Impact of Deprescribing Medications on Muscle Strength and Muscle Mass in Older Patients with Sarcopenia after Stroke.

Polypharmacy is prevalent among older adults with sarcopenia and may negatively impact clinical outcomes. Although sarcopenia is associated with polypharmacy, the effect of deprescribing on muscle health in hospitalized patients with sarcopenia remains poorly understood. This study aimed to investigate the association between deprescribing during hospitalization and muscle strength and mass at discharge in sarcopenic patients after stroke. This study retrospectively analyzed data from a cohort of sarcopenic patients aged ≥65 years admitted to a convalescent rehabilitation ward after stroke. Sa

Comparing Fourteen Behavioral Science Electronic Health Record Deprescribing Tools in Older Adults: NUDGE-EHR Adaptive Trial.

BACKGROUND: Interventions to reduce prescribing of high-risk medications like benzodiazepines and sedative hypnotics to older adults have had modest success. Electronic health record (EHR)-based alerts, especially those incorporating behavioral science, can improve prescribing but are under-evaluated for deprescribing. METHODS: In Novel Uses of adaptive Designs to Guide provider Engagement in Electronic Health Records (NUDGE-EHR), we randomized primary care providers (PCPs) in a large healthcare system to usual care (no tool), 1 of 14 EHR tools (alerts with or without in-basket messages) desi

Barriers to and Enablers of Supporting Deprescribing Benzodiazepines in Older Adults: A Survey of European Nonphysician Healthcare Professionals.

Although physicians are primarily responsible for Benzodiazepine Receptor Agonist (BZRA) deprescribing, nonphysician healthcare professionals (HCPs) can support deprescribing. This study explored barriers to and enablers of BZRA deprescribing among nonphysician HCPs. We surveyed 258 HCPs (63.2% nurses) working in hospital settings across six European countries using a questionnaire based on the Theoretical Domain Framework (TDF). Logistic regression assessed associations between TDF domains and both intentions to support and routine engagement in BZRA deprescribing. Major barriers (TDF items w

Feasibility of a theory-based intervention towards benzodiazepine deprescribing in Belgian nursing homes: protocol of the END-IT NH cluster-randomised controlled trial.

INTRODUCTION: Despite several calls to deprescribe benzodiazepine receptor agonists (BZRA) in older adults, their use among nursing home residents (NHRs) remains high. Therefore, we developed an intervention targeting general practitioners' and healthcare professionals' behaviours regarding BZRA deprescribing in nursing homes (NHs): The END-IT NH (bENzodiazepines Deprescribing InTerventions Nursing homes) 6-component intervention. Before moving on to a large-scale effectiveness and cost-effectiveness evaluation, this feasibility study aims at: (1) assessing the feasibility of the intervention

Glucocorticoid discontinuation in patients with early rheumatoid and undifferentiated arthritis: a post-hoc analysis of the BeSt and IMPROVED studies.

OBJECTIVES: To evaluate the success rate of glucocorticoid discontinuation and to study which factors are associated with successful discontinuation. METHODS: Data from two treat-to-target studies, BeSt (target Disease Activity Score (DAS) ≤2.4) and IMPROVED (target DAS <1.6), were evaluated for all patients initially treated with a tapered high dose of prednisone with conventional synthetic disease-modifying antirheumatic drugs. Prednisone was discontinued when DAS ≤2.4 was maintained for 28 weeks in BeSt and as soon as DAS was <1.6 in IMPROVED. Discontinuation was considered successful if t

Deprescribing zolpidem reduces falls and fractures in patients with cirrhosis.

BACKGROUND & AIMS: Benzodiazepines are associated with an increased risk of harm in patients with cirrhosis. However, stopping benzodiazepines must be done with care to avoid withdrawal or other unintended consequences. The impact of deprescribing on patients with cirrhosis is unknown. METHODS: We emulated a hypothetical 3-year trial of benzodiazepine deprescription among Medicare enrollees with compensated cirrhosis who lacked other life-limiting diagnoses. All received continuous benzodiazepine prescriptions for the 6-months prior to their diagnosis of cirrhosis. During a 90-day landmark pe

Clinical decision support systems for polypharmacy optimization in older patients: a narrative review.

PURPOSE: Multimorbidity and polypharmacy are increasingly prevalent in the older population and are associated with a higher risk of potentially inappropriate medications (PIM), drug-drug interactions (DDI), and adverse drug reactions (ADR). Although medication review (MR) and deprescribing are effective strategies, their manual implementation can be complex, time-consuming, and prone to clinical variability. Clinical Decision Support Systems (CDSS) offer advanced digital solutions to optimize polypharmacy by analyzing multidimensional clinical data and generating personalized recommendations.

Opioid Deprescribing Rates and Predictors Among Medicare Enrollees With Cirrhosis and Chronic Pain: Retrospective Cohort Study.

Older adults with cirrhosis commonly experience chronic noncancer pain managed with chronic opioid therapy. Current guidelines recommend opioid deprescribing for high-risk populations, including in cirrhosis, but data on tapering and discontinuation are scarce. We described opioid discontinuation rates and identified predictors of tapering or discontinuation. This retrospective cohort study of Medicare fee-for-service beneficiaries (N = 800,763) ≥ 65 years with continuous opioid use for ≥ 90 days. The primary outcome was opioid discontinuation (i.e., a gap in refills > 30 days). The secondary

Policies for Deprescribing: An International Scan of Intended and Unintended Outcomes of Limiting Sedative-Hypnotic Use in Community-Dwelling Older Adults.

Policies have been put in place internationally to reduce the overuse of certain medications that have a high risk of harm, such as sedative-hypnotic drugs for insomnia or opioids for chronic non-cancer pain. We explore and compare the outcomes of policies aimed at deprescribing sedative-hypnotic medication in community-dwelling older adults. Prescription monitoring policies led to the highest rate of discontinuation but triggered inappropriate substitutions. Financial deterrents through insurance scheme delistings increased patient out-of-pocket spending and had minimal impact. Pay-for-perfor

Management of Aneurysmal Subarachnoid Hemorrhage Patients with Antiplatelet Use Before the Initial Hemorrhage: An International Survey.

BACKGROUND: The case fatality in aneurysmal subarachnoid hemorrhage (aSAH) is 50% because of the initial hemorrhage or subsequent complications, such as aneurysmal rebleed or delayed cerebral ischemia. One factor that might influence the initial brain damage or subsequent complications is the use of antiplatelet medication before the initial hemorrhage. The goal of this survey was to assess the different management options of patients with aSAH with antiplatelet use before the initial hemorrhage. METHODS: An anonymous survey of 11 multiple choice questions about management of patients with aS

Deprescribing versus continuation of chronic proton pump inhibitor use in adults.

BACKGROUND: Proton pump inhibitors (PPIs) are a class of medications that reduce acid secretion and are used for treating many conditions such as gastroesophageal reflux disease (GERD), dyspepsia, reflux esophagitis, peptic ulcer disease, and hypersecretory conditions (e.g. Zollinger-Ellison syndrome), and as part of the eradication therapy for Helicobacter pylori bacteria. However, approximately 25% to 70% of people are prescribed a PPI inappropriately. Chronic PPI use without reassessment contributes to polypharmacy and puts people at risk of experiencing drug interactions and adverse events
