 protein truncating variants (PTVs), including 10 previously unreported patients and found a male to female ratio of 2.7:1 (19:7) and a pattern of common features: macrocephaly (62.5%), tall stature (47%), developmental delay and/or intellectual disability (81%), ASDs (84%), sleep difficulties (50%), gastrointestinal p

Treatment for Acute Stage Complex Regional Pain Syndrome Type II with Polydeoxyribonucleotide Injection.

Complex regional pain syndrome (CRPS) type II is a syndrome that develops after nerve injury. Symptoms may be severe, and vary depending on the degree of sympathetic nerve involvement. As yet, there is no satisfactory treatment. We report the case of a female patient who had an L5 left transverse process fracture and an S2 body fracture, who developed symptoms of CRPS type II in her left lower leg that were aggravated during ambulation in spite of absolute bed rest for one month after the trauma. Several treatments, including bed rest, medication, and numerous nerve blocks were attempted, but

[Epidural obstetrical analgesia--a friend or an enemy?].

Epidural analgesia in labour using local anaesthetics is very efficient, but the technique has been associated with undesired motor block and an increased use of instrumental deliveries. A new epidural analgesia technique, using a combination of low-dose local anaesthetics and opioids (fentanyl and sufentanil), has recently come into practice. The new epidural technique provides excellent analgesia, minimises motor block, allows the mother to ambulate, and minimises the need for instrumental delivery. Furthermore, it constitutes a good alternative to parenteral pethidine. We are of the opinion

Orthopedic rehabilitation of the stroke patient.

Rehabilitation of hemiplegic patients begins with setting reasonable functional goals and a treatment plan to reach them. During the initial illness an effort is made to begin range of motion exercising and positioning to prevent contractures. Transfer from bed to chair is recommended as soon as the patient's general condition permits.Upper extremity function depends on sensory and motor function as well as visual and central cerebral impairment. Spastic symptomatic contracture of the shoulder must be prevented by adequate orthopedic management of any musculoskeletal problems such as arthritis

Using a divided-attention stepping accuracy task to improve balance and functional outcomes in an individual with incomplete spinal cord injury: A case report.

A spinal cord injury (SCI) frequently results in impaired balance, endurance, and strength with subsequent limitations in functional mobility and community participation. The purpose of this case report was to implement a training program for an individual with a chronic incomplete SCI using a novel divided-attention stepping accuracy task (DASAT) to determine if improvements could be made in impairments, activities, and participation. The client was a 51-year-old male with a motor incomplete C4 SCI sustained 4 years prior. He presented with decreased quality of life (QOL) and functional indep

[Links between depressive disorders and dependent personality disorders: The important effect of locus of control].

CONTEXT: Empirical researches have proved that there are powerful correlations between dependent personality and depression. Different hypotheses were described to conceptualize links between these two entities. The dysfunction of attributive style seems to be linked to dependency and to depression. Interpersonal dependency can be considered to be a mode of adaptation to the external direction of the locus of control. The self-esteem so subjected to the climate of social interactions can lead, by the discontinuity of its protective relations, to the depression. In a coordinated model, this stu

Agreement of aspiration tests using barium videofluoroscopy, salivagram, and milk scan in children with cerebral palsy.

To study the agreement between three tests for aspiration, barium videofluoroscopy, salivagram, and milk scan we studied 63 children with severe non-ambulant spastic quadriplegic cerebral palsy (CP) aged 14 months to 16 years (32 males, 31 females). The salivagram was most frequently positive (56%, 95% confidence interval 43 to 68%); the next most frequently positive was barium videofluoroscopy when aspiration was defined as the presence of either laryngeal penetration of material or frank aspiration (39%, 95% confidence interval 26 to 53%). The milk scan was rarely positive (6%, 95% confidenc
