Rupture of splenic abscess through the diaphragm.

Splenic abscess is an uncommon disease that has a high mortality rate if not treated surgically. With the aid of new diagnostic tools, such as ultrasonography, scanning and computerized axial tomography, it is possible to diagnose the entity antemortem. Even though it often has a nontypical clinical course, it may overlap other systemic diseases. We describe an unusual case of splenic abscess with rare complications, the operation and the outcome.

Diaphragm activation with intramuscular stimulation in dogs.

We studied in 10 supine anesthetized dogs diaphragm contraction produced by electrical activation with intramuscular electrodes surgically implanted in the ventral surface of the diaphragm and compared this with activation of the ipsilateral phrenic nerve (C5, 6, and 7) before it entered the thorax. Repetitive 40-Hz pulse trains with supramaximal current stimulus were used after hyperventilation of the animals to apnea. A single intramuscular electrode within 1 to 2 cm of the site of phrenic nerve entry into the diaphragm produced a mean transdiaphragmatic pressure of 12.0 cm H2O +/- 0.97 SE a…

[Traumatic lesions of the diaphragm (personal experience)].

From 1971 to 1982, 126 blunt thoraco-abdominal trauma were seen: in 9 cases (7,1%), diaphragmatic rupture was present. Automobile accidents were the most frequent cause of diaphragmatic rupture and, in only one case, the injury was single. Diaphragmatic rupture was suspected preoperatively in two patients, who had positive physical and radiologic chest findings. The lesion was seen intraoperatively in the other patients, who underwent surgical operation because of severe intra-abdominal injuries. In the author's experience, roentgenologic examinations and adjuntive studies were not important f…

[Herniations and prolapses of the diaphragm (author's transl)].

Diseases and changes of the diaphragmal region should be investigated radiologically. Computed tomography will give additional information. Special attention should be paid to patients after traumatic injuries of breast and body. 95% of which are sliding gastro-esophageal herniations.

Mucosal diaphragms of the gastric antrum in adults.

Fifty-six adult patients with antral mucosal diaphragms have been reported in the literature. These abnormalities are located at the antrum of the stomach at variable distances from the pylorus and are composed of mucosa which encloses a membrane of submucosa without involvement of muscularis or serosa. They may be congenital in origin or secondary to chronic peptic ulcer disease. Symptoms are episodic and obstructive in nature and imply a diaphragmatic aperture of less than 1 centimeter. The diagnosis relies on the history, contrast roentgenologic studies and endoscopic findings. Unfortunatel…

Contractile properties and fatigue of the diaphragm in man.

We studied the pressure developed by the diaphragm in response to stimulation of the phrenic nerve in the neck, in three normal men. When the phrenic was electrically stimulated at increasing frequencies the diaphragm responded by increasing transdiaphragmatic pressure to give a frequency-pressure curve similar to the frequency-force curve for other skeletal muscles. The subjects than breathed through an inspiratory resistance for as long as possible and the frequency-pressure curve was repeated. It was found that the diaphragm developed low frequency fatigue, in the same way as previously des…

Liver scans in evaluation of lesions of the diaphragm.

Three patients had chest radiographs suggesting pathology associated with the right hemidiaphragm. The patients' histories included trauma in one case, recently diagnosed leukemia in a second, and Hodgkin's disease in the third. In all three cases, the Tc-99m-sulfur colloid liver scan provided a noninvasive identification of the nature of the diaphragmatic densities seen on the chest radiographs, thus affecting patient management.

Antral mucosal diaphragms in infants and children.

The antral mucosal diaphragm in infants and children has been reported in the literature in 48 patients. We have encountered antral mucosal diaphragms in 6 children. Only 3 of these 6 patients required surgery. These 6 cases and a review of the published cases suggest that patients fall into two groups: the first group usually requires surgery because of recurrent clinical signs suggesting a partial gastric outlet obstruction. The second group have the diaphragm detected incidentally or have unimpressive clinical findings. This group can be followed without surgery. In all cases, the diagnosis…

[Significance of electromyography of the diaphragm in respiratory physiopathology].

Adequate electromyographic method are available in order to record the electrical activity of the vertebral (oesophageal surface electrodes) and the right or left costal (concentric needle electrodes) parts of the diaphragm and in order to stabilize the EMG's base line (rejection of the electrical movement's artefacts by means of RC filters). The electromyographic patterns of the agonistic and antagonistic activity of the diaphragm in normal subjects have been clearly defined in different circumstances such as quiet ventilation and hyperventilation. Disturbances of the ventilatory diaphragmati…

[Traumatic rupture of the diaphragm (author's transl)].

Traumatic rupture of the diaphragm is diagnosed during the acute stage in only 50% of the cases. The reasons for this failure are that in many of these cases clinical symptoms of damage to abdominal organs or of injuries to the limbs predominate. Traumatic rupture of the diaphragm is usually the result of an indirect trauma. Clinical signs of extensive prolapse of viscera into the thoracic cavity are: dyspnoea, tachycardia, cyanosis or intestinal sounds heard over the thorax during auscultation. Once the diagnosis has been established surgical repair should follow. During the acute stage lapar…

The "Flat Diaphragm": Does the Degree of Curvature of the Diaphragm on Postoperative X-Ray Predict Congenital Diaphragmatic Hernia Recurrence?

PURPOSE: The appearance of the diaphragmatic curvature and the rib insertion level of the diaphragm on postoperative chest X-ray (CXR) may predict recurrence. Our purpose was to examine the relationship between the curvature of the diaphragm on postoperative CXR and recurrence. METHODS: We performed a retrospective review of left-sided, Bochdalek congenital diaphragmatic hernia (CDH) surgical repairs from 2004 to 2015 at a single institution. We developed a tool to measure the flatness of the diaphragm on postoperative CXR, termed the diaphragmatic curvature index (τ). The primary outcome of …
